Book excerpt: Excerpt from Political Economy of Health In the first the people are simply counted; men, women and children, - all have equal share in this enumeration; the infant and the mature, the strong and the weak, the healthy and the sick, are all presumed to contribute an equal portion to the body politic. Numbers have, in themselves, no power. They are merely representatives of things that may be nominally alike, but infinitely various in their degrees of value. A community of children in the forming stages of life, or of invalids, or of patients in hospital, or lunatics, is very differ ent from one that includes only persons in the mature and effective periods of life. Available in PDF, EPUB and Kindle. Book excerpt: Originally published in 1984, this book attempted to fill a gap by providing a broad-ranging structural analysis of the health care sector and the political and economic forces which influence its shape and contents, both in the western world and developing countries. The contributors examine the relationships of capitalism to health care, in terms of its influence on the physical environment, the incidence of social diseases and the prevailing (20th Century) view of what constitutes health itself; and in terms of the consequences of the new medical industrial complex it has created, such as the declining provision of health care for the poor and disadvantaged and the growing power of the pharmaceutical industry.

Available in PDF, EPUB and Kindle. Book excerpt: Why, despite vast resources being expended on health and health care, is there still so much ill health and premature death? Why do massive inequalities in health, both within and between countries, remain? In this devastating critique, internationally renowned health economist Gavin Mooney places the responsibility for these problems firmly at the door of neoliberalism. Mooney analyses how power is exercised both in health-care systems and in society more generally. In doing so, it reveals how too many vested interests hinder efficient and equitable policies to promote healthy populations, while too little is done to address the social determinants of health. Instead, Mooney argues, health services and health policy more generally should be returned to the communities they serve. Taking in a broad range of international case studies - from the UK to the US, South Africa to Cuba - this provocative book places issues of power and politics in health care systems centre stage, making a compelling case for the need to re-evaluate how we approach health care globally.

Available in PDF, EPUB and Kindle. Book excerpt: The Oxford Handbook of Health Economics provides an accessible and authoritative guide to health economics, intended for scholars and students in the field, as well as those in adjacent disciplines including health policy and clinical medicine. The chapters stress the direct impact of health economics reasoning on policy and practice, offering readers an introduction to the potential reach of the discipline. Contributions come from internationally-recognized leaders in health economics and reflect the worldwide reach of the discipline. Authoritative, but non-technical, the chapters place great emphasis on the connections between theory and policy-making, and develop the contributions of health economics to problems arising in a variety of institutional contexts, from primary care to the operations of health insurers. The volume addresses policy concerns relevant to health systems in both developed and developing countries. It takes a broad perspective, with relevance to systems with single or multi-payer health insurance arrangements, and to those relying predominantly on user charges; contributions are also included that focus both on medical care and on non-medical factors that affect health. Each chapter provides a succinct summary of the current state of economic thinking in a given area, as well as the author's unique perspective on issues that remain open to debate. The volume presents a view of health economics as a vibrant and continually advancing field, highlighting ongoing challenges and pointing to new directions for further progress.

Book excerpt: Excerpt from Medicine and Public Health in the People's Republic of China In 1969 the Fogarty International Center began a series of studies of research and health activities in other countries in relation to their political and economic structure. The first study dealt with Soviet programs in research and public health as they have evolved in the political structure established following the Bolshevik Revolution of November 1917. In late 1970 the Fogarty International Center began a search for in formation about medical and public health activities in the People's Republic of China. A long silence has shrouded that nation, which has almost one-fourth of the world's population, an ancient and respected civilization, and a long and honorable medical tradition. During 1971 communication between the United States and Mainland China re sumed after a hiatus of more than twenty years. For those who believe that the universal desire for health and relief from disease and suffering may be the strongest key to peace and international cooperation, it is significant and heartening to observe that physicians were among the first groups granted permission to visit the Pe0p1e's Republic of China. Book excerpt: The healthcare sector is one of the fastest growing areas of social and public spending worldwide, and it is expected to increase its government shares of GDP in the near future. Truly global in its scope, this book presents a unified, structured understanding of how the design of a country's health institutions influence its healthcare activities and outcomes. Building on the 'public choice' tradition in political economy, the authors explore how patient-citizens interact with their country's political institutions to determine the organisation of the health system. The book discusses a number of institutional influences of a health system, such as federalism, the nature of collective action, electoral competition, constitutional designs, political ideologies, the welfare effects of corruption and lobbying and, more generally, the dynamics of change. Whilst drawing on the theoretical concepts of political economy, this book describes an institution-grounded analysis of health systems in an accessible way. We hope it will appeal to both undergraduate and graduate students studying health economics, health policy and public policy. More generally, it can help health policy community to structure ideas about policy and institutional reform.

Available in PDF, EPUB and Kindle. Book excerpt: Health care is a social concern, closely connected to the pursuit of collective goals. That is why some have said that mortality and morbidity, too important to be left to the individual and the market, are properly the focus for public policy. One reason for intervention might be equity: the nation might take the view that none of its citizens, irrespective of economic status or geographical location, ought to be denied adequate access to medical attention. Another reason might be economy: the cost of care is rising and a rational society will want to ensure that the rise is not excessive. A third reason might be efficacy: scarce inputs should not be wasted, and that means economic analysis as well as scientific testing in order to identify value for money. The Political Economy of Health Care examines the potential contribution of state intervention to health care. Impartial and unbiased, it reaches the conclusion that a wide range of differing opinions must be carefully considered if informed debate is ever to lead to consensus in this complex, confusing but vitally important area of social life.

Book excerpt: Excerpt from Roundtable Discussion on the Economics of Health Care, Vol. 2: Hearing Before the Joint Economic Committee Congress of the United States; One Hundred Third Congress, Second Session The Committee met, pursuant to notice, at 10:00 a.m., in room 2200, Rayburn House Office Building, the Honorable Lee H. Hamilton (Member of the Committee) presiding. Present: Representative Hamilton and Senator Bennett. Also present: Patricia Ruggles, George Foy, Steve Baldwin, Morgan Reynolds, Michelle Davis, Ken Nelson and Caleb Marshall, professional staff members. Opening Statement of Representative Hamilton, Member Representative Hamilton. The Joint Economic Committee will come to order. Today is another in a series of roundtable conversations that the Joint Economic Committee is holding with prominent economists to discuss economic policy. Today's topic, the economics of health care, is certainly one of the most important economic policy issues of the 1990s. We are pleased to have as our guest today Dr. Uwe Reinhardt, James Madison Professor of Political Economy at the Woodrow Wilson School, Princeton University. Over the years, Dr. Reinhardt has made important contributions on the economics of health care. Dr. Reinhardt wrote Physician Productivity and the Demand for Manpower in 1975, and he undertook a study of West Germany's health-care system for the Pepper Commission in 1990. He has published numerous articles on health care in leading journals, and has also served on the U.S. Physician Payment Review Commission since it was established by the Congress in 1986. Dr. Reinhardt, we are very pleased to have you. We look forward to a good discussion with you. I understand you were very cooperative last night. We kept changing the schedule on you because of the schedule here. This has been a fairly hectic week for us in the Congress. I am reasonably sure I won't fall asleep in this meeting, but I am working on very few hours of sleep for the entire week.
