In support of these efforts, some employers have made changes in their policies and facilities to support physical activity and healthier eating, and some employers connect with community resources for health education, health fairs, and other services. This diverse array of activities most typically has been planned, managed, and assessed - to the extent they exist in the workplace at all - by different, often uncoordinated departments within the business entity. Some employers have reconceptualized their safety, prevention, and promotion initiatives and attempted to bring them together into a coherent whole. The National Institute for Occupational Safety and Health (NIOSH) has supported this integration, defining Total Worker Health as "a strategy integrating occupational safety and health protection with health promotion to prevent worker injury and illness and to advance health and well-being." In May 2014, with support from NIOSH, the Institute of Medicine organized a workshop on Total Worker Health. Rather than a review of published literature, this workshop sought input from a wide variety of on-the-ground stakeholders regarding their experiences with integrating occupational safety and health protection with health promotion in the workplace. Promising and Best Practices in Total Worker Health is the summary of the discussions and presentations of the event. This report identifies prevalent and best practices in programs that integrate occupational safety and health protection with health promotion in small, medium, and large workplaces; employer and employee associations; academia; government agencies; and other stakeholder groups.

Due to the amount of time spent at the workplace and the impact of work on health over an individual’s life course, the workplace has evolved as an important arena for population health promotion. Risk factors within the physical and psychosocial working environment, as well as inadequate organizational support, are associated with increasing work-related health problems, which result in psychosocial and economic implications for the individual, the family, the organization and the society. Recent estimates revealed an increasing level of sickness absence due to work related factors, among others. In recognition of the importance of worker health and a healthy working life, but also in line with numerous occupational health goals, many organisations set aside significant amounts of financial resources annually to promote work well-being. However, studies have shown that despite this positive disposition among employers, both employee participation and the impact of such programs remain minimal. According to the Ottawa declaration for health promotion and the Luxembourg declaration for Workplace Health Promotion (WHP), WHP should be strategic. It is recommended that WHP be conducted in a systematic and continuous process of needs analysis, priority setting, planning, implementation and evaluation. Unfortunately, available studies show that many companies have policies currently in place but lack knowledge regarding proper implementation and evaluation. The foregoing phenomenon raises questions regarding the level of knowledge of and attitudes towards WHP among people in management positions. The goal is to determine how OSHA could best use its limited resources to improve worker safety and health in light of incentives already created through the labor market, state workers' compensation insurance programs, and the legal system. After an introduction, the next section develops the general economic model of production and the workplace when there are inevitable risks to safety and health. The third and fourth sections present the institutional details of OSHA and the nature of workplace fatalities and nonfatal injuries in the United States. The survey moves on to examine the evolving enforcement of OSHA regulations over the forty years of its existence and the corresponding empirical estimates of its effectiveness in improving worker safety and health. The next three sections discuss the other three pillars of the US safety policy system, detailing their strengths and weaknesses in generating appropriate safety incentives. The following sections evaluate the likely impact on worker safety of expanding OSHA enforcement power and assess the cost-effectiveness of OSHA in its entirety and some of its regulations individually. The survey concludes with recommendations on how best to improve OSHA's effectiveness in promoting worker safety and health.
