Book excerpt: We have completed recruitment, data collection and data entry for this study group of new mothers attending the well-baby clinic at the Naval Medical Center, San Diego. We enrolled more than 2500 eligible women and obtained essential data from 2433 of them. Approximately 20% of the study participants are active duty women. Results so far suggest some social, demographic and behavioral risk factors that appear related to increased postpartum weight retention in both active duty and dependent mothers. In particular, high weight gain during the first trimester of pregnancy and history of weight cycling appear to be risk factors for high postpartum weight retention. In addition, history of weight cycling contributes to excessive pregnancy weight gain, and may be an important indicator of women at risk for subsequent obesity after pregnancy.

Despite physical training and emphasis on meeting weight standards, weight retention was as common among active duty women as among military dependents. Multivariate analysis suggested that high level of maternal weight gain during pregnancy, history of weight cycling, postpartum depression, and financial insecurity were associated with increased weight after birth. Results were inconsistent and somewhat difficult to interpret for "healthy" dieting and physical activity, perhaps because women who have the most trouble losing weight are also those who may most consistently attempt to increase exercise and reduce food intake.

Given the unprecedented environment in the United States in which two-thirds of the adult population meets the criteria for being overweight or obese, the implications for women in the reproductive age period are unique in the history of the country. The concerns for maternal and infant health are real. The questions and answers tackled by committee members and workshop participants were not easy. Nevertheless, having an opportunity to explore what is known, examine the gaps in knowledge, and explore what to do now and in the future build a pathway for further inquiry and action. This report summarizes the workshop proceedings and highlights key themes that deserve further attention. The participants in this workshop describe what is known about recent trends in maternal weight gain and the impact of maternal weight during pregnancy on the health of mothers and their children. The workshop provided a valuable opportunity to assess trends that have occurred since the publication of an earlier study by the Institute of Medicine (IOM), which included guidelines for recommended weight gain during pregnancy.

Book excerpt: This study addresses the impact of pregnancy-induced changes in body composition and physical fitness on postpartum return to duty readiness. Physical activity, weight, FFM and fat mass, fitness, strength, and iron status was measured in women with- low to high pre-pregnancy BMI through a complete reproductive cycle. Physical activity was quantified by the doubly-labeled water method. Body composition was measured by hydrodensitometry, total body water, 40K counting, prompt-gamma activation, and DXA. Data obtained prior to pregnancy strongly indicate a need to revise military equations used to predict body fat. Postpartum weight retention at 6 mo averaged 2.7 i 3.9 kg, and was greater in the high BMI group (4.0 +/- 4.4 kg) than the low and normal BMI groups (2.4 i 3.5 kg). BMI had increased by approximately one unit in all groups. FM increased by 2.1+4.1 kg FM at 6 mo postpartum, with means of 2.6 +/- 3.7,1.7 +/- 4.1 and 3.O +/- 5.O kg in the low, normal and high BMI groups, respectively. There was evidence of deconditioning at 6 wk postpartum with near recovery at 6 mo postpartum. The effect of gestational weight gain, fat accretion and deconditioning would affect a woman's ability to return to weight, body fat and fitness standards in the 6 mo following delivery.

In an effort to understand this disconnect, Megan McFarlane analyzes military documents and conducts interviews with enlisted servicewomen and female officers. She finds a policy/culture disparity within the military that pregnant servicewomen themselves often co-construct, making the policy changes significantly less effective. McFarlane ends by offering suggestions for how these policy changes can have more impact and how they could potentially serve as an example for the broader societal debate.

The current epidemic of overweight and obesity in the United States affects the military services. The pool of available recruits is reduced because of failure to meet body composition standards for entry into the services and a high percentage of individuals exceeding military weight-for-height standards at the time of entry into the service leave the military before completing their term of enlistment. To aid in developing strategies for prevention and remediation of overweight in military personnel, the U.S. Army Medical Research and Materiel Command requested the Committee on Military Nutrition Research to review the scientific evidence for: factors that influence body weight, optimal components of a weight loss and weight maintenance program, and the role of gender, age, and ethnicity in weight management.
