Book excerpt: L'étude a comme objectif principal de décrire la population de l'unité médicale des urgences psychiatriques adultes du Centre Hospitalier Esquirol (CHE) située au sein du Service d'Accueil et d'Urgence (SAU) du CHU de Limoges et comme objectif secondaire, de définir la place actuelle de cette unité médicale dans le parcours de soins des patients. Il s'agit d'une étude descriptive prospective, portant sur une population de 316 sujets qui a été conduite en 2018 pendant 3 mois du 1er Avril au 30 Juin sur l'unité médicale des urgences psychiatriques adultes du CHE. Cette unité médicale reçoit essentiellement des patients de la Haute-Vienne, pour la plupart déjà connus de la Psychiatrie, arrivant essentiellement d'eux même et le plus souvent pour des troubles anxieux et addictologiques. Au décours du passage aux urgences, la plupart des usagers rentrent sur leur lieu de vie avec un suivi ambulatoire spécialisé. La place actuelle de cette unité médicale dans le parcours de soins du patient ne correspond donc pas à la mission initiale qui est une mission de premier moyen d'accès à la Psychiatrie, de prise en charge des situations de crise, des urgences mixtes (tentative de suicide, intoxication éthylique aigue) et leur mission de liaison. Il est important de s'interroger sur la cohérence de cette mission supplémentaire semblant être devenue prioritaire à ce jour. Est-ce le bon endroit pour cette mission ? Les locaux semblent être peu adaptés aux urgences psychiatriques avec un manque de confidentialité, l'absence de chambre d'isolement et donc un risque de fugue, de passage à l'acte et un risque pour les usagers, les familles et les soignants. L'autre risque est l'embolisation du SAU par des patients ne nécessitant pas de surveillance somatique. Le taux d'adressage aux urgences par les médecins traitants est faible. Nous pouvons formuler l'hypothèse d'un bon maillage territorial et de l'efficacité du travail de communication auprès de ces derniers. Il serait intéressant d'améliorer la communication auprès des usagers et du grand public sur les voies d'accès aux soins psychiatriques. Actuellement, est en train de se structurer au sein de l'unité médicale d'Evaluation et d'Orientation Morel du CHE, une mission de consultations d'urgences psychiatriques pour les patients connus, ayant un suivi psychiatrique déjà en place (le CMP Morel). Cette unité médicale a pour but d'aider les usagers ou les médecins traitants qui n'arrivent pas à adresser les patients rapidement à leur psychiatre référent pour avoir un avis psychiatrique en urgence. Une étude similaire serait nécessaire sur la place de l'hospitalisation complète et du CMP Morel dans le parcours de soins. Il y a également un important travail de terrain à réaliser, une réflexion et une articulation à penser entre ces deux unités médicales (Urgences psychiatriques adultes et Morel) afin que l'usager puisse être orienté rapidement au bon endroit et au bon professionnel pour la prise en charge la plus coordonnée possible et ce même dans l'urgence.
