Homeless pregnant women provide a unique challenge to health care providers and social workers. These women are at increased risk for many negative factors that could affect their pregnancy outcomes. The purpose of the study was to collect information from pregnant women experiencing homelessness in Lane County. Data was analyzed to identify demographic characteristics, service use patterns and recurring themes. It is expected the results will be the basis for further research with homeless pregnant women in Lane County. The project utilized a mostly qualitative research design supported by limited quantitative data. The sources of data included client records, written surveys, and face-to-face personal interviews. Findings indicated that there is a service gap for women less than eight months pregnant with no other children in her custody and that over half of the participants had other children that were not in their current custody. The investigator recommends that further research examine the psychosocial aspects of homelessness and pregnancy. Also, the association between current pregnancy and the previous removal of a child from a mother's custody should be further explored.

The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Hence, prenatal care and support system is necessary to meet all her needs during pregnancy. However, these healthcare needs are rarely provided for the pregnant women particularly those living homeless in the streets. Homeless women who are pregnant present a number of challenges to healthcare providers. As a group, they are at risk of variety of illnesses that can affect their pregnancies. Using descriptive phenomenological approach, the researcher explored the lived experiences of current pregnant women living homeless in the streets of Manila by using semi-structured interview. Ten (10) pregnant mothers were included as participants in this study using purposive sampling based on the following criteria: They are currently pregnant at any stage (1st trimester, 2nd trimester or 3rd trimester) and not particularly first time mothers; they had been homeless and living in the streets for 5 to 20 years; they are 18 to 35 years old; and they are willing to articulate, share, participate and describe their lived experiences. Collected narratives transcribed and these were reflected using Colaizzi’s analytical process. The researcher had observed such bracketing to prevent biases. Three main themes emerged from the encounter with these pregnant mothers living homeless in the streets of Manila: (1) Uncertainty and Ambivalence (2) Constant Fear (3) Hardship and (4) Resilience. The findings of the study will provide a better understanding of their lived experiences and will assess their health care needs as homeless pregnant living in the streets.

Book excerpt: Stress during pregnancy can lead to many adverse health effects for both the pregnant mother and her developing child. Pregnant women who are experiencing homelessness experience heightened levels of stress and are at an increased risk of poor pregnancy outcomes including preeclampsia, preterm birth, and postpartum depression. Mindfulness practices including prenatal yoga and meditation can be used to reduce stress and improve health and wellness for pregnant women, although homeless women face many barriers in accessing wellness services. This DNP/FNP scholarly project developed and implemented a low-barrier mindfulness program for pregnant women living in an emergency shelter in a midwestern urban area. Evaluation of the program indicated that short mindfulness practices, integrated into the existing shelter routine, was preferable to a designated mindfulness group. Participants gave positive verbal and written feedback about yoga and mindfulness and expressed interest in further mindfulness practices including aromatherapy.
