Book excerpt: Le Trouble du Spectre de l'Autisme (TSA) est un trouble neurodéveloppemental dont les accompagnements éducatifs, pédagogiques et thérapeutiques sont menés de façon pluridisciplinaire. Il existe peu d'études et recommandations relatives au TSA à l'adolescence, bien que cette période de construction identitaire constitue un risque pour cette population qui présente davantage de comorbidités que la population générale. L'objectif de notre thèse était d'étudier les prises en charge des adolescents avec TSA par la Psychiatrie de l'Enfant et de l'Adolescent de la Fondation Bon Sauveur de la Manche au moyen d'une étude épidémiologique rétrospective entre 2018 et 2021. 109 dossiers ont été étudiés, avec un sex-ratio de 7,38:1. Les prises en charge étaient individuelles (64,2%) et groupales (40,3%) ; rééducatives, comportementales et psychodynamiques. 34,9% recevaient des psychotropes, dont 44,7% en polymédication. Les antipsychotiques étaient majoritaires (60%), suivis des stimulants (17,5%). Seuls 11% des familles d'adolescents bénéficiaient d'un soutien. Aucune prise en charge spécifique au TSA n'était identifiée et systématisée. Une enquête sur l'expérience des soignants complétait l'état des lieux, révélant un manque de connaissances des recommandations (57,1%) et une volonté de faire évoluer les prises en charge (81%). Développer l'éducation thérapeutique, la formation des professionnels et des aidants, permettraient de mieux répondre aux besoins de notre population. D'autres études auprès des adolescents avec TSA pourraient être menées afin de développer des programmes spécifiques et identifier des facteurs pronostics d'évolution favorable.

Book excerpt: Les troubles du sommeil (TDS) sont un motif de consultation très fréquent en pédopsychiatrie et notamment dans le cadre d'un trouble du spectre de l'autisme(TSA). Les données de la littérature montrent que les troubles du sommeil sont plus fréquents chez les enfants avec un TSA que les enfants à développement typique. Les principaux TDS retrouvés sont l'insomnie chronique et le trouble du rythme circadien à type de retard de phase. Des anomalies dans le système veille/sommeil dans ce groupe de population ont été observées telles qu'un déficit en mélatonine. Il existe plusieurs hypothèses à cette observation telles que la piste d'une étiologie génétique, d'une atteinte du système GABAergique, du système sérotoninergique. Ces TDS peuvent avoir un impact comportemental, émotionnel, cognitif sur l'enfant avec un TSA ainsi qu'un impact sur l'entourage qui y est confronté. Il existe plusieurs méthodes d'évaluation dans le sommeil dont le Gold standard reste la polysomnographie. L'architecture du sommeil des enfants avec TSA présente des particularités. Il a été retrouvé une augmentation de la latence à l'endormissement, une augmentation du sommeil lent léger, une diminution du sommeil paradoxal, une diminution du temps de sommeil total. La polysomnographie nécessite un appareillage qui peut rendre cet examen difficile à tolérer, notamment chez des enfants avec un TSA. L'actimétrie et la vidéosomnographie présentent alors un intérêt tout particulier. La prise en charge thérapeutique commence par l'élimination de comorbidités et d'autres troubles primaires du sommeil responsables d'une plainte d'insomnie et pouvant être traités. La mise en place de règles d'hygiène de sommeil est essentielle avant de pouvoir envisager des techniques psychoéducatives comportementales. Dans l'arsenal thérapeutique médicamenteux, la mélatonine a montré son efficacité et son innocuité dans plusieurs études. Elle sera à favoriser en première intention avant l'usage d'autres thérapeutiques. Devant une formulation non adaptée à la population pédiatrique pouvant présenter un trouble de l'oralité, la nouvelle formule pédiatrique de la mélatonine à libération prolongée semble prometteuse. Enfin, une piste thérapeutique pourrait être intéressante à explorer : l'usage de la luminothérapie.

L'approche psychanalytique, désormais très controversée et de plus en plus rejetée par les familles d'enfants autistes, tend à s'effacer pour laisser place à une approche dite comportementale. Elle constitue une manière plus pragmatique de pousser l'enfant à communiquer afin de l'aider à s'intégrer le mieux possible dans le monde qui l'entoure.
