Book excerpt: INTRODUCTION: La formation de tout médecin passe par la participation obligatoire à des stages dans les cabinets médicaux. Ces stages ne sont possibles qu’avec la participation des médecins généralistes libéraux qui acceptent de recevoir un étudiant au sein de leur cabinet, devenant des maîtres de stage des universités. Devant ce manque d’engouement à devenir MSU, j’ai essayé d’analyser les facteurs influençant l’accès à la maîtrise de stage et de dégager des pistes de solutions pour y remédier. METHODE: J’ai réalisé une étude qualitative par entretiens semi-dirigés auprès de médecins généralistes non maître de stage au sein de la Picardie. Les résultats de ces entretiens ont été analysés selon la grounded theory. RESULTATS: Dix-huit entretiens semi-dirigés ont été réalisés. Le stage chez le généraliste est apparu très utile pour une formation de qualité. Les facteurs influençant sont le manque de temps, la surcharge de travail, la formation pédagogique initiale mais aussi une non-reconnaissance avec un mal-être professionnel qui freine l’accès à la maîtrise de stage. DISCUSSION: Deux axes se sont dégagés pour lutter contre ces freins. Le premier est d’essayer d’établir un lien entre les protagonistes concernés afin d’améliorer la communication concernant notamment l’information sur la maîtrise de stage. Le second axe est d’améliorer l’attractivité pour celle-ci en suscitant des intérêts et en valorisant le statut pour notre spécialité. CONCLUSION: Les freins qui bloquent les généralistes, pour devenir maître de stage des universités, sont multiples mais ne sont pas, en réalité, insurmontables. Améliorer la communication et rendre attractive la maîtrise de stage sont des pistes de solutions possibles.

Book excerpt: INTRODUCTION : En 2004, la médecine générale est devenue une spécialité, engendrant la création de stages en médecine générale, obligatoires en 2ème et 3ème cycles des études médicales. Devant l’augmentation des étudiants, il était nécessaire de recruter de nouveaux maîtres de stage universitaires (MSU). Pour cela, une campagne de recrutement a été faite en 2012, en Picardie. Il apparaissait intéressant de connaître les motivations et réticences des médecins généralistes (MG) présents aux réunions, ainsi que les effets de cette campagne. METHODE : Une étude qualitative par entretiens semi-directifs a été réalisée auprès des MG présents aux réunions de recrutement. Les entretiens ont été analysés selon la méthode de la théorisation ancrée. RESULTATS : Vingt entretiens ont été réalisés. Le désir de partager ses connaissances, se remettre en question et revaloriser la profession étaient les principales motivations. Les MG étaient avant tout freinés par leur charge de travail, et par le dossier de candidature à fournir pour devenir MSU. La modification de la relation médecin malade a aussi été soulevée. Globalement, les réunions ont été appréciées par les MG, permettant d’obtenir cinq nouveaux MSU. DISCUSSION : Des solutions ont été proposées pour recruter d’autres MSU. Il est nécessaire d’améliorer la communication et l’organisation de ces réunions, tout comme les relations entre les différentes instances impliquées dans cette problématique. Etre MSU pourrait participer à la revalorisation de la profession. CONCLUSION : Les MG ont mis en avant de nombreux avantages à être MSU. Cependant, des réticences persistent encore, empêchant certains MG à franchir ce pas.

Alors qu'ils sont un pilier essentiel de l'apprentissage de la MG, peu d'études s'intéressent exclusivement aux MSU. L'objectif de ce travail est de comprendre comment la maîtrise de stage universitaire s'articule au sein du parcours du médecin généraliste. Méthode : Il s'agit d'une étude qualitative descriptive par entretiens de type récit de vie auprès de MSU. L'analyse des entretiens a été menée selon le cadre théorique de l'analyse interprétative phénoménologique afin de dégager un pattern commun au microcosme « MSU ». Résultats : 20 MSU ont été interrogés de janvier 2019 à juillet 2020 : deux axes principaux ont été mis en évidence : Pour les MSU interrogés, la maîtrise de stage est une composante à part entière de la médecine au sens large : on observait une nécessité de faire rayonner la médecine générale, spécialité dénigrée durant le parcours des interrogés, ils développaient un esprit militant et engagé pour ne pas reproduire les erreurs du passé. La maîtrise de stage apparaissait également comme une suite logique du parcours professionnel d'un médecin engagé au sein d'un groupe et d'un territoire : les rencontres et le partage étaient les pivots de leur développement en tant que professionnel épanoui, la maîtrise de stage s'inscrivait dans les suites d'un parcours marqué par le refus de l'isolement comme un moyen d'attirer les internes et d'en faire de futurs médecins installés. Conclusion : Ce travail a permis d'identifier comment la maîtrise de stage s'intégrait au sein du parcours des médecins interrogés, il permettra également de mieux cibler les médecins pouvant être intéressés par cet exercice et ainsi de faciliter leur recrutement.

Il existait peu d'informations sur ces MSU. L'objectif était de mieux les connaître. Cette étude a été réalisée par questionnaire par courrier électronique. Elle suivait deux axes : la comparaison du groupe des MSU de la région Centre à un groupe « témoin » non MSU, et une analyse de la fonction d'enseignant de ces MSU de la région Centre. Les MSU différaient des non MSU de manière significative sur six critères : ils étaient plus jeunes et étaient installés depuis moins longtemps. Ils exerçaient plus souvent une activité mixte et réalisaient plus de consultations selon le relevé interindividuel d'activité professionnelle (RIAP). Ils recevaient moins les visiteurs médicaux et étaient plus nombreux à avoir un diplôme médical complémentaire. Les femmes MSU étaient significativement plus nombreuses à participer. Les MSU semblaient aussi avoir plus souvent soutenu leur thèse à Tours, développé un mode d'exercice particulier et participé à des groupes de pairs. La majorité des MSU était récemment formée à la maîtrise de stage, et en activité pédagogique principalement pour les internes de niveau 1. Le caractère déclaratif des réponses, le mode de réponse électronique au questionnaire et la méthode de recrutement du groupe non MSU constituaient les principales imperfections de cette étude. Par les caractères mis en évidence au cours de l'étude, le MSU préfigure ce que sera le médecin généraliste de demain en région Centre : un médecin, jeune, plus actif, installé dans la région où il a effectué ses stages en médecine générale.

Ceci, nous ont amené à nous poser des questions sur l'impact du stagiaire lors d'une consultation. Matériel et méthodes: L'étude a été réalisé avec des questionnaires anonymes remplis après les consultations, en dehors de la présence des médecins. Ils ont été envoyés aux maîtres de stage de la région Picardie, ayant reçus des étudiants inscrit en 3éme cycles des études médicales. Résultats: La majorité des patients ont été favorables à la présence de l'étudiant avec un taux d'acceptation de 97,35%. Le stage chez le praticien a été plébiscité avec 29,8% des patients qui ont apprécié aider à former les futurs médecins et 10,5% qui ont profité d'un deuxième avis. Les maîtres de stage avec ce mode d'exercice, ont amélioré leur image pour 17,5%. Néanmoins, le motif de consultation est resté un élément de refus de la présence du stagiaire, lors des problèmes intimes, que ce soit familiaux (8,8%), conjugaux ou gynécologique (4,4%), ou génito-sexuels (3,5%), notamment chez les femmes (91,6%). Notre étude, a révélé également que les patients n'ont pas eu une confiance totale envers le stagiaire car ils se sont moins confiés en sa présence ( -18,46%) et ont pris en considération leurs prescriptions, du fait que leur médecin traitant supervisait la consultation pour 21,1% d'entre-eux. Conclusion: Dans la majorité des cas la relation médecin-malade n'est pas altérée dans son ensemble mais elle peut être modifiée en fonction du contexte de la consultation et démontre ainsi que le stagiaire n'est pas sans impact sur celle-ci.
