Un positionnement tridimensionnel correct de l'implant associé à des volumes osseux et gingivaux suffisants est impératif. Grâce au flux de travail numérique continu, il est dorénavant possible de planifier à l'aide de logiciels l'ensemble des étapes du traitement implantaire jusqu'à la conception et la fabrication de guides chirurgicaux assistées par ordinateur. Il existe plusieurs méthodes de fabrication pour réaliser ces guides, notamment l'impression 3D. L'objectif de ce travail est d'évaluer l'apport de l'impression 3D dans la fabrication et la précision des guides chirurgicaux. Après une présentation de l'évolution des concepts de chirurgie guidée et les différentes technologies d'impression 3D, une analyse de la littérature a été effectuée, nous avons sélectionné 13 articles à l'aide de mots-clés ou associations de mots-clés dans la base de donnée The National Library of Medicine (MEDLINE, PUBMED). Pour illustrer nos propos, un cas clinique sera également présenté. La pérennité de la prothèse supra-implantaire découle en majeure partie du positionnement de l'implant dans le tissu osseux, qualité par conséquent dépendante de l'expérience du praticien. La planification implantaire a pour objectif « d'assembler » différentes données : les données radiographiques et les données « cliniques » matérialisées par les modèles d'étude. L'association de ces données permet de réaliser une pose d'implant virtuelle qui servira de base à la conception de guides chirurgicaux. La réalisation par un prothésiste dentaire de guides chirurgicaux issus d'un wax up diagnostique a permis de limiter les erreurs de positionnement des implants. Cependant, la planification implantaire numérique ou chirurgie « virtuelle » permet de d'augmenter la précision, de diminuer le temps opératoire de l'intervention et d'aboutir à une chirurgie moins invasive. Cette dernière méthode simple et rapide d'utilisation permet de systématiser la planification implantaire numérique, de l'édentement unitaire à l'édenté total.

En l'occurrence, elles requièrent du temps pour l'apprentissage de l'utilisation des logiciels de planification, de la chaine numérique, à la fabrication du guide. De plus, elles représentent un coût conséquent notamment pour la pose d'implant unitaire. Tous ces facteurs sont des freins à l'utilisation des guides chirurgicaux dans la pratique implantaire quotidienne. Pourtant la chirurgie guidée, a pour vocation de conforter l'implantologie comme une discipline transversale faisant partie de l'arsenal thérapeutique de l'omnipraticien. L'objectif de cette thèse est de proposer un protocole accessible, reproductible et à moindre coût afin de sécuriser la pratique implantaire au cabinet. Cette démarche devrait permettre de promouvoir l'intérêt de l'apport du numérique dans la planification et la chirurgie implantaire. Cette thèse se veut avant tout clinique. Les étapes décrites seront celles du parcours implantaire du patient, de la première consultation, en passant par l'examen clinique, radiologique, jusqu'à la confection du guide chirurgical et la pose des implants. Elle traitera l'exploitation, et le traitement des données recueillies sur le logiciel Blueskyplan de la société BIOSUMMER3D. Quelques cas cliniques, ainsi qu'un tutoriel sur le logiciel Blueskyplan seront présentés pour permettre une prise en main facilitée du logiciel de planification. Notre profession évolue en permanence, et l'implantologie prend une part importante dans le traitement de l'édentement. L'intérêt de ce travail est d'aider le praticien à se rapprocher de la position idéale de l'implant en alliant sûreté et sécurité. Malgré l'impression de facilité que peut laisser paraître la pose des implants en virtuel, l'expérience opératoire reste indispensable à acquérir en chirurgie implantaire. Les recherches et essais cliniques de ce protocole ont été réalisés au sein du service de chirurgie orale et d'implantologie du centre de soins dentaires de Lyon.
