Book excerpt: The purpose of the current study was to identify characteristics contributing to or hindering utilization of mental health services in the past 12 months among working age adults with mental illness using Andersen's Behavioral Model of Health Services Utilization. Originally developed in 1968, Andersen's model hypothesizes that three sets of factors, (predisposing, enabling, and need) differentially impact health services utilization. Historically mental health services have been underutilized in the U.S. and have unique mechanisms for care and reimbursement that are less prevalent for general health services. The current study utilized data from the National Survey on Drug Use and Health, a population based-survey capturing patterns of drug use, mental health, and health services use in U.S. households of individuals 12 and older. The current study focused on mental health data for adults aged 18-64 who met criteria for mild, moderate, or serious mental illness. All data were publically available through the Substance Abuse and Mental Health Services Administration and SSPS 22 was used to analyze data. Statistical analyses included descriptive frequencies, bivariate logistic regression, and multivariate logistic regression with odds ratios reported. The final sample included 7,335 respondents. More than half of participants did not receive any mental health treatment. Among predisposing factors, being female and age 35 or older were associated with a greater likelihood of service utilization while being a race other that Non-Hispanic white was associated with a decreased use of services. Among enabling factors, individuals not currently working and having public insurance increased mental health service utilization. Individuals who had up to a high school diploma or GED, lacked insurance coverage, and made less than $10,000 annually were less likely to use mental health services. Among need factors, having moderate or serious mental illness and reporting unmet need was associated with an increased use of mental health services. Cost was the most reported reason for unmet need followed by thinking they could handle the problem without treatment for both users and non-users of mental health services. In order to increase the use of mental health services, future policy and treatment interventions should focus on enabling factors.

Book excerpt: Abstract: Equitable access to physical and mental health services among the severely mentally disabled (SMD) is a significant public health concern. This research aims to describe the SMD population of Franklin County, Ohio, receiving services from the public mental health system; to describe and to compare the Medicaid enrolled SMD population with the non-Medicaid enrolled SMD population with respect to race, age, sex, and psychiatric diagnoses; to examine the impact of psychiatric diagnoses and race on physical and mental health services utilization, and related health care costs among a cohort of individuals with serious mental disabilities enrolled for three years in Medicaid in Franklin County, Ohio. The population studied for research aims 1 and 2 included those individuals designated as SMD by the Ohio Dept. of Mental Health, including those in Franklin County Alcohol, Drug and Mental Health Board administrative (ADAMH) database for SFY 1996 and identified as African American or Caucasian, over the age of 18 and living in the community (n=6550). The population for research aims 3 and 4 included comparable individuals designated as SMD by the Ohio Dept. of Mental Health. Subjects must also be continuously enrolled in Medicaid for three years (SFY 1996-1998). Research aims 1 and 2 employed a cross-sectional design to examine ADAMH administrative claims data for SFY 1996 and the claims from Ohio Dept. of Jobs and Family Services for SFY 1996. The dependent variables were psychiatric diagnoses and Medicaid status. Independent variables included age, sex, and Medicaid status. Analysis used bivariate and multivariate statistics. Research aims 3 and 4 involved a multiyear cohort design that examined the ODJFS and ADAMH databases for SFY 1996-1998. Dependent variables for this aim included health service utilization, prescription drug utilization, mental health treatments and health care costs. Independent variables included race, age sex, Medicaid status and psychiatric diagnoses. Analyses used bivariate and multivariate statistics. Analyses of aim 1 indicated that of the SMD population included in the study 69.5% were Caucasian and 30.5% were African American. Fifty nine percent were females and 41% were males. The age category of the SMD clients reflects a wide range although the majority of clients are under the age of 44 (60%). Affective Psychoses (34%) and Schizophrenic Disorders (13%) comprised the largest diagnostic category of this population. Race was determined to influence psychiatric diagnoses with African Americans more likely to be diagnosed with Schizophrenic Disorders, Drug Dependence and Nondependent Drug Abuse. Caucasians were more likely to be diagnosed with Neurotic Disorders and Paranoid States. Analyses of aim 2 demonstrated that Medicaid status was variable and influenced by psychiatric diagnoses, age, and race. Over 50% of those with Schizophrenic Disorders and Affective Psychoses were receiving Medicaid in SFY 1996. Comorbid psychiatric diagnoses and race were predictors of current Medicaid enrollment status. This study suggests that psychiatric diagnoses have an important relationship to the receipt of public insurance. Analyses of aim 3 indicated that utilization of acute care services among the SMD population was remarkably high. Conversely, services received for women's preventive health care, and dental and vision care were below that of the general population, and below recommended clinical standards. Clients with Schizophrenic Disorders and Affective Psychoses received less physical health services and more mental health services. Analyses of aim 4 indicated that racial differences in psychiatric diagnoses, Medicaid and Medicare enrollment status, and the utilization of pharmaceutical agents occurred in this SMD population supporting previous research findings. Further research is needed to plan adequate and cost effective mental health service delivery systems.
