Available in PDF, EPUB and Kindle. Book excerpt: For patients and their loved ones, no care decisions are more profound than those made near the end of life. Unfortunately, the experience of dying in the United States is often characterized by fragmented care, inadequate treatment of distressing symptoms, frequent transitions among care settings, and enormous care responsibilities for families. According to this report, the current health care system of rendering more intensive services than are necessary and desired by patients, and the lack of coordination among programs increases risks to patients and creates avoidable burdens on them and their families. Dying in America is a study of the current state of health care for persons of all ages who are nearing the end of life. Death is not a strictly medical event. Ideally, health care for those nearing the end of life harmonizes with social, psychological, and spiritual support. All people with advanced illnesses who may be approaching the end of life are entitled to access to high-quality, compassionate, evidence-based care, consistent with their wishes. Dying in America evaluates strategies to integrate care into a person- and family-centered, team-based framework, and makes recommendations to create a system that coordinates care and supports and respects the choices of patients and their families. The findings and recommendations of this report will address the needs of patients and their families and assist policy makers, clinicians and their educational and credentialing bodies, leaders of health care delivery and financing organizations, researchers, public and private funders, religious and community leaders, advocates of better care, journalists, and the public to provide the best care possible for people nearing the end of life.

Available in PDF, EPUB and Kindle. Book excerpt: Winner of the 2015 FWSA Book Prize The body is a site of impassioned, fraught and complex debate in the West today. In one political moment, left-wingers, academics and feminists have defended powerful men accused of sex crimes, positioned topless pictures in the tabloids as empowering, and opposed them for sexualizing breasts and undermining their natural function. At the same time they have been criticized by extreme-right groups for ignoring honour killings and other culture-based forms of violence against women. How can we make sense of this varied terrain? In this important and challenging new book, Alison Phipps constructs a political sociology of womens bodies around key debates: sexual violence, gender and Islam, sex work and motherhood. Her analysis uncovers dubious rhetorics and paradoxical allegiances, and contextualizes these within the powerful coalition of neoliberal and neoconservative frameworks. She explores how feminism can be caricatured and vilified at both ends of the political spectrum, arguing that Western feminisms are now faced with complex problems of positioning in a world where gender often comes second to other political priorities. This book provides a welcome investigation into Western politics around womens bodies, and will be particularly useful to scholars and upper-level students of sociology, political science, gender studies and cultural studies, as well as to anyone interested in how bodies become politicized.

Available in PDF, EPUB and Kindle. Book excerpt: Experts estimate that as many as 98,000 people die in any given year from medical errors that occur in hospitals. That's more than die from motor vehicle accidents, breast cancer, or AIDSâ€"three causes that receive far more public attention. Indeed, more people die annually from medication errors than from workplace injuries. Add the financial cost to the human tragedy, and medical error easily rises to the top ranks of urgent, widespread public problems. To Err Is Human breaks the silence that has surrounded medical errors and their consequenceâ€"but not by pointing fingers at caring health care professionals who make honest mistakes. After all, to err is human. Instead, this book sets forth a national agendaâ€"with state and local implicationsâ€"for reducing medical errors and improving patient safety through the design of a safer health system. This volume reveals the often startling statistics of medical error and the disparity between the incidence of error and public perception of it, given many patients' expectations that the medical profession always performs perfectly. A careful examination is made of how the surrounding forces of legislation, regulation, and market activity influence the quality of care provided by health care organizations and then looks at their handling of medical mistakes. Using a detailed case study, the book reviews the current understanding of why these mistakes happen. A key theme is that legitimate liability concerns discourage reporting of errorsâ€"which begs the question, "How can we learn from our mistakes?" Balancing regulatory versus market-based initiatives and public versus private efforts, the Institute of Medicine presents wide-ranging recommendations for improving patient safety, in the areas of leadership, improved data collection and analysis, and development of effective systems at the level of direct patient care. To Err Is Human asserts that the problem is not bad people in health careâ€"it is that good people are working in bad systems that need to be made safer. Comprehensive and straightforward, this book offers a clear prescription for raising the level of patient safety in American health care. It also explains how patients themselves can influence the quality of care that they receive once they check into the hospital. This book will be vitally important to federal, state, and local health policy makers and regulators, health professional licensing officials, hospital administrators, medical educators and students, health caregivers, health journalists, patient advocatesâ€"as well as patients themselves. First in a series of publications from the Quality of Health Care in America, a project initiated by the Institute of Medicine

Book excerpt: Virtual Clinical Excursions Pacific View Regional Hospital is a groundbreaking workbook/CD-ROM package that brings learning to life in a "virtual" hospital setting. This guided learning experience features textbook reading assignments that correspond with the CD-ROM and workbook activities. The workbook acts as a map, guiding students through the virtual hospital as they care for patients in the virtual hospital to help students make connections between what they experience through the CD-ROM and what they've learned in their textbook. Each virtual hospital visit allows the student to access realistic information resources essential to patient care resulting in a true-to-life, hands-on learning experience. Real-world problems place users in nursing scenarios where they can set priorities for care, collect data, analyze and interpret data, and reach conclusions about complex problems within a health-illness transition. The highly-engaging format encourages active learning and provides opportunities to identify information to collect and process data for evidence-based patient care; work with data entry and retrieval screens; optimize use of available information resources; understand hospital information systems; manage information for decision-making; and maintain and improve core computing skills. Lessons in the workbook complement the content in the textbook and guide the student through the virtual hospital. Includes over 250 NCLEX-RN examination-style questions based on the VCE patients that offer built-in testing of clinical knowledge. The virtual hospital gives students the opportunity to monitor and care for six unique patients/cases, each of whom has complex medical-surgical conditions. The online testing program includes more than 250 NCLEX-style questions for each patient, allowing instructors to assess and measure students' understanding and clinical judgment for each case. Each workbook activity includes time icons showing how long it should take the average student to complete the exercise and linkage icons to indicate that a previous activity must be completed before attempting the one at hand. Augments knowledge of physical assessment by allowing the student to examine and document vital signs, as well as assess information related to the head and neck, chest, back and spine, upper extremities, abdomen, pelvic, and lower extremities. The CD-ROM offers users the opportunity to visit each patient and access realistic information essential for patient care, including: charts, medical administration records, electronic patient records, video clips of nurse-client interactions, and other related materials. Offers students the opportunity to monitor and care for one or more patients during three 45-minute periods of care within a 12-hour shift. Clinical Alerts highlight changes in the patient's condition within these 45-minute periods. A Time Icon appears under each lesson, indicating how much time it will take the student to complete the activity. Includes 6 unique new patients/cases, each with complex conditions for students to care for and monitor within the virtual hospital. A new Medication Room that introduces students to a wide variety of drug storage areas, such as a Unit Dosage Cabinet, Automated Dispensing System, IV Storage, and a Refrigerator. A medication administration feature enables users to select, prepare, and administer medications, reinforcing the precepts of safe medication in a harm-free environment. Over 50 common medication errors imbedded within the software to boost clinical realism and prepare students for current practice. Greater faculty support with the addition of the Fac Pac, a new instructors' website, and a radically expanded testing component. A drug guide provides complete drug monographs for all medications used within the program.
