Book excerpt: [Résumé en français] Les personnes victimes de traumatismes crâniens présentent des séquelles physiques, cognitives, comportementales et psychologiques qui engendrent de réelles difficultés de réinsertion familiale, professionnelle et sociale. Les troubles de la communication, qui s'inscrivent dans le tableau des séquelles cognitives, constituent aussi un véritable handicap en situation d'échanges. Le but de ce travail est d'étudier les conséquences que les troubles de la communication peuvent avoir sur la qualité de vie d'une personne traumatisée crânienne. Nous avons ainsi proposé le Test Lillois de Communication (TLC) et le questionnaire de qualité de vie du sujet traumatisé crânien (le QOLBI) à 26 patients traumatisés crâniens. Nous avons également apparié à ce groupe un échantillon de population de 26 patients sains. Nous avons observé des compétences communicationnelles perturbées, notamment la communication verbale, ainsi qu'un niveau de satisfaction de qualité de vie plus faible que la population témoin. Nos résultats montrent aussi que, du point de vue du patient, ce sont les troubles de la communication non-verbale qui les gênent le plus, notamment sur leur qualité de vie globale, fonctionnelle et personnelle. En revanche, selon les accompagnants, les troubles de la communication verbale semblent avoir plus de conséquences sur la qualité de vie globale, fonctionnelle et intellectuelle de la personne traumatisée crânienne. Le rôle de l'orthophoniste sera de mesurer la répercussion des troubles pour optimiser une prise en charge écologique.
