Abstract: Background: With the increasing public health concern over the alcohol related health burden and mortality globally, the World Health Organization (WHO) has listed alcohol use as the top three risk factors in Non-Communicable Disease (NCD) and the alcohol related mortality and morbidity could be avoided through early intervention and prevention. The Department of Health (DH) of Hong Kong Special Administration Region (HKSAR) has declared the alcohol epidemic was alarming with the increasing in prevalence of alcohol use and binge drinking especially among young people with the westernization of Hong Kong society. In combating local alcohol use epidemic, DH has put priority to reform health care sector system and to ensure that the local health care system is responsive to the local alcohol use problem. Brief intervention (BI) was found to be effective in dealing with at-risk alcohol use especially in primary health care settings in the various past systematic reviews. However, most reviews mainly focus on physician-led BI while the role of nurse in BI delivery in alcohol drinking had not been under great attention. The effectiveness of nurse-led BI to at-risk drinking has not been fully examined as compared with physician-led interventions in the past reviews. Evaluation of treatment components in terms of intensity, treatment components and service settings may also shed light to public health policy makers in development of local model of BI in dealing with drinking problem in the Chinese population. Objective: To investigate the effectiveness of physician-led or nurse-led BI on quantity of alcohol consumption, number of drinking days, number of binge drinking episode and health care utilization. The potentially effective treatment intensity, treatments components and setting of intervention were also investigated. Methods: All the studies published from 1990 to 2012 in MEDLINE, would be evaluated on the effectiveness of BI delivered by physicians and/or nurses to adult at-risk drinkers in primary health care settings, were searched and identified using a combination of keywords. Results: A total of 13 randomized controlled trials out of 134 articles from MEDLINE were included in this systematic review. The included studies used different outcome measurements to compare the effectiveness of BI by physicians and/or nurses in treating at-risk drinking. Similar demographics and clinical characteristics of the subjects between the intervention and control groups were reported. The studies were from 5 countries. The age range of subjects was from 14 to 75 years old. Majority of subjects drank beyond the recommended limits defined by Alcohol Abuse and Alcoholism (NIAA), United State (US). Through there were discrepancies among the results generated in the included studies on the effectiveness of physician-led and/ or nurse-led BI, the benefits of nurse-led BI in treatment of at-risk drinking cannot be dispelled and could be considered as an alternative or supplement to the physician-led BI in busy primary health care setting today. BI with at least two 5-15 minute sessions was found to be more effective than very BI with one 5-minute session only. The purpose of this study was to assess preliminary efficacy of screening for alcohol use and brief interventions (BIs) on patients who present to the emergency department (ED) at-risk for alcohol use disorder (AUD) utilizing established nurse navigators. This is the first study to assess the initial efficacy of a BI to reduce alcohol use in at-risk patients utilizing nurse navigators in the ED who were not bedside nurses. Theoretical Framework. The theoretical framework guiding this study is based on the Health Belief Model (HBM) and the Transtheoretical Model and is the first study to assess the HBM constructs for patients with AUD in the ED. Methods. A pilot, non-equivalent, two-group quasi-experimental study assessed the preliminary effectiveness of a registered nurse-led BI to reduce alcohol consumption and dependency, improve readiness to change alcohol use, lower depressive symptoms, decrease perceived barriers, and improve the perceived susceptibility, seriousness, and benefits among individuals presenting to the ED who were at-risk for AUD. Two groups were enrolled: an intervention group and a usual-care comparison group. Results. At 3-month follow-up, the intervention group had a significant decrease in standard drinks on a typical day and the construct of perceived seriousness was significantly higher in the intervention group than in the comparison group. Within groups significance was found at the 3-month follow-up for both the intervention and comparison groups for most outcome variables. The Patient Health Questionnaire (PHQ-9) scores for depression were significantly greater at baseline for participants who did not return for follow-up. Conclusion. This pilot study examined the feasibility of a nurse navigator-administered BI in the ED for patients at-risk for AUD and may be beneficial for future randomized control trials to assess the efficacy of a nurse navigator-led BI in the ED. Implications. Nurses should be screening patients on admission for alcohol use based on current evidenced-based practice tools. They should be planning interventions with their patients at risk for alcohol use disorder. They should be assisting with referrals to treatment in the community and offering options to the patient who is ready to change their alcohol use. Electronic health records can be utilized to alert nurse navigators for consult to provide the BI and refer patients to treatment.

Book excerpt: A public health approach to the harms that arise from alcohol misuse requires that alcohol problems be identified early and intervened with before serious social and medical consequences occur. An intervention directed at at-risk drinkers, known as alcohol brief intervention, has well established efficacy when delivered by physicians working in primary health care clinics and some hospital Emergency Departments. However, the adoption by physicians has not been widespread, due to a number of barriers including lack of physician time, skill, and knowledge about alcohol. An important question thereby arose as to whether other health professionals, including social workers, achieve the same positive patient outcomes as physicians when delivering the intervention. Using data from the largest effectiveness study of alcohol brief intervention in Emergency Departments (ED's) to date (Academic Emergency Department Screening, Brief Intervention, and Referral to Treatment Research Collaborative), changes in alcohol use from patients who were treated by physicians were compared to changes in those patients treated by non-physician health professionals. Participants in the current study (n=279) were drinking at levels above the National Institutes of Health limits. Patients were divided into two groups: 137 received the intervention from a physician and 142 received the intervention from a non-physician health professional. In addition to baseline data on demographic, education and health variables, measures on three quantity and frequency of alcohol use variables were collected at baseline and at 3-months post intervention. Results of an Analysis of Co-Variance that adjusted for differences in baseline drinking, indicated that controlling for gender, marital status, education level, and severity of addiction, the ED patients who received the alcohol brief intervention from trained non-physician health providers did as well as at 3-month follow-up on the three assessed outcomes of reduction in alcohol use as those patients who received the intervention from a physician. Findings suggest that non-physician health professionals who are trained in alcohol brief intervention be considered to provide the intervention, removing significant barriers to physician delivery that currently exist, and increasing opportunities for wide-spread implementation of this efficacious public health approach to reducing the harms of alcohol misuse.

Of those with a substance use disorder, approximately 60 percent also have a mental health disorder. As many as 80 percent of patients with behavioral health conditions seek treatment in emergency rooms and primary care clinics, and between 60 and 70 percent of them are discharged without receiving behavioral health care services. More than two-thirds of primary care providers report that they are unable to connect patients with behavioral health providers because of a shortage of mental health providers and health insurance barriers. Part of the explanation for the lack of access to care lies in a historical legacy of discrimination and stigma that makes people reluctant to seek help and also led to segregated and inhumane services for those facing mental health and substance use disorders. In an effort to understanding the challenges and opportunities of providing essential components of care for people with mental health and substance use disorders in primary care settings, the National Academies of Sciences, Engineering, and Medicine's Forum on Mental Health and Substance Use Disorders convened three webinars held on June 3, July 29, and August 26, 2020. The webinars addressed efforts to define essential components of care for people with mental health and substance use disorders in the primary care setting for depression, alcohol use disorders, and opioid use disorders; opportunities to build the health care workforce and delivery models that incorporate those essential components of care; and financial incentives and payment structures to support the implementation of those care models, including value-based payment strategies and practice-level incentives. In 2012-11 there were nearly 1 million alcohol-related violent crimes and 1.2 million alcohol-related hospital admissions. The problem has developed because cheap alcohol is too readily available; increasing numbers of people drink at home before going on a night out ("pre-loading"); the Licensing Act failed to deliver a cafâ culture; too many places cater for people who drink to get drunk regardless of the consequences for themselves or others; and individuals who cause the problems have not been challenged enough over their behaviour. The availability of cheap alcohol will be curtailed through the introduction of a minimum unit price for alcohol. The exact level is to be agreed, but if it was 40p, it is estimated there would be 50,000 fewer crimes each year and 900 fewer alcohol-related deaths by the end of the decade. Consultations will also aim to end multi-buy promotions. Local areas and agencies will be given powers to challenge people's behaviour and make it easier to take action against, and even close down, problem premises. Other measures include early morning restriction orders and a late night levy so that businesses open late contribute to the costs of policing. The drinks industry has a crucial role to play in changing the drinking culture towards positive socialising. And the risks of excessive consumption will be widely circulated.

Much applied research of brief alcohol intervention (BI) has shown its efficacy in reducing excessive alcohol consumption among individuals misusing alcohol in primary care. However, to date there has been no sustained method to introduce BI into the general healthcare system. The goal of this paper will be to review different methods of implementing BI into primary care, and which strategy has the potential to be routine, with sustained efficacy in reducing harmful alcohol consumption. Methods: Qualitative analyses were performed on published articles accessed from the Weill Cornell Medical College Library website, and the PubMed search engine. All articles were in English. Results: Ninety-eight articles were initially found; four articles were actually used in the review. Conclusion: The articles chosen compared different modalities of implementing brief alcohol intervention (BI) into the primary care setting. The varying modalities included interactive voice response technology (IVR-BI), electronic clinical reminders (e-SBI), and web-based alcohol screening and intervention. Some of the modalities reviewed have shown promising efficacy in reducing harmful alcohol consumption, with a sustained effect after a two-year follow-up. With promising results, these studies warrant further investigation in real-world settings, while other studies need to be replicated to validate previous findings. To approach this issue, a research proposal should be designed to study a comparison of which implementation method would be most utilized among patients and physicians, and which is the most effective in reducing harmful alcohol consumption among patients misusing alcohol over time.
