Patient heterogeneity is high, disease pathology can occur years to decades before becoming clinically apparent, and diagnostic and treatment biomarkers are lacking. In addition, the lack of validated targets, limitations related to the predictive validity of animal models - the extent to which the model predicts clinical efficacy - and regulatory barriers can also impede translation and drug development for nervous system disorders. Improving and Accelerating Therapeutic Development for Nervous System Disorders identifies avenues for moving directly from cellular models to human trials, minimizing the need for animal models to test efficacy, and discusses the potential benefits and risks of such an approach. This report is a timely discussion of opportunities to improve early drug development with a focus toward preclinical trials.

One of the current approaches is stem cell-based therapy that has been shown to be potentially helpful for the application of neuronal cell replacement for neuroregeneration. It is vital that the neurodegenerative disorder being detected at an early stage as it can provide a chance for treatment that may be helpful to prevent further progression of the fatal disease. Thus, research has focused on developing effective non-invasive diagnostic methods for early detection of these disorders. Molecular diagnostics can provide a powerful method to detect and diagnose various neurological disorders. Such diagnosis can enhance early detection, provide subsequent medical counsel based on medical pathway, as well as to gain better insight of neurogenesis and hopefully eventual cure of the neurodegenerative diseases. With research reports, reviews, mini-reviews and commentary, this research topic covers a wide range of areas in neurodegeneration research, including diagnosis and prognosis; regulating central nervous system; biomarkers and brain injury induced neurobehavioral outcomes among other timely reports on neurodegeneration.

In vertebrates, multiple motile and primary (immotile) cilia fulfill sensory and motility functions that are critical for embryonic development and the homeostasis of adult tissues. Cilia malfunction has been shown to cause a growing number of diseases presenting phenotypes such as brain and skeletal developmental problems, intellectual disability, blindness, infertility, obesity, and kidney cysts. Despite their clinical relevance, we are still far from fully understanding the basic biology of these multiple cilia types. For example, we do not know the molecular compositions of all types of cilia nor how cilia structural and functional diversity is generated. Also, we are still identifying the genetic causes of ciliopathies and the impact of mutations on ciliary protein function, cilia biogenesis/maintenance, ciliary functions, and disease development. Understanding all these aspects concerning vertebrate cilia will greatly contribute to the design of innovative therapeutic approaches for cilia-related disorders. Understanding the basic biology of vertebrate cilia is critical for the understanding of cilia-related disorders. However, there are still many open questions about these organelles. For example, we do not know the proteomes and protein-protein interaction networks specific of each cilium type. Also, the function of ciliary proteins is often studied mostly in vitro and in the context of primary cilia. This limits our possibility to answer questions like “why does a certain disease mutation affect only certain types of cilia in specific tissues?”. Therefore, this Research Topic aims to address these issues and focus on the latest findings on the basic biology of vertebrate cilia such as their molecular composition and biogenesis pathways, and also on advances on our understanding of the molecular mechanisms of ciliopathies. The goal of this Research Topic is to cover novel research focused on furthering our understanding on the molecular mechanisms underlying the development of cilia-associated diseases. Book excerpt: Amyotrophic lateral sclerosis (ALS), which was described since 1869 by Jean Martin Charcot, is a devastating neurodegenerative disease characterized by the selective and progressive loss of upper and lower motor neurons of the cerebral cortex, brainstem and the spinal cord. The cognitive process is not affected and is not merely the result of aging because may occur at young ages. The only known cause of the disease is associated with genetic mutations, mainly in the gene encoding superoxide dismutase 1 (familial ALS), whereas there is no known cause of the sporadic form of ALS (SALS), which comprises >90% of cases. Both ALS types develop similar histopathological and clinical characteristics, and there is no treatment or prevention of the disease. Because effective treatments for ALS, as for other neurodegenerative diseases, can only result from the knowledge of their cellular and molecular pathophysiological mechanisms, research on such mechanisms is essential. Although progress in neurochemical, physiological and clinical investigations in the last decades has identified several mechanisms that seem to be involved in the cell death process, such as glutamate-mediated excitotoxicity, alterations of inhibitory circuits, inflammatory events, axonal transport deficits, oxidative stress, mitochondrial dysfunction and energy failure, the understanding of the origin and temporal progress of the disease is still incomplete and insufficient. Clearly, there is a need of further experimental models and approaches to discern the importance of such mechanisms and to discover the factors that determine the selective death of motor neurons characteristic of ALS, in contrast to other neurodegenerative diseases such as Parkinson’s and Alzheimer’s disease. Whereas studies in vitro in cell cultures, tissue slices or organotypic preparations can give useful information regarding cellular and molecular mechanisms, the experiments in living animal models obviously reflect more closely the situation in the human disease, provided that the symptoms and their development during time mimics as close as possible those of the human disease. It is necessary to correlate the experimental findings in vitro with those in vivo, as well as those obtained in genetic models with those in non-genetic models, aiming at designing and testing therapeutic strategies based on the results obtained.
