Ampio spazio è stato dedicato all’iconografia, anche a colori, finalizzata ad esemplificare sia i quadri di anatomia normale sia gli aspetti di imaging delle principali patologie descritte, senza escludere però la rappresentazione anche di alcuni casi particolarmente rari e desueti. L’impegno principale degli Autori non è stato pertanto quello di creare tanti piccoli radiologi e nemmeno quello di fornire il materiale minimo per superare l’esame, ma piuttosto quello di presentare, in forma semplice e, si spera, interessante le enormi potenzialità e gli straordinari progressi che negli ultimi decenni hanno completamente trasformato la Diagnostica per Immagini rendendola la disciplina clinica con il più elevato turn over di conoscenze. Il nostro auspicio è che il manuale possa anche essere conservato e consultato per un rapido riferimento post-laurea, nella pratica medica quotidiana.

Gli specialisti possono tutt’al più trovarci i concetti generali di Diagnostica per Immagini di altre specialità, che saranno loro comunque utili nella pratica quotidiana. In molti casi è stato necessario operare una selezione degli argomenti, minimizzando o trascurando alcuni aspetti poco frequenti o obsoleti: le decisioni non sono state quasi mai facili, talvolta dolorose, ma derivanti da circa vent’anni di esperienza di insegnamento e pratica quotidiana, il che mi ha permesso di operare delle scelte sulla base dell’osservazione clinica e dei suggerimenti degli studenti. Così, non sono stati trattati alcuni aspetti di Diagnostica francamente ultraspecialistici. Allo stesso modo, ho scelto di ridurre gli spazi tradizionalmente riservati ad alcune metodiche diagnostiche “classiche” (come l’urografia o gli esami contrastografici del digerente), la cui importanza quotidiana, come evidenziato dal numero delle richieste, è in costante diminuzione. Come contropartita alle rinunce, però, ho cercato di impostare una trattazione quanto più possibile aggiornata, caratteristica indispensabile di qualsiasi testo di Diagnostica per Immagini che aspiri ad essere realmente utile.

Obesity is not necessarily associated with comorbidities: there are indeed metabolically healthy obese individuals. Thus, we need to consider individuals presenting simple with obesity separately from those at risk of developing or who have already developed complex clinical states potentially leading to disability. Comorbidities can tip the balance of independence in patients who already have functional limitations mainly due to the excess of mass itself or who develop conditions such as diabetes, cardiovascular conditions, non-alcoholic fatty liver disease, where an abnormal metabolism of adipose tissue prevails. Morbid obesity with comorbidities leading to disability represents a real social and economic burden for National Health Systems worldwide. The presence of multiple and associated comorbidities often represents an obstacle to being admitted to hospitals for the treatment of metabolic diseases. On the other hand, clinical units with optimal standards for the treatment of pathological conditions in normal-weight patients are often structurally and technologically inadequate for the care of patients with extreme obesity. The aim of this book is to focus on the pathophysiological and rehabilitative aspects of disabling obesity, highlighting multidisciplinary rehabilitation interventions as key to counteracting the disabling aspects of complicated obesity.

But while we have lost our faith in utopias of all hues, the human aspiration that made this vision so compelling has not died. Instead it is re-emerging today as a vision focused not on the future but on the past, not on a future-to-be-created but on an abandoned and undead past that we could call retrotopia. The emergence of retrotopia is interwoven with the deepening gulf between power and politics that is a defining feature of our contemporary liquid-modern world—the gulf between the ability to get things done and the capability of deciding what things need to be done, a capability once vested with the territorially sovereign state. This deepening gulf has rendered nation-states unable to deliver on their promises, giving rise to a widespread disenchantment with the idea that the future will improve the human condition and a mistrust in the ability of nation-states to make this happen. True to the utopian spirit, retrotopia derives its stimulus from the urge to rectify the failings of the present human condition—though now by resurrecting the failed and forgotten potentials of the past. Imagined aspects of the past, genuine or putative, serve as the main landmarks today in drawing the road-map to a better world. Having lost all faith in the idea of building an alternative society of the future, many turn instead to the grand ideas of the past, buried but not yet dead. Such is retrotopia, the contours of which are examined by Zygmunt Bauman in this sharp dissection of our contemporary romance with the past.
