Le diagnostic peut en etre malaise au debut de son evolution, en raison de l'absence de signe clinique specifique, du caractere inconstant des signes biologiques, et du retard d'apparition des erosions articulaires radiologiques. La polyarthrite rhumatoide est le plus frequent des rhumatismes inflammatoires de l'adulte. Sa prevalence est variable suivant les pays, avec taux moindre en Europe du Sud et une tendance a la diminution avec le temps. En France, elle est situee a . Il existe une nette predominance feminine avec un sex-ratio de 3/1, mais cette difference semble s'attenuer avec l'age. Le pic de frequence se situe autour de la quarantaine, cependant la maladie peut debuter a tout age y compris chez l'enfant (cf. arthrites juveniles idiopathiques). Il existe certains facteurs de risque environnementaux, dont le tabagisme, le surpoids, l'exposition a la poussiere de silice. La polyarthrite rhumatoide est une maladie auto-immune d'origine inconnue. It began with the development of the different types of tomography and the adoption of many contrast agents, then proceeded rapidly to serioscopy, subtraction of images, direct enlargement, echography, thermography, and xerography. Today, even before all these innovations have come into common use, another branch of radiologic technology has evolved: computerized (axial) tomography. More than just an innovation, its true dimensions are unfore seeable. Radiology has become in less than a century an indispensable adjunct to the practice of medicine. The development of radiology as a speciality followed its technical advances, which varied greatly from country to country. This rapid development led quickly to subspecialization, even the very early development of radiotherapy and radiodiagnostics as separate entities. However, the entry of radiology into the university has preserved it a single branch of medicine, avoiding the frequent tendency toward auto nomy of the branches of a speciality. My education in this disease began in 1962 when I arrived at the National Institutes of Health to begin a Clinical Asso ciateship with Dr. Joseph J. Bunim. Bunim introduced me to a pat ient with Sjogren's syndrome of 8 years duration who had devel oped malignant lymphoma 6 months previously. He told me that there were other such patients. I obtained serum samples from these patients and studied them by the then new technique of immuno electrophoresis. We observed that an initial hypergammaglobulin emia could progressively decline to hypogammaglobulinemia with loss of autoantibodies. One patient in this initial series had macrog lobulinemia. We published this report and suggested that the auto immunity predisposed to the malignant transformation. Thus began my love affair with this disease. In those days many rheumatologists considered Sjogren's syn drome simply a variant of rheumatoid arthritis. It's curious that two decades ago there was little confusion between Sjogren's syndrome and systemic lupus erythematosus, whereas today there is great con fusion. There is still a great need for internationally agreed upon di agnostic criteria, which merely illustrates once again the difficulty of accurate diagnosis in our profession. The multidisciplinary aspects of Sjogren's syndrome require au thorities in several areas of medicine. The various chapter contribu tors are experts in their field and have often put aside other respon sibilities to complete their contributions and not delay publication.
