Book excerpt: Choosing Unsafe Sex focuses on the ways in which condom refusal and beliefs regarding HIV testing reflect women's hopes for their relationships and their desires to preserve status and self-esteem. It also discusses the related issue of seropositivity concealment or non-disclosure. Many of the inner-city women who participated in Dr. Sobo's research were seriously involved with one man, and they had heavy emotional and social investments in believing or maintaining that their partners were faithful to them. Uninvolved women had similarly heavy investments in their abilities to identify or choose potential partners who were HIV-negative. In either case, women sought to present and to view themselves as wise and their men as monogamous. Women did not see themselves as being at risk for HIV infection, and so they saw no need for condoms. But they did recommend that other women use them; they saw other women as quite likely to be involved with sexually unfaithful men. Choosing Unsafe Sex includes recommendations for educational strategies that are sensitive to cultural expectations for relationships. Dr. Sobo's findings have significance not only for inner-city HIV/AIDS educators but for all who seek a deeper understanding of mainstream assumptions about heterosexual relationships.

To do the research for the dissertation, I spent quite a bit of time at the Center of Disease Control and Prevention archives as a graduate student. I also interviewed medical doctors and others who knew about the subject matter. Since I wrote this dissertation, there has been more research published that I will continue to research and add to my archival collection. The issues of this dissertation were discussed as the emergent theoretical model and its components, which included implications of research, practice, stigma, burden, advocacy, and awareness. Leadership, education, and community resources were the dominant themes that emerged in the study. The study findings imply an increased need for leaders to present public awareness about the affects HIV/AIDS has on the African American community. Future research should consider the explicit nature of the answers, which benefited the study. The information would be helpful while improving the quality of life available for African American women and would enable leaders to interact with a leadership perspective (USAID, 2009).

Accordingly,thetopicsaddressedinIdentitiesandSexualitiesofMinorityWomen were developed based on input from minority women who participated in focus groups conducted in diverse regions of the United States. These focus groups were held speci?cally to provide an opportunity for sexual minority women in minority communities to identify those issues that from their perspective are most salient and relevant to their lives. It is not surprising, in view of the variation in process by which the topics were identi?ed, as well as the differences in perspective as- ciated with differences in sex and gender, that this resulting compilation of topics departs substantially from the focus of the companion text addressing health issues of minority men who have sex with men.

Since then, the epidemic has spread throughout the world, but at an uneven pace. It is estimated that more than 60 million people worldwide have lived with HIV/AIDS since the beginning of the epidemic and 20 million of these have died (UNAIDS 2002). HIV/AIDS now affects every country in the world. Despite advances made in knowledge about HIV prevention, the disease continues to spread. Globally, sub-Saharan Africa is the most severely affected, with the Southern African Development Community (SADC) being home to half of the estimated 24 million people living with HIV/AIDS in this region (UNAIDS 2000). Country level estimates of HIV infection are usually based on surveys of women attending antenatal clinics. In South Africa over the past decade, this has been the primary means of monitoring the spread of HIV. Antenatal surveillance systems provide countries with a low-cost tool for regularly monitoring key aspects of the HIV epidemic. For example, the data can be used to track the epidemic in different parts of a country or among a specific age cohort. The antenatal survey's major usefulness is to track trends of HIV infections over time.
