This hands-on resource is a collection of articles detailing the most advanced methods used by leading healthcare operational experts on how to provide high-quality care at less cost; manage financial risk more efficiently; design operational, clinical and information systems to meet the needs of patients, practitioners and managed care organizations; structure financial incentives to promote successful collaborations and make the transition from fee-for-service to risk-sharing arrangements. You'll find practical examples of how to build the trust necessary to create win-win solutions to problems that arise between competing yet interdependent interests of the various stakeholders. Edited by Peter Boland, PhD, and based on a "best practices" approach, each of the articles in the book illustrate compensation methodologies that have been successfully implemented with the support of physicians and hospitals.

If not negotiated and managed properly, the provider could be out of business or forced to sell their practice to a healthcare system. Physicians and their office managers need to have a clear understanding of what capitiation is and how it will effect their practice, how to negotiate with HMOs, and the risks of capitation. This book shows physicians how to negotiate and implement a capitated contract for a positive cash flow. It discusses the dynamics of capitated contracting, what to look for and how to negotiate with payors and how a capitated contract can effect cash flow and reimbursement.

Book excerpt: All physician groups reported using gatekeeping and preauthorization for certain referrals or tests. Most also used profiling of utilization patterns (79%), guidelines (70%), and managed care education (69%). Most physician groups asked gatekeepers to submit preauthorization requests for specialty referrals and restricted patient self-referral. For example, 60% of groups required preauthorization for an internal medicine subspecialty referral, and 7% allowed patient self-referral. Most groups also asked gatekeepers to obtain preauthoirzation for many tests (for example, 95% for magnetic resonance imaging and 53% for pulmonary function tests). Preauthorization requests were denied infrequently (less than 10% of the time) by more than 75% of groups. Of the 54 groups reporting utilization profiles to their physicians, 61% never adjusted for case-mix among patients and more than 60% suggested practice changes to their physicians based on utilization. Fewer than 35% of the groups used written guidelines for expensive tests that required preauthorization (such as angiography).

Available in PDF, EPUB and Kindle. Book excerpt: Enormous changes are occurring in the organization and financing of the U.S. health care systemâ€"rapid changes that are being driven by market forces rather than by government initiatives. Although it is difficult to predict what they system will look like once it begins to stabilize, the changes will affect all components of the health care workforce, and the numbers and types of health care professionals that will be needed in the futureâ€"as well as the roles they will fillâ€"will surely be much different than they were in the past. Despite numerous studies in the past 15 years showing that we might have more doctors than we need, the number of physicians in residency training continues to grow. At the same time, there is evidence that the demand for physician services will decrease as a result of growth of managed care. All of this is evidence that the demand for physician services will decrease as a result of growth of managed care. All of this is taking place at a time when, coincident with the result of failure of comprehensive health care reform, there is no coordinated and widely accepted physician workforce policy in the United States. The present study examines the following three questions: (1) Is there a physician policy in the United States? (2) If there a surplus, what is its likely impact on the cost, quality, and access to health care and on the efficient use of human resources? (3) What realistic steps can be taken to deal with a physician surplus? December
