En réponse à cette situation, de nombreux auteurs s’accordent à dire que le maintien d’un patient à domicile n’est envisageable qu’avec l’implication des proches aidants. Ce rôle implique une charge physique,émotive, familiale, parfois économique et financière. Chacun comprend bien que le stress provoqué peut rapidement mener à une situation d’épuisement, mettant ces personnes dans une situation à risque. En conséquence, le rôle du professionnel consiste en la mise en place d'interventions ciblées et adaptées à la personne afin de promouvoir la santé.Méthode : Revue de littérature utilisant les bases de données scientifiques : Cinhal, Medline et la Base de Données en Santé Publique. Les mots-clés utilisés pour les recherches sont les suivants : aidants naturels, proches aidants, soignants informels, domicile, soins à domicile, fin de vie, soins palliatifs, phase terminale de cancer, infirmier, rôle infirmer, soin infirmier, rôle professionnel, compétences professionnelles, soutien et accompagnement. Les limites utilisées sont en lien avec l'âge de la population cible, l’année de parution des articles, la langue et le domaine spécifique.Résultats : Nos premières recherches ont été fructueuses car de nombreux articles traitaient de nos différents concepts. Cependant ils ne faisaient pas le lien entre eux. Une recherche plus affinée nous a permis de sélectionner huit articles touchant quatre dimensions : l'expérience des proches naturels, les types d'aides nécessaires, l'impact psychologique du rôle de proche naturel et l'impact du savoir-être de l'infirmière sur la qualité du soutien.Discussion et perspectives : D'une façon consensuelle, tous les auteurs reconnaissent le fardeau qui pèse sur les épaules des proches aidants et tous cherchent à l'alléger en proposant trois types d'aide : pratique, matérielle, et informationnelle. Les interventions professionnelles proposées ont donc pour but la prévention de l'épuisement du proche aidant. Elles s'inscrivent dans le domaine de l'évaluation clinique permettant de mettre en évidence la vulnérabilité des proches aidants afin de les prendre en charge de manière ciblée et adaptée. Un axe est également focalisé sur l'enseignement et l'information, notamment au niveau des gestes quotidiens et de la gestion des médicaments. Enfin, un volet traite de la relation, condition de départ pour permettre une mise en place optimale des interventions de soutien.Conclusion : Les résultats de notre revue de littérature contribuent d'une part, à l'élaboration d'interventions infirmières basées sur des preuves scientifiques, d'autre part, ils constituent une ressource d'argumentation fondée sur des résultats probants, permettant de défendre la posture professionnelle infirmière.

Book excerpt: Childen's palliative care has developed rapidly as a discipline, as health care professionals recognize that the principles of adult palliative care may not always be applicable to children at the end of life. The unique needs of dying children are particularly evident across Africa, where the scale of the problem is overwhelming, and the figures so enormous that they are barely comprehensible: over 400,000 children in Africa died from AIDS in 2003, and out of the 166,000 children a year diagnosed with cancer, 85% of these are in the developing world. Despite the enormous need, provision of children's palliative care in Africa is almost non-existent, with very few health workers trained and confident to provide care for dying children. The challenges of providing palliative care in this setting are different to those in more developed countries, contending with the shortage of physical and human resources in addition to the vast scope of the care needed. Written by a group with wide experience of caring for dying children in Africa, this book provides practical, realistic guidance by improving access to, and delivery of, palliative care in this demanding setting. It looks at the themes common to palliative care--including communication, assessment, symptom management, psychosocial issues, ethical dilemmas, end of life care, and tips for the professional on compassion and conservation of energy--but always retains the focus on the particular needs of the health care professional in Africa. While containing some theory, the emphasis is on practical action throughout the book. Children's Palliative Care in Africa provides health care professionals working in Africa, and other resource-poor settings, with the confidence, knowledge, and capacity to improve care for the terminally ill child in constrained and demanding environments.
