Book excerpt: Substantial relief of discomfort may be anticipated by most patients suffering from pelvic and lower-extremity pain who are treated by arterial infusion of nitrogen mustard. Seventy-three patients with intractable pain secondary to malignancy arising in the pelvis received 83 percutaneous pelvic arterial infusions of this drug. Sixty infusions (72%) resulted in marked relief from pain for periods averaging 6-8 weeks. Advantages of the procedure are low toxicity, relative simplicity and availability of technique, and an acceptable rate of complications with minimal morbidity. Patients experiencing satisfactory results may expect significant relief from a second infusion for recurrent pain. The most rewarding result is the freedom from the cyclic return of pain characterized by oral and intramuscular analgesic therapy. Little or no relief can be expected in patients with pain caused by compression fractures of the vertebrae, or where the tumor burden is so great that adequate perfusion of the involved nerves is not possible. One should consider this procedure for controlling pain before resorting to the more dangerous and potentially disabling techniques of spinal cordotomy or intrathecal alcohol injection. Book excerpt: In the 1960s a firm rationale was developed for using raised temperatures to treat malignant disease and there has been a continuous expansion of the field ever since. However, a major limitation exists in our ability to heat human tumours, especially those sited deep in the body, with a reasonable degree of temperature uniformity. This problem has resulted in engineers and physicists collaborating closely with biologists and clinicians towards the common goal of developing and testing the clinical potential of this exciting treatment modality. The aim of the physicist and engineer is to develop acceptible methods of heating tumQur masses in as many sites as possible to therapeutic temperatures avoiding excessive heating of normal structures and, at the same time, obtaining the temperature distribution throughout the heated volume. The problem is magnified by both the theoretical and technical limitations of heating methods and devices. Moreover, the modelling of external deposition of energy in tissue and knowledge of tissue perfusion are ill-defined. To this must be added the conceptual difficulty of defining a thermal dose. The NATO course was designed to provide a basis for the integration of physics and technology relevant to the development of hyperthermia. There were 48 lectures covering the theoretical and practical aspects of system design and assessment, including, as far as possible, all the techniques of current interest and importance in the field.

It is an attempt to establish a critical mass of oncology literature covering virtually all oncology topics, revised frequently to keep the coverage up to date, easily available on a single library shelf or by a single personal subscription. We have approached the problem in the following fashion. First, by di viding the oncology literature into specific subdivisions such as lung cancer, genitourinary cancer, pediatric oncology, etc. Second, by asking eminent authorities in each of these areas to edit a volume on the specific topic on an annual or biannual basis. Each topic and tumor type is covered in a volume appearing frequently and predictably, discussing current diagnosis, staging, markers, all forms of treatment modalities, basic biology, and more.

Book excerpt: The ninth annual multidisciplinary symposium on clinical oncology organized by the Royal College of Radiologists was jointly arranged with the International Society for Radiation Oncology. It was held in London in February 1987 and discussed the biological and clinical basis of the effects of radiotherapy. Wherever possible lectures by an experimental scientist were paired with those of a clinical scientist in order to emphasize clinical relevance. It is hoped that this has resulted in a widely balanced view of the subject. The volume presents an updated version of these subjects based on those talks. After surgery, radiotherapy is the main treatment used in the management of patients with cancer. Its empirical success when first introduced is now backed up by a wealth of laboratory, clinical and experimental experience. New techniques for adminis tering the conventional X-ray therapy have been supplemented by methods which can be used to modify the radiation response. These include changes in dose rate or fractionation, and combined modality treatments including sensitization by drugs or heat. Other types of radiation, such as neutrons and other particles, are also now available which have enhanced physical and biological advantages.
