Leur reconnaissance et leur prise en charge sont rendues complexes du fait de l'existence d'une hyperalgésie et de possibles réticences à la prescription d'opiacés. Dans la première partie de nos travaux, un état des lieux sur les connaissances et les pratiques de médecins non spécialistes de l'addiction sur la prise en charge de la douleur de patients sous médicament de substitution aux opiacés montre un besoin de formation sur les phénomènes d'hyperalgésie, de tolérance à la douleur et de tolérance croisée aux opioïdes. La deuxième partie de notre travail met en évidence une persistance significativement moindre à 12 mois chez les patients exposés à un phénomène douloureux aigu, y compris après ajustement sur le médicament de substitution. Elle révèle également une réponse différentielle sur le devenir à long terme des patients selon le médicament de substitution, avec un moindre maintien chez les patients traités par buprénorphine. Enfin, la troisième partie de notre travail décrit les pratiques de prise en charge de la douleur aiguë chez des patients traités par buprénorphine ou méthadone et leurs résultats analgésiques. Elle met l'emphase sur la nécessité de faire diminuer les scores de douleur d'au moins 30% lors de la prise en charge de ces patients, afin de diminuer l'impact à long terme de cette douleur sur leur traitement de substitution. Cette thèse souligne enfin l'intrication complexe entre douleur aiguë, douleur persistante et maintien du traitement substitution, et met en évidence les difficultés de suivi chez ces patients.

D'un point de vue pharmacologique, le recours à un ultra-opioïde semble pertinent, mais son utilisation est une solution dangereuse et ne doit pas être réalisée. Enfin, l'arrivée sur le marché des nouvelles formes de buprénorphine élargit le questionnement de la prise en charge en cas d'urgence.

Book excerpt: Introduction : le traitement de la douleur aiguë nociceptive des patients sous traitement de substitution aux opiacés (TSO) est complexe en raison de préjugés et d'une physiologie particulière de la douleur chez ces patients. L'absence de recommandations clairement établies sur ce sujet accentue cette difficile prise en charge. L'objectif était d'étudier les représentations, les ressentis et les pratiques des médecins sur ce sujet. Méthode : cette étude descriptive transversale a été menée auprès des médecins généralistes exerçant dans les Alpes-Maritimes et recevant des patients sous TSO. Ils ont été tirés au sort et interrogés entre octobre 2017 et janvier 2018 via un questionnaire en ligne. Résultats : sur les 42 médecins inclus dans l'étude, 60 % ont exprimé une gêne face à la plainte douloureuse des patients sous TSO et 81 % avaient des préjugés les rendant réticents à la prescription d'antalgiques opiacés. Les pratiques d'analgésie étaient disparates selon l'intensité de la douleur et le type de TSO prescrit. Face à une douleur modérée, 28 % à 32 % des médecins avaient des pratiques permettant une analgésie optimale. En cas de douleur intense, alors que 24 % des médecins avaient des pratiques permettant une analgésie optimale des patients sous méthadone, chez les patients sous buprénorphine ils n'étaient que 5 %. Seuls 18 % à 32 % des médecins ont modulé le TSO pour prendre en charge la douleur. Opioid overdose is easily reversed with the opioid antidote naloxone and with basic life support. Such care is generally only available in medical settings, however. These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.
