Book excerpt: Oral antibiotics are often used prophylactically in patients undergoing dental procedures, with the aim of preventing oral bacteria from entering the bloodstream, and causing serious complications due to infection among susceptible patients. Among the conditions for which this measure has been used prophylactically are infective endocarditis in patients who have had cardiac implants, and prosthetic joint infection. While this practice has been commonplace, it has been carried out with little supporting clinical evidence. Given the frequency with which dental procedures are performed, the potential for antimicrobial resistance, and potential cost, clinical effectiveness should be demonstrated. However, current practice is largely based on a theoretical basis for prophylaxis, and because some patient groups are considered to be at high risk for serious complications in the event of infection. The present review was conducted to explore the existence of recent clinical evidence for the prophylactic use of oral antibiotics during dental procedures in patients with cardiac or orthopedic implants, and to review current evidence-based guideline recommendations for these two patient groups.
