This Surgeon General's Report brings together for the first time the scientific evidence related to the prevention, assessment, diagnosis, and treatment of bone disease. More importantly, it provides a framework for moving forward. The report will be another effective tool in educating Americans about how they can promote bone health throughout their lives. This first-ever Surgeon General's Report on bone health and osteoporosis provides much needed information on bone health, an often overlooked aspect of physical health. Book excerpt: Osteoporosis, a skeletal disorder characterized by compromised bone-strength and an increased risk of bone fracture, is becoming increasingly prevalent, and the health and economic burden of osteoporosis is only expected to increase as the population ages. Osteoporosis disproportionately effects women, and past studies looking at exercise as a preventative factor have primarily focused on white women. Through the FIERCE study, total body bone mineral density (BMD) measurements of 213 post-menopausal African- American women living in the Washington D.C. area were collected via dual-energy x-ray absorptiometry (DXA), the gold standard in the field. Additionally, medical histories and anthropometric measurements were recorded. Participants were assigned to one of three exercise groups after the baseline visit- Supervised exercise, home based exercise, or control. We hypothesized that participants assigned to an exercise group would experience improvements or preservation in BMD levels among postmenopausal Black women at increased risk for breast cancer, as prior studies have showed exercise affecting hip BMD and resistance training preventing spine bone loss. After six months of maintaining their exercise assignments, BMD measurements were taken via DXA once again. A number of risk assessment tools are available to predict fracture incidence over a period of time, and these may be used to aid decision making. These tools are limited in that they may not include all risk factors, or may lack details of some risk factors. Tools are dependent on the accuracy of the epidemiological data used to derive them and tools validated in other populations may not apply to the UK. Two tools, FRAX and QFracture, are available for use in the UK. It is not clear whether these tools are equally accurate and whether choice of tool should depend on circumstances. This short clinical guideline aims to provide guidance on the selection and use of risk assessment tools in the care of people who may be at risk of fragility fractures in all settings in which NHS care is received.

The literature indicates this experience may vary for ethnic groups. This study examined two major risk factors, bone mineral density (BMD) and hip axis length (HAL), in older Filipina and Mexican American women and compared results with Caucasian women. Identical questionnaires/interviews for all three groups assessed lifestyle behaviors, demographics, disease history, and medication/nutritional supplement use. Acculturation levels among the ethnic minority women were assessed using a validated instrument. BMD, total bone mineral content (g) divided by the area (cm2), was measured at the hip, lumbar spine, and total body by a DEXA Hologic-2000 bone densitometer. HAL (cm), the distance from the base of the greater trochanter to the inner pelvic brim, was measured using Hologic software.
