Book excerpt: Premature termination is a pervasive barrier to effective implementation of outpatient psychotherapy that frequently results in decreased treatment gains for clients and lowered morale for therapists. Unfortunately, despite its high prevalence and cost, premature termination remains poorly understood. The current study addressed some gaps in the literature using a national online survey design that permitted investigation of a broader range of potential predictors, exploration of more specific reasons for premature termination, and examination of longer term treatment outcomes than has been possible in most previous research. Participants were 278 workers from Amazon.com's Mechanical Turk, an online labor market regularly used for social science research. Participants completed an online survey about their treatment history, their most recent outpatient therapy experience and therapist, termination status, reasons for terminating prematurely (if applicable), treatment satisfaction, therapeutic outcome, and demographics. Over half of the participants reported prematurely terminating their most recent episode of therapy. Results revealed that premature termination of previous therapy episodes, a weak therapeutic alliance, and primary or comorbid depression were the best predictors of premature termination. These predictors were highly accurate in distinguishing premature terminators from treatment completers. Results indicated that being a woman, identifying as non-heterosexual, seeking treatment from a hospital outpatient psychiatric clinic, and having a therapist low in perceived multicultural competence were also associated with increased risk of premature termination. However, these predictors of premature termination did not remain significant when controlling for other variables. The three most common reasons for premature termination were environmental obstacles, dissatisfaction with services, and lack of motivation for therapy. Finally, with respect to therapeutic outcomes, treatment completers reported greater problem improvement, greater satisfaction with therapy, and less current functional impairment than premature terminators. However, contrary to expectations, no differences in outcomes were found between early premature terminators (five or fewer sessions) and late premature terminators (at least six sessions). Clinical implications, limitations of the study, and recommendations for future research are also discussed.

Book excerpt: Premature termination is a common clinical phenomenon in university counseling setting, often very disruptive to the therapeutic process, can be dangerous for clients at risk, evokes considerable reactions and among therapists, and often causes treatment to be not as beneficial to them. The existing literature reflects both clinical conceptualizations and empirical investigations into the nature and effects on premature termination in psychotherapy, both on clients and therapists. However, there are only few studies that examined, from an objective personality assessment standpoint, profiles of clients who tend to drop-out of therapy prematurely. The current study investigated if clients at a university counseling center, who were classified to two groups (prematurely terminated and non-prematurely terminated), significantly differ on their Personality Assessment Inventory (PAI; Morey, 2003) scales. A canonical discriminant function analysis was conducted to determine whether the PAI scales could predict premature termination within a sample of university counseling center clients. The results indicated that the overall predictors differentiated between the two groups. The within-groups correlations between the predictors, two scales demonstrated significant relations with the discriminant function--SCZ and ANT-A. These results suggest that clients with antisocial behaviors (ANT-A) tend to prematurely terminate, whereas clients with schizophrenic tendencies (SCZ) tend to remain in treatment. In order to receive more accurate results and better range of those scales descriptive statistics were utilized, checking the percentage of students from the ANT-A group that received moderate results on the scale (T=>60) as well as those from the SCZ group who also receive moderate results on the scale (T=>60) . Results suggested that 83% of the prematurely terminating group had moderate elevation of ANT-A symptomatology (T=>60T), and 66% of the non-premature terminating group had moderate elevation of SCZ symptomatology (T=>60T). An integrative discussion of the results, via the lens of Motivational Interviewing perspective, allows for explanation and possible implications for clinical work.

The highest rate of attrition occurs after the first session. High rates of termination typically continue through the third or fourth sessions. This period is associated with the formation of the therapeutic alliance. This study attempts to predict which client-therapist dyads are likely to terminate prematurely using short paper and pencil instruments administered to clients and therapists before and after the first session. I divided the therapy dyads into three groups: Early premature terminators attending one session, late premature terminators attended between two and four sessions, and continuers attended five or more sessions. I drew the sample from a community mental health training clinic. Sixty-eight dyads participated in the study. Thirty-five dyads provided complete data sets. I assessed interpersonal variables with the INTREX short form questionnaires based on the Structural Analysis of Social Behavior (SASB). Clients and therapists completed the introject forms before the first session. After the first session, the clients and therapists rated each other's interpersonal behavior on the relationship forms. I hypothesized that hostility non-autonomous behavior, excessive focus on the therapist, non-complementary interactions, and conflicted patterns of communication would predict premature termination. The results showed that hostility, autonomous behavior and excessive focus on the therapist predicted premature termination. A discriminant analysis using the INTREX variables was able to correctly classify 85.7% of the dyads. A discriminant analysis using the pre-session INTREX variables was able to correctly classify 83.0% of the first session terminators versus those attending two or more sessions.
