Book excerpt: Introduction : Malgré une diminution importante des cancers du col de l'utérus depuis 20 ans, environ 1 000 femmes en décèdent encore chaque année en France. Si la généralisation du dépistage du cancer du col de l'utérus chez les femmes de 25 à 65 ans atteignait son objectif de 80% de couverture sur le territoire français, l'incidence et le nombre de décès par cancer du col de l'utérus diminuerait de 30 % à 10 ans. Une étude qualitative a été réalisée pour appréhender la perception des femmes du dépistage du cancer du col de l'utérus en milieu militaire et leurs attentes vis-à-vis du médecin d'unité. Matériels et méthode : L'étude a été réalisée du 10 novembre 2016 au 1er février 2017 dans le département du Val de Marne. Un questionnaire a été envoyé à 900 patientes militaires assurées de la Caisse Nationale Militaire de Sécurité Sociale, par courrier. Les thèmes abordés dans le questionnaire sont leur pratique du frottis, leurs représentations par rapport à la réalisation de ce frottis, leurs connaissances du frottis, des éléments biographiques et leurs recours aux centres médicaux des armées. Résultats : Le nombre de répondantes était de 280 femmes militaires d'actives de 28 à 60 ans. Les femmes répondantes déclaraient faire le dépistage du cancer du col de l'utérus (92,2%). La pratique du dépistage du cancer du col utérin sur leur lieu professionnel était perçue comme une entrave à leur vie privée. Elles ne voulaient pas mélanger leur vie intime avec leur vie professionnelle. Elles refusaient que le médecin militaire s'occupe de leur suivi gynécologique car il n'était pas spécialiste de gynécologie. Paradoxalement, elles reprochaient au médecin des forces de ne pas assez se montrer ouvert à leurs questionnements en matière gynécologique, ce qui supposait qu'elles étaient favorables à recevoir plus d'informations. Elles n'envisageaient de consulter en gynécologie qu'en cas de "problèmes" ce qui suggérait leur manque de demande de dépistage. Deux profils de patientes qui pourraient échapper à notre système de surveillance ont été mis en évidence par l'analyse des correspondances multiples : les femmes militaires qui consomment peu le système de soin parce qu'elles se disent débordées et les militaires du rang, qui correspondait aux femmes à plus faible revenu et/ou à niveau de diplôme inférieur aux autres grades. Conclusion : Les femmes répondantes sont réceptives à une information qui les inciterait à plus de prévention. L'étude tendrait aussi à montrer que la préférence des femme militaires à consulter un praticien civil pour leur santé intime est l'une des causes principales du faible niveau d'intervention du médecin généraliste militaire dans le dépistage du cancer du col de l'utérus. Par ailleurs, le manque d'intérêt du médecin des forces pour la gynécologie n'encourage pas les femmes militaires à parler de ce sujet en consultation. La valorisation du rôle du médecin des forces dans la prévention du cancer du col de l'utérus et la collaboration avec des partenaires civils permettrait aux femmes militaires de répondre à leurs besoins dans le domaine gynécologique et de renforcer les liens des médecins du SSA avec les praticiens civils de proximité.
