Une analgésie efficace et une évaluation régulière de la douleur sont indispensables. Le but de l'étude était de décrire la prise en charge de la douleur après césarienne dans une maternité de niveau 3. METHODE : Cette enquête rétrospective sur dossiers de césarienne de l'année 2009 a concerné 3 groupes de 35 patientes tirées au sort au CHU de Bordeaux sélectionnées selon l'anesthésie : rachianesthésie (RA), péridurale (APD), anesthésie générale (AG). RESULTATS : L'évaluation de la douleur a été faite exclusivement au repos par échelle numérique. En unité de surveillance (US), l'évaluation de la douleur a été régulière dans 89 % des cas : 53 % de l'effectif et 91 % des patientes du groupe AG ont été douloureuses. La prescription initiale à la sortie du bloc était le plus souvent multimodale, de posologie adaptée, avec un traitement conditionnel dans 96 % des cas. Entre J1 et J4, en secteur, la proportion de patientes douloureuses dans les 3 groupes était autour de 40 % avec une douleur sévère dans 6 % des cas. L'évaluation de la douleur a été régulière dans 43 % des cas. Le relais oral en secteur a été variable. En secteur, 97 % des prescriptions étaient faites par des sages-femmes. La traçabilité des prescriptions était faible (10 %). Le paracétamol a presque toujours été utilisé, les anti-inflammatoires non stéroidiens (AINS) ont été administrés 3 à 4 jours, la morphine n'a presque jamais été utilisée. Un traitement conditionnel a été administré chez 30 % des patientes douloureuses. Une prescription antalgique à la sortie a été retrouvée chez 20 % des patientes. CONCLUSION : Une réactualisation collégiale du protocole antalgique, une démarche-qualité et des sessions de formation continue pourraient permettre d'améliorer la standardisation de la prise en charge de la douleur des patientes césarisées au CHU de Bordeaux.
