Book excerpt: Early and accurate detection of dementia of the Alzheimer type (DAT) is critically important given recent encouraging results with pharmacological interventions which demonstrate their greatest efficacy early in the disease process. Although excellent diagnostic accuracy has previously been reported for various neuropsychological tests, the cut-off scores for dementia derived in these clinically-based studies may be inappropriate when screening the general population in which medical disorders and questionable dementia are prevalent. The present study was designed to identify individual neuropsychological tests and combinations of tests that would accurately discriminate patients in the early stages of DAT from normal elderly (NE) individuals, and to evaluate the distinct qualitative features that underlie the neuropsychological deficits in this population-based sample. Evaluation of 1354 subjects aged 65 and older participating in an on-going epidemiological survey identified 52 individuals with DAT, 82 "at risk" (AR) for DAT (i.e., questionable dementia), and the remainder cognitively normal or possessing other dementias. An extensive neuropsychological battery was administered to 402 subjects (199 with suspected dementia and 203 randomly selected NE subjects). A logistic regression model comprised of measures of memory, language, and cognitive flexibility, correctly classified 98% of NE subjects and 82% of DAT patients. When the test scores of AR subjects were subjected to the logistic model, 77% were classified as normal. Examination of various error types, however, revealed a pattern of performance in the AR subjects that closely resembled that of the DAT patients. The results of this study demonstrate that several neuropsychological tests can identify patients with DAT in the community with reasonable accuracy, but the sensitivity and specificity for the diagnosis are less than previously reported with clinically-based samples. The discrepancy between these and previously reported higher values most likely represents the inclusion of subjects in the present study with medical disorders and questionable dementia who would have been excluded from clinically-based studies. The results of the error analyses provide support for a process approach to neuropsychological assessment for the early detection of DAT. Longitudinal studies are needed to validate the predictive and diagnostic accuracy of these measures
