Nous rapportons ensuite les résultats d'une série rétrospective de 69 accidents traités au centre hyperbare de l'hôpital Poincaré de Garches entre 1993 et 2003. Nous avons voulu savoir si les accidents traités à Paris ressemblaient aux accidents traités sur la côte. Ces accidents présentent certaines particularités. Les facteurs de risque comme le voyage en avion et la réalisation de plongées techniques sont plus souvent retrouvés. Les symptômes sont souvent atypiques (63%), les délais d'apparition sont assez longs (plus de deux heures après l'émersion dans 59% des cas), les premiers soins sont trop souvent insuffisants (seulement 23% des patients ont reçu oxygène, aspirine et hydratation). Un tiers des patients garde des séquelles parfois invalidantes de leur accident, un quart est gèné dans la vie quotidienne. Ceux qui replongent modifient peu leur façon de plonger.

Dans un souci d'amélioration de la qualité des soins apportés aux patients, chaque médecin doit s'inscrire dans ne démarche de Développement Personnel Continu, cette dernière peut être réalisée par la mise en place d'une évaluation des pratiques professionnelles au sein du service. L'objectif de ce travail est de mettre en place au sein du pôle RUSH, une méthodologie d'évaluation de la prise en charge pré-hospitalière des accidents de plongée de l'alerte à la recompression, simple d’exécution et reproductible. La méthodologie de l'audit clinique ciblé a été utilisée. Une grille dévaluation de la filière spécifique de prise en charge ainsi que son guide d'utilisation ont été élaborés sur la base de recommandations actuelles. Cette grille a été testée sur l'ensemble des dossiers d'accidentés du mois de juin 2012 ayant consulté au centre hyperbare. La durée de passation de la grille a été estimée à 3 minutes par dossiers. Dans 83% des cas, les seuls dossiers du centre hyperbare ont suffit. Les résultats de l'audit clinique ciblé mettent en évidence le respect des recommandations de conne pratique en ce qui concerne les soins donnés par l'entourage, l'avis du médecin hyperbare consulté quasi systématiquement, les critères de médicalisation respectées, des moyens d'évacuation adaptés et notamment des délais entre l'alerte et l'oxygénothérapie hyperbare conformes. Des axes d'amélioration peuvent être proposés sur la nécessiter d'alerter en cas d'accident car ceci entraîne un dépassement du délai préconisé entre les premiers signes et l'oxygénothérapie hyperbare. Cette amélioration passe certainement par une meilleure information des plongeurs et des directeurs de clubs sur la nécessité de déclarer rapidement tout signe anormal survenus dans les 24 heures suivant une plongée

En effet, les connaissances sont en constante évolution dans ce secteur. Par ailleurs, ces dernières années, de plus en plus de chercheurs tentent de répondre à cette question : “la plongée serait-elle bénéfique pour la santé ?”Plusieurs études ont déjà prouvé les bienfaits de ce sport sur le stress, l'angoisse, la tachycardie... Méthode : une étude rétrospective monocentrique sur 6 ans a regroupé les dossiers traités au centre de médecine hyperbare de l'hôpital sainte marguerite à Marseille. Une étude épidémiologique descriptive et des modalités de pris en charge pré hospitalière est conduite. Résultats : les accidentés sont majoritairement une population masculine (75.8%), jeune (44 ans) avec un Indice de Masse Corporelle de 24.6 +/- 0.4 et pour 77% non-fumeurs. Dans la majorité des cas ils n'ont pas d'antécédents ni de traitements en cours. Chez les plongeurs loisir, les plus accidentés sont de niveau III (42.7%) et chez les professionnels se sont les MF1 (33%). Conclusion : les accidents de plongées touchent une population jeune en bonne santé. Lors de la rédaction du certificat de contre-indication des situations à risque doivent être identifiées. La prise en charge des accidents est variable. La mise en place de registres nationaux doit permettre l'analyse des facteurs de risque et l'uniformisation des prises en charge.

Book excerpt: Psychoanalytic Studies on Dysphoria: The False Accord in the Divine Symphony depicts the profound dysphoria afflicting certain individuals, and includes the author's own personal experience of this as a German Jewish child during the Holocaust. Marion M. Oliner explores the impact of catastrophic events on the lives of individuals and their descendants from a broadly psychoanalytic perspective. The book focuses on the interplay between the experience and the unconscious meaning attributed to the trauma, and the ways in which patients may feel guilt, and blame themselves for the events and effects of their trauma. Drawing on the work of Freud and Winnicott, and with emphasis on the traumas suffered during the Second World War, Oliner offers new ways of understanding how resistant to treatment such traumas can be, and how the analyst can understand the experiences. The chapters span the evolution undergone in the nearly four decades of practice by the author. The book references a range of works including some taken from the German and French psychoanalytic literature, some never published in English. Taken together they aim at keeping the vitality of psychoanalysis without idealization, while discarding concepts whose essence is static, and therefore unhelpful. Psychoanalytic Studies on Dysphoria will appeal to psychoanalysts as well as other mental health professionals working with self-defeating behavior as a result of trauma.
