Book excerpt: Introduction : La dépression et le trouble anxieux généralisé sont des troubles mentaux fréquents en soins primaires et sont souvent pris en charge exclusivement par les médecins généralistes. Il existe une comorbidité importante entre ces deux pathologies. Si plusieurs études ont montré que la comorbidité augmente la morbi-mortalité des patients, il n'existe pas de recommandations spécifiques sur la prise en charge des patients anxiodépressifs. But de l'étude : Recueillir les habitudes de prise en charge des patients anxiodépressifs par les médecins généralistes et les moyens de les améliorer. Méthode : Etude qualitative par entretiens semi dirigés auprès de 14 médecins généralistes de la Vienne. Résultats : La prise en charge des patients anxiodépressifs associe un traitement médicamenteux, principalement les antidépresseurs IRS et les BZD à une prise en charge non médicamenteuse. La psychothérapie est largement conseillée aux patients qui sont orientés préférentiellement vers les CMP, assez faciles d'accès. Les médecins généralistes adoptent une attitude thérapeutique alliant écoute et suivi régulier mais pratique rarement des psychothérapies structurées. La prise en charge des patients dépressifs est surtout basée sur l'expérience des médecins et n'est pas influencée par la présence d'une comorbidité anxieuse. Des améliorations ont été proposées pour optimiser la prise en charge des patients anxiodépressifs. Elles concernent la formation des médecins généralistes, le remboursement des psychologues et la facilitation de l'accès à la psychiatrie avec une meilleure collaboration entre généralistes et psychiatres. Discussion : Cette étude a permis de recueillir les pratiques des médecins généralistes concernant la prise en charge des anxiodépressifs ainsi que des propositions pour l'améliorer. Ces suggestions sont des pistes à considérer pour permettre une optimisation de la prise en charge des patients. Des études complémentaires permettraient de définir plus précisément les attentes des médecins concernant leur formation en psychiatrie et leur collaboration avec les psychiatres pour prendre en charge les anxiodépressifs.
