The current state of data suggests equivalency in most procedures with regard to traditional outcome measures, equal or somewhat elevated costs, with specific areas of superiority. The first section of this textbook, Surgical Robots, covers the history, economics, training, and medico-legal aspects of robotic surgery that will be of interest to students, residents, fellows, surgical staff, and administrators or public health specialists who seek to gain a comprehensive background on robotic surgery, or justification for purchasing a robotic system for their institution. Book excerpt: Robotic surgical assistants provide a novel way to improve the versatility and effectiveness of minimally invasive surgery but are still a maturing technology. There are many limitations associated with these robots which include a lack of haptic feedback, constrained nonintuitive workspace, and visual distortion. It is difficult to directly address many of the above limitations as the dynamics and kinematics of the robotic arms are significantly different than human anatomy. The research in this dissertation aims to overcome some of the above limitations by addressing problems related to the automation or surgical subtasks such as suturing. Automating subtask completion would overcome many limitations of robotic surgical assistants and allow the surgeon to complete procedures faster and with less fatigue. Ultimately, the surgeon would rely on the robot to perform common surgical subtasks, enabling the surgeon to focus on the overall surgical procedure. This work decomposes the problem of automated surgical subtasks into three primary parts: perception, planning, and control. Advances are made in both perception and planning. The first contribution of this work is the successful tracking of dynamic suture thread using stereo imaging. The tracking is capable of operating in real time and is robust against changes in the suture thread length. Visual tracking algorithms are also used to track a Magnetic Resonance Imaging (MRI) guided catheter. The tracking, completed using a catadioptric stereo system, is required for modeling and control of the catheter. The second contribution of this work is successful lumped modeling of the suture needle-tissue interaction forces. The lumped model is fast and accurately predicts the forces sensed during a circular needle drive. These lumped models are used in conjunction with best practices to plan out the needle drive trajectory. The goals of the trajectory are to successfully drive the suture needle such that the tissue trauma and scarring are minimized while the suture strength is maximized. The contributions of this dissertation focus on automating subtasks commonly used during surgical procedures. By automating the minutia of the surgical procedure, the robot may enable the surgeon to optimize the overall procedure without distraction.

Although these systems can provide great benefits, there are several limitations with these robots including lack of haptic feedback, limited robotic workspace, narrow laparoscopic camera view, and nonintuitive human-machine interaction. To address many of the above limitations, It would be ideal to merge the advantages of human perceptual and cognitive skills with the machine's speed and precision to create a surgical solution that is adaptable, precise, and fast. This thesis targets to merge human advantages with machine power by developing autonomous robotic surgical assistants to perform low-level surgical manipulation tasks such as suturing. Automating these surgical subtasks would address limitations of robotic surgical assistants by providing faster completion of procedures with less fatigue on the surgeon. Ultimately, the automation framework would keep the surgeon as the decision maker while relying on the robotic system to manage the execution of low-level motions. Executing any surgical subtask autonomously requires three main pillars of robotics: perception, planning, and control. This dissertation advances the perception and control aspects of the subtask automation problem. In this thesis, visually-guided autonomous low-level surgical manipulation tasks are studied. First, a three-dimensional surgical needle localization and tracking algorithm using stereo endoscopic video image streams is presented. The algorithm is able to maintain tracking in complex scenes and under intermittent occlusions of the needle. This is followed by computing the transformation between the endoscopic camera and the robotic manipulators (hand-eye calibration) in real time using an external tracking system. The computed calibration accuracy is sufficient for basic surgical manipulation tasks, such as surgical needle grasping. Finally, a visually-guided needle driving procedure for the purpose of autonomous suturing is developed. The procedure successfully drives the surgical needle under the visual guidance while minimizing the tissue trauma and scarring. Applying the contributions of this thesis to existing robotic surgery systems will bring robotic surgical assistants closer to autonomous surgical subtasks. By automating the surgical subtasks, the surgeon can optimize the overall procedure without distraction. These developments may eventually enable surgical robots to perform surgeries fully autonomously.
