Book excerpt: "A fascinating study of war surgery in World War I, where huge medical developments were made and the foundations of modern war surgery were laid World War I resulted in an enormous number of casualties who had sustained filthy contaminated wounds from high explosive shellfire, bomb and mortar blast, and from rifle and machine gun bullets. Such wounds were frequently multiple and severe, and almost invariably became infected. Surgical experience from previous conflicts was of little value, and it became quickly apparent that early surgical intervention with radical removal of all dead and revitalised tissue was absolutely vital to help reduce the chances of infections, especially the lethal gas gangrene, from developing. War Surgery 1914-18 explains how medical services responded to deal with the casualties. It discusses the evacuation pathway, and explains how facilities, particularly casualty clearing stations, evolved to cope with major, multiple wounds to help reduce mortality. There are chapters dealing with the advances made in anaesthesia, resuscitation and blood transfusion, the pathology and microbiology of wounding and diagnostic radiology. There are also chapters dealing with the development of orthopaedic surgery, both on the Western Front and in the United Kingdom, the treatment of abdominal wounds, chest wounds, wounds of the skull and brain, and the development of plastic and reconstructive surgery for those with terribly mutilating facial wounds. Book excerpt: Contexte : L'Evaluation des Pratiques Professionnelles (EPP), rendue obligatoire par la Haute Autorité de Santé (HAS) vise à améliorer la qualité des soins. La pratique de l'anesthésie locorégionale comme traitement de la douleur post opératoire (DPO) est de plus en plus répandue. La levée de bloc analgésique (LBA) entraîne une douleur parfois difficile à traiter. Méthode : Un premier audit de la prise en charge de la DPO a été mis en place sur des patients ayant bénéficié d'un bloc analgésique en plus d'une anesthésie générale ou une rachianesthésie pour une chirurgie orthopédique. Les patients ont été hospitalisés à l'hôpital de Dunkerque entre juin et août 2014. Après une phase d'évaluation des résultats et d'élaboration d'un plan d'action, un audit de contrôle a été réalisé avec les mêmes critères d'inclusion en avril 2015. Résultats : 65% des patients ont présenté une DPO modérée à intense imputable à la LBA. 74% d'entre eux ont été informés de la LBA. Le patient a été correctement informé sur les antalgiques qui allaient être administrés (84%) et sur leurs modalités d'administration (68%). Les évaluations de la DPO étaient établies à partir d'échelles standardisées. Les actions correctrices mises en place après analyse des résultats de l'audit avaient pour but la prévention de la LBA. Il s'agissait d'améliorer l'information des équipes soignantes et du patient, que ce soit oralement ou par écrit. Des réunions d'information avec les équipes soignantes ont été mises en place. L'administration des coantalgiques était rendue systématique jusqu'à la LBA. 87% des patients interrogés lors de l'enquête de contrôle avaient été informés de la LBA et 59% avaient eu une DPO modérée à intense. La différence avec le premier audit était non significative. L'information des patients concernant la LBA et ses conséquences tend à être meilleure que dans le premier audit (87,5% vs 73,7%, p=0,31 non significatif). Conclusion : Cette EPP a permis de soulever le problème de la LBA et de proposer des actions de prévention comprenant l'information et l'administration des antalgiques. Une proportion importante de patients avait toujours une LBA douloureuse. Cette étude ne permettait pas une analyse qualitative des prescriptions médicales. Compte tenu de la fréquence de survenue de LBA douloureuse, il est possible qu'un ajustement de ces prescriptions soit nécessaire.

The objective is to present the theoretical foundations of deformative transparency, as grotesque and esperpentic transparency, and illustrate the validity of such approach to understand the strategic and ethical implications of the proactive disclosure of the "shocking", "ugly" or "outside the norm". Four areas are discussed: political communication with particular focus on populist politicians as the deceased Venezuelan president Hugo Chávez, the campaign and presidency of Donald Trump, and the tenure in office of the mayor of Toronto, Rob Ford; propaganda strategies of Islamist terrorist organizations such as the Islamic State’s escalation of the visually horrific; and public health campaigns that use "disturbing images" to promote public awareness and eventually influence behavioural change. This study on the transparently grotesque is part of a research program about the economy of emotions in public communication.
