Avec l’âge de nombreux antidiabétiques oraux sont contre-indiqués et l’insulinothérapie devient le traitement de référence. Chez les personnes âgées dépendantes vivant en institutions, la gestion du traitement est complexe. Nous avons voulu savoir si les moyens mis en œuvre dans ces structures étaient suffisants et adaptés pour permettre un bon équilibre glycémique sans altérer la qualité de vie des résidents.Matériel et méthodes : nous avons réalisé une étude entre novembre 2012 et mars 2013 dans les EHPAD/foyers logements de Mayenne à l’aide d’un recueil médical sur le résident et d’un questionnaire à remplir par une infirmière afin de noter les difficultés liées à cette prise en charge et de proposer des solutions.Résultats : 122 patients de plus de 75 ans ont été inclus. Pour 86% d’entre eux, le diabète avait été découvert depuis plus de 10 ans et 68% était sous insuline depuis plus de 10 ans. Un bilan trimestriel d’HbA1C était réalisé chez 62% des résidents et l’objectif glycémique était atteint chez 46% des personnes. La principale difficulté relevée était l’impression d’altérer la qualité de la vie des personnes âgées. Les soignants ont aussi souligné les difficultés liées à l’alimentation des résidents ainsi que l’isolement décisionnel quant à la gestion des variations glycémiques. Les améliorations proposées étaient la formation spécifique en diabétologie du personnel, la précision du régime prescrit ainsi que la présence systématique de schémas d’adaptation individuels.Conclusion : Les moyens mis en place en structure sont adaptés mais insuffisants pour une prise en charge optimale des personnes âgées diabétiques.
