Book excerpt: Background: More than a third of people who die by suicide visited their primary care provider in the month before their death. Primary care staff thus have a valuable opportunity to treat this preventable public health problem. However, in order to do so successfully they require appropriate and effective training. If a person presenting with suicide risk has a negative experience when opening up to a health care professional, it may lead to further despair and silence. Despite the clear need for education in this area, existing trainings often do not meet the content needs and time constraints of primary care staff. Often, the content is insufficient in addressing the challenges faced by providers when dealing with suicide concerns in their practice. Also, available trainings often fail to consider the time constraints primary care staff face which is a barrier for training in the first place. A recently developed video-based training, called SafeSide Primary CARE aims to address these requirements and to equip providers with the necessary tools to tackle suicide risk in the primary care context. Initial studies of early versions of this training have yielded promising results, leading to a further iteration that now requires evaluation. Objective: This dissertation describes a program evaluation of SafeSide Primary CARE encompassing 4 primary care practices plus nursing students from an Ivy League School in the northeast of the US. A total of 130 primary care staff and nursing students participated in this evaluation. These staff included physicians, nurse practitioners, physician assistants, staff nurses, and administrative personnel. Methods: Participants were invited to complete a pre-test that included knowledge and self-efficacy items, as well as questions about their role and experience. They participated in three 50-minute sessions of SafeSide Primary CARE training. After the training, participants completed a post-training evaluation of knowledge, self-efficacy, and their perception of their ability to transfer training to a clinical setting, as well as an assessment of overall satisfaction. Results: Knowledge and self-efficacy items were analyzed using a paired t- test and revealed statistically significant gains. Data on perceptions of ability to transfer training and overall satisfaction collected in the post-test assessment were examined in relation to different learner groups with very positive results. Open-ended questions revealed themes that pointed towards changes in attitudes, knowledge skills and practice in suicide prevention. Conclusion: These findings are in line with prior program evaluations of the preceding iterations of the SafeSide Primary CARE training. This program evaluations demonstrates that the SafeSide Primary CARE program provides a training model and content that is relevant to the Primary CARE. The results of this program evaluation will aid the development of future iterations of this training, which will have a direct impact on providers and staff.

Additional aims were to compare the in-person and online training formats, assess training satisfaction, and explore themes of reflections and themes in application of training. Design: Cross-sectional, two-group, pre-test/post-test design to evaluate performance on knowledge test. In-person participants enrolled September 2016 to October 2017, online participants enrolled November 2016 to February 2018. Content analysis was used to identify themes in participants' responses to open-ended questions. Setting: The online training was available on the University of Washington Continuing Nursing Education (UW CNE) website. In-person training sessions were conducted at conference centers in Seattle. Subjects: Convenience sampling for recruitment. Knowledge test responses: Total N=2,107; Online n=1,642; In-person n=465. Evaluation survey responses: Total N=2,088; Online n=1,452; In-person n=636. Qualitative "ah-ha!" question responses: Total N=50; Online n=29; In-person n=21. Population of interest were registered nurses in Washington State affected by the mandatory suicide prevention training requirement; training was not restricted to this group. Intervention: UW CNE collaborated with subject matter experts to develop suicide prevention training for registered nurses that fulfilled Washington State Department of Health's required content areas. The training was offered in both in-person and online formats. Participants self-selected into the online or in-person version of the six-hour, suicide prevention training. Primary outcome measures: Mean pre-test, post-test, and change in score on the knowledge test. Training satisfaction ratings. Themes of applying of training to practice and reflections on training. Results: For both training formats, there were considerable gains in knowledge, when examining each knowledge test item. The mean difference in pre-test scores between the in-person and online formats was not significantly different (t=1.79, p=0.074), nor was the difference between pre-test and post-test scores (t=0.42, p=0.677). In both formats, the majority of participants rated teaching effectiveness highly and the overall course as "excellent." Effective communication, adjusting personal attitudes, and knowledge of resources were themes from qualitative responses on applying training to practice. Themes from "ah-ha!" responses indicated positive outcomes of the training on nurses' practice, attitudes, individual development, and ability to impact others. Conclusions: Participants exhibited comparable increases in knowledge across both formats. The majority rated the training as effective in delivery and in addressing learning aims. The study was limited in access to demographic variables and restricted by one observational time point for assessing knowledge.

While much attention has been given to this topic, no research has examined what specific information is being taught to pre-service school counselors. The purpose of this descriptive study was to evaluate the pre-service suicide prevention training in CACREP accredited school counseling programs. Fifteen areas of suicide prevention were assessed. The participants were 89 CACREP accredited programs. The instrument utilized was entitled Suicide Prevention Survey, and included fifteen areas of suicide prevention that could be included in program curriculum. The results were analyzed using frequencies and measures of central tendency. Respondents to the survey indicated that no CACREP programs offer a required course in suicide prevention. However, the issues of suicide were reported as being addressed in 39 different required and elective courses. Four percent (n=9) of the programs reported covering all fifteen categories included in the survey. One program reported coverage in area of suicide was non-existent. Implications for CACREP accredited programs as well as recommendations for future research is given.
