Book excerpt: The European Convention for the Prevention of Torture and Inhuman or Degrading Treatment or Punishment was adopted by the Committee of Ministers of the Council of Europe in June 1987. It entered into force in February 1989 and all 47 member States are Parties to the Convention. The Convention has already established itself as an important human rights instrument. Its approach is quite different from that of the European Convention on Human Rights. Whereas the ECHR provides a remedy for particular human rights violations after the event, the Convention for the Prevention of Torture (ECPT) seeks to prevent human rights violations, through a system of visits to places of detention. The Convention is intended to be an integrated part of the Council of Europe system for the protection of human rights, placing a proactive non-judicial mechanism alongside the reactive judicial mechanism established under the ECHR. The Yearbook of the European Convention for the Prevention of Torture offers an essential annual overview of developments in relation to the ECPT. Part One contains information on ratifications and other such issues in the authentic English and French texts. Part Two has details in English and French of the membership and activities of the Convention. Part Three reprints the twenty-first annual General Report of the ECPT in the official English and French texts. The diagnosis of a genetic condition affects not only the patient and biological family members who may themselves be at risk, but also "family" more generally as support may be sought from those considered kin and who may or may not be at risk themselves. It is considered best practice in genetic consultations to explore who will be informed within the family when a genetic condition or risk is diagnosed, particularly when the health of other family members is at risk. There is little guidance or consensus on how to achieve the implicit goal of informed family members while respecting patient confidentiality, however. There is a need for practitioners to be aware of the dynamics of family communication and to have guidance about how they may sensitively facilitate communication about genetics within families. This handbook facilitates the development of clinical practices relating to family communication about genetics. Relevant theories of family communication are summarized and related to a clinical genetics milieu and, from this, frameworks for practitioners are presented. A book of this nature is particularly timely as the completion of the Human Genome Project will result in an unprecedented amount of information about genetic constitution and health risks becoming available to individuals and their families. The presence of a potentially genetic condition in a family is not a new phenomenon. However, the growth in testing for genetic conditions, common complex conditions and variants that may influence health as well as drug metabolism means that a greater number of individuals will face decisions about communicating this information to their relatives. Many health professionals in all levels of health care will be confronted with issues of responsibility and practice in family communication about genetic information as they become providers of this testing.

D'autre part des réticences à aborder ce sujet persistent chez les médecins généralistes. Notre objectif était d'explorer l'influence de leur représentations dans l'abord de la prévention et du dépistage du VIH. Méthodes : Il s'agit d'une étude qualitative par entretiens semi-dirigés auprès de médecins généralistes recrutés de manière raisonnée et en variation maximale. L'analyse thématique des verbatims a été réalisée à l'aide du logiciel QSR Nvivo 10. Résultats : Seize entretiens ont été nécessaires pour obtenir la saturation des données. Malgré une reconnaissance de leur rôle dans la prévention et le dépistage du VIH il existe des difficultés pour beaucoup de médecins à aborder ces thèmes du fait des représentations qu'ils en ont. Certaines sont en rapport avec le contexte des consultations, la relation médecin-patient, les patients ou leurs réactions. D'autres sont liées aux médecins eux même : leur regard sur le VIH, les groupes à risque, l'abord de la sexualité, leur expérience ou leur formation. La plupart sont favorables à un dépistage plus systématique comme le préconise l'HAS mais beaucoup expriment des freins à sa mise en place. Conclusion : Les tests de diagnostic rapide, l'instauration de protocoles, et une meilleure information de la population semblent être des pistes pour rendre l'abord de ces sujets plus acceptable aux yeux des patients et plus systématique dans la pratique des médecins. Néanmoins leur implication et leur formation sont essentielles. Notre étude montre que leurs représentations peuvent fortement conditionner leurs pratiques et la façon dont ils envisagent de les faire évoluer.
