As a result, healthcare organizations are continuously seeking new ways to improve patient satisfaction, preferably with minimal use of resources or expenditures. Research has demonstrated hourly rounding to be a cost-effective, nurse led intervention that positively influences patient satisfaction (Petras, Dudjak and Bedner, 2013; Sherrod, Brown, Vroom and Sullivan, 2012; Brosey and March, 2015). Hourly rounding is a proactive approach to patient care where basic patient needs are anticipated and fulfilled through regularly scheduled checks by healthcare professionals. This evidence based practice (EBP) project proposes implementation of hourly rounding to improve patient satisfaction at Canyon Vista Medical Center's (CVMC) emergency department (ED). A thorough description of the problem and proposed solution will be discussed. A detailed implementation plan including rationale, evidence, approval, implementation logistics and resources will be provided. An evaluation plan consisting of evaluation tools and data collection methods will be presented in addition to a strategy for dissemination of findings to key stakeholders and the greater nursing community. Finally, a literature review including analysis and appraisal of fifteen peer-reviewed research articles on the topic of hourly rounding and patient satisfaction will be presented.

Patients must be monitored and cared well to improve their health conditions and to avoid further complications during hospital stay. Risk of hospitalized patients fall is the most concern for nurses. Hospital acquired new incidents like patient fall and pressure ulcers has a negative impact on patient recovery, causes lengthy hospital stay, financial burden for hospitals and increases nursing burnouts. Patients with stroke are at high risk for fall. In earlier studies it is found that 14% of the stroke patients fell at least once during their hospital admission (Tutuarima, Meulen, and Limburg, 2012). Frequent monitoring of the patients by nurses can prevent never events. But nurses attempt to be proactive on avoiding these issues are impaired by factors like high patient ratios, lack of time , frequent call lights and stress from meeting the patients demands. Planned hourly rounding is a proactive approach to patient care. Hourly rounding reduces patient fall, skin break downs, call light use, noise level and also improve patient satisfaction. Studies on hourly rounding to increase the patient safety and patient satisfaction showed a 52% decline in call light use after implementing hourly rounding and unit was quiet and nurses could spend more time for patient care and logs (Ford, 2010). Intentional rounding researches showed significant reduction in fall rate with one hour rounding. With the implementation of hourly rounding on the stroke and cardiac inpatient unit of the Texas Health Resources Hospital, the purpose is to prevent unwanted events and to improve patient satisfaction and safety.

That information can be used to determine if hourly rounds increase patient satisfaction and safety. Nurses will also be interviewed about the hourly rounding process. The process will be reviewed and determined the method is effective I increasing patient satisfaction and safety.

The patient population consists of cardiovascular and thoracic post-operative surgical patients of all ages primarily adults. The implementation of the evidence based practice of hourly rounding has been proven to increase overall patient satisfaction by improving staff responsiveness and increasing patient safety within many hospitals. The measurement of effectiveness and success will be evidenced by seeing an increase in patient care scores and a reduction of falls within the unit; when these metrics are met, there is data that proves that this evidence based practice will drive the results we are looking for. In this microsystem, there are many indicators that a more efficient workflow is necessary to be established. There is the constant sound of call lights ringing about an estimated 12-15 in an eight hour shift for non-urgent needs. Upon interviewing many staff, I found there was an inconsistent way and lack of education on how hourly rounds should be performed. Basic patient needs were not addressed within each visit from a staff member. By using a process map, timeline, PDSA, Lewin's Change Theory, SWOT analysis and graphs, I can show you my process of improving staff responsiveness within the nursing unit. In conclusion, as a Clinical Nurse Leader and patient advocate, I have utilized the information gained from the microsystem data and implementation of hourly rounding, thereby proving how to improve quality, safety and patient care experiences within the busy, medical surgical nursing unit with the evidence based practice of hourly rounding.

Book excerpt: The purpose of this practice improvement project was to determine if a relationship exists between purposeful hourly rounding and incidence of patient falls and patient satisfaction with information received from nurses for male and female patients over 18 years in one 60-bed acute rehabilitation setting. The first question that guided this project was: Are patient falls in an acute rehabilitation setting reduced when purposeful hourly rounding occurs? The second question was: Do patient satisfaction scores improve when purposeful hourly rounding was implemented in acute rehabilitation setting? This project implemented purposeful hourly rounding by nursing staff over a three-week intervention period in one rehabilitation hospital with an average daily census of 40 male and female patients. Employee education occurred in February, 2017. Fall rates were gathered from the hospital's internal incident reporting system. Patient satisfaction scores were obtained from Press Ganey surveys. The patient satisfaction results and the fall rates of 39 patients who did not experience purposeful hourly rounding were compared to the patient satisfaction and fall rates of 68 patients who experienced the purposeful hourly rounding. This project used a quasi-experimental correlational design. An independent t-test analysis did not demonstrate a difference between the patient satisfaction scores on 10 surveys received from the first group of patients and the 5 surveys received from the patients who experienced rounding. A chi-square analysis of the fall rates did not demonstrate a statistical difference between the groups. Although the results were not statistically significant, the project provided a baseline for future projects such as identifying barriers for staff to perform purposeful hourly rounding and improving the preparation for staff to perform hourly rounding.

Book excerpt: Effective implementation of nursing staff proactive rounding, a structured method of consistent proactive inquiry and responsiveness to patient needs, has been linked to improved patient satisfaction. Barriers and challenges to proactive rounding processes must be overcome for successful and effective implementation and sustainment to take place. The purpose of this evidence-based quality improvement practice project was to pilot implementation of structured proactive nursing staff hourly rounding and effectively integrate it into current practice to improve patient satisfaction. The project utilized a quasi-experimental nonequivalent group design on a 39-bed medical surgical/telemetry unit to compare top box Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) patient satisfaction scores before and after implementation of a structured proactive rounding process. A total of 59 nursing staff participants took part in the educational sessions. This included clinical care supervisors, RNs, and patient care assistants. Two-sample t-tests performed on HCAHPS top box composites performed on August 2015-August 2016 data (including project month), revealed that the t-statistics were not significant at the .00625 Bonferroni corrected critical alpha level. The differences in top box scores were not significant. A clinically significant increase was noted in the domain “Response of Hospital Staff” from July to August 2016, increasing from 51.7 to 58.9 percent although less than the score reported from August 2015 (61.5 percent). Clinically significant increases in HCAHPS composite top box scores during the three-week timespan of returned surveys were noted in all but three top box scores: Care Transitions reduced from 35.6 to 16.7 percent, Responsiveness of Hospital Staff scores fluctuated between 50 to 100 percent, and Hospital Environment fluctuated between 50 percent and 75 percent. Leader rounding patient satisfaction data revealed clinically significant percentage changes. One-sample t-tests between proportions were performed for each of the leader rounding questions results. The difference between yes responses and no or na/no response answers for each question both before and after the start of the project were significant at the Bonferroni corrected critical alpha level of .007. Two-sample t-tests between proportions were performed on each leader-rounding question to determine whether there was a significant difference between the yes responses for each question both before and after project implementation. The t-statistic was not significant at the Bonferroni corrected critical alpha level of .007. The difference between yes responses were not significant. Clinical significance could be observed through increased positive responses from before project implementation to after in the areas of pain control, staff rounding every hour, staff assisting timely, and whether patients perceived they were getting rest at night. Reductions in responses were noted in the areas of whether caregivers are explaining what they are doing and why, and an increase was noted in the number who had questions regarding their discharge. Communication, understanding, accountability, and engagement are core essentials that can be utilized in the development of processes that contribute to patient satisfaction, including proactive rounding. Process development, implementation, and evaluation are shared actions involving both nursing staff and leaders. Further research should assess sustainability of proactive rounding and competency of the practice through a long-term study utilizing multiple units or multiple sites.

Halm, RN, PhD, CNS-BC, director of nursing research and quality at Salem Hospital in Oregon, reviewed 11 hourly rounding studies and reported in a 2009 American Journal of Critical Care article that 83 percent of studies examined showed a reduction in call lights, 77 percent a decrease in fall rates and 88 percent an improvement in patient satisfaction' (Wood, 2014, para. 8). 'Swanson's Nursing Care Theory supports the purposed use of hourly rounding as it centers on practices that promote patient satisfaction and safety by focusing on the connection between caring processes and patient well-being and outcomes' (Tongues, Ray, 2011, pages374). Hourly rounding was introduced previously at this facility but the implementation was poorly received by nurses. In an effort to reintroduce hourly rounding, a team will be formed to champion the process, a presentation to the stakeholders will be done outlining the plan as well as the costs involved, and ongoing education will be provided for nursing as well as tools to aid the nurse in incorporating hourly rounding into their daily routines. Weekly meetings with a specific agenda will be held encouraging staff feedback on how the hourly rounding trial is proceeding. At the end of the trial period an assessment will be given to each nurse seeking their ideas and any new problems that need to be addressed. The team will evaluate data from the meetings, patient's comments and any occurrence reports concerning falls and pressure sores and call light usage information obtained from the facilities call light system. This information will be shared with the stake holders as well as nursing staff. As the nursing shortage increases, hospitals as well as nursing leaders are searching for ways to provide quality care, enhance patient safety and satisfaction and to increase nurse's satisfaction. 'Hourly patient rounding proposes to be one answer to both patient and nurse satisfaction' (Wood, 2014, para.6).

Rounding with the purpose of checking on patients every our around the clock. Studies have proven that by doing hourly rounds patient falls decrease, because their needs are being addressed and met in a timely manner. Research also has shown that rounding will allow for the nursing staff to set expectations and goals the patients' and family members as well as planning for their care. When nurses check on patients, their focus is to address the 4 Ps, positioning, potty (elimination needs), pain, and possessions (proximity of personal belongings). (Halm, 2009) At the time of admission, patients and their family members will be informed of the hourly rounding process in order to set realistic expectations of the plan of care. Research performed in 14 hospitals has found that by setting these goals, the use of call lights have decreased by 37%, patient falls decreased by 52%, and patient satisfaction scores increased by 12%. (Stanford Medicine, 2015).

Initial studies showed that hourly rounding not only improved patient satisfaction rates and decreased patient falls but also increased the amount of reimbursement for the hospital. In the last year Yakima Valley Memorial Hospital has experienced an increase in patient falls. Although new nurses are oriented to the policy and procedure of hourly rounding, there is no education on the reasoning of why this practice has been implemented or how it is beneficial not only to the nurse but also to the facility. Implementing Continued education into the bi-annual skills fair would help to not only provide meaning of evidence-based practice but will also help nursing staff to have a clear understanding of why it is part of their daily routine. This proposal will include an implementation plan and give a clear picture of the resources and tools needed to make this plan a success. Once this plan has been implemented for the time of one year there will be a re-evaluation on the comfort levels of the staff with hourly rounding and data will be collected on the number of documented falls that have or have not occurred during this time. The goal of this presentation is to improve patient safety and improve overall staff satisfaction.
