Book excerpt: The Western Pacific Region of the World Health Organization is highly diverse as it is composed of countries with advanced economies, transition economies, and the Pacific island countries and areas (PICs). Despite this diversity, the Region is experiencing some common epidemiological, governance and demographic trends which include population ageing and mobility, rapid urbanization, decentralization, persistent inequities despite rapid economic growth, unfinished agendas in reproductive health and infectious disease control, rise of noncommunicable diseases (NCDs) and increased environmental threats. Balanced against these threats are opportunities presented by the revitalization of the primary health care (PHC) values of equity and social solidarity, through a renewed regional focus on the values of PHC as part of a multisectoral commitment to universal health coverage (UHC). This report presents a policy and strategy overview of the experience of primary health care in the Western Pacific Region over the last 10 years and discusses options for future policy directions.

It provides recommendations for building and strengthening primary health care, including key components and actions for Member States to consider in planning primary health-care reforms. The Framework also compliments and provides a platform to facilitate implementation in delivering on other frameworks endorsed by the Regional Committee. This Framework will hopefully lead to more bold actions and policies aimed at building primary health-care systems designed to best serve the 1.9 billion people across the Western Pacific Region. WHO remains committed to working with all of the 37 countries and areas of the Region in making this ambitious vision for health a reality.

Experience from across the Pacific and globally suggests that the best trigger for progress towards the Healthy Islands vision and UHC for most (Pacific island countries and areas) PICs will be a focus on refining and strengthening primary health care (PHC). PHC is the foundation of the health system and the most equitable and efficient approach for UHC, with a strong affinity with the Healthy Islands vision.

Book excerpt: Recent years have seen a growth in the evidence base on policies and actions to promote health equity. Despite efforts, however, the evidence shows that inequalities are increasing rather than decreasing in many countries. This may partly be due to faulty policy decisions. There is clearly need for a better evidence-based approach on health policies to achieve equity. At the same time, understanding is inadequate on how policy-makers can best make use of the growing evidence base on promoting equity in health. There is a need for stronger links between evidence and health policy-making and implementation. To respond in part to this need, the WHO Western Pacific Regional Office convened the High Level Meeting on Promoting Health Equity: Evidence, Policy and Action from 16-18 October 2007 in Phnom Penh, hosted by the Royal Government of Cambodia, to provide an opportunity to exchange experiences and identify ways to promote the more systematic use of equity research in health policy and action.

Book excerpt: This Primer is about the 'how' of primary health care (PHC) and brings together best practices and knowledge that countries have generated through 'natural experiments' in strengthening PHC with the best available research evidence. Despite the progress made towards PHC globally, the concept is still often misunderstood, even within the public health community. The Primer offers a contemporary understanding of PHC and more conceptual clarity for strengthening PHC-oriented health systems. It does so by consolidating both scientific evidence and an extensive sample of practical experiences across countries for the needed evidence to address practical implementation issues. The Primer is organized in three parts. Part I explains the PHC approach, its history, core concepts and rationale, and draws out lessons for transformation. Part II addresses operational and strategic levers that make PHC work. It covers governance, financing and human resources for health, medicines, health technology, infrastructure and digital health, and their role in implementing change. Part III concludes with a cross-cutting view of the impacts of PHC on the health system, efficiency, quality of care, equity, access, financial protection and health systems resilience, including in the face of climate change.

Book excerpt: "This report, Women and health in the Western Pacific Region: remaining challenges and new opportunities, is a further step in advancing WHO's commitment to women's health. As the report shows, women and girls have particular health needs that remain invisible and little understood and that health systems are failing them. Some of these are health needs that only women have because of their biological role. Others are those related to all of the health problems that they share with boys and men throughout their lives. The Western Pacific Region has achieved considerable improvements in women's health. However, a significant unfinished agenda remains, with millions of women in the Region facing a variety of avoidable health challenges throughout their lives. Accelerating progress requires renewed political commitment, stronger government leadership, women-friendly health policies, programmes and services and actions across sectors to address the multiple determinants of women's health. It is hoped that this report contributes to strengthening the evidence base on the health of women in the Region and provides the basis for more vigorous action."--Page iii.

Available in PDF, EPUB and Kindle. Book excerpt: The U.S.-Associated Pacific Basin consists of six island jurisdictions: American Samoa, the Commonwealth of the Northern Mariana Islands, Guam, the Federated States of Micronesia, Republic of the Marshall Islands, and the Republic of Palau. This book examines one aspect of the ties and U.S. involvement with this part of the worldâ€"its role in the region's health care delivery system. Although the health status of the islanders and the challenges faced by the health care systems naturally vary within and among the jurisdictions, in general, almost all health indicators for the islanders are worse than those of mainland Americans. The health systems in the area must deal with conditions normally seen in developing countries (e.g., malnutrition, tuberculosis, dengue fever, and cholera) and in developed countries alike (e.g., diabetes, heart disease, and cancer). In examining the strengths and weaknesses of the area's systems, the volume provides a regional health overview and assessments of health care in individual jurisdictions, evaluates the Pacific Basin Medical Officers Training Program, and lays out a strategic plan for future health services in the U.S.-Associated Pacific Basin.
