With European health systems facing a plethora of challenges that can be addressed through public health interventions there is renewed interest in strengthening public health services. Yet there are enormous gaps in our knowledge. How many people work in public health? How much money is spent on public health? What does it actually achieve? None of these questions can be answered easily. This volume brings together current knowledge on the organization and financing of public health services in Europe. It is based on country reports on the organization and financing of public health services in nine European countries and an in-depth analysis of the involvement of public health services in addressing three contemporary public health challenges (alcohol obesity and antimicrobial resistance). The focus is on four core dimensions of public health services: organization financing the public health workforce and quality assurance. The questions the volume seeks to answer are: o How are public health services in Europe organized? Are there good practices that can be emulated? What policy options are available? o How much is spent on public health services? Where do resources come from? And what was the impact of the economic crisis? o What do we know about the public health workforce? How can it be strengthened? o How is the quality of public health services being assured? What should quality assurance systems for public health services look like? This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. It accompanies two other Observatory publications: Organization and financing of public health services in Europe: country reports and The role of public health organizations in addressing public health problems in Europe: the case of obesity alcohol and antimicrobial resistance.

Book excerpt: Evidence indicates that actions within four main themes (early child development fair employment and decent work social protection and the living environment) are likely to have the greatest impact on the social determinants of health and health inequities. A systematic search and analysis of recommendations and policy guidelines from intergovernmental organizations and international bodies identified practical policy options for action on social determinants within these four themes. Policy options focused on early childhood education and care; child poverty; investment strategies for an inclusive economy; active labour market programmes; working conditions; social cash transfers; affordable housing; and planning and regulatory mechanisms to improve air quality and mitigate climate change. Applying combinations of these policy options alongside effective governance for health equity should enable WHO European Region Member States to reduce health inequities and synergize efforts to achieve the United Nations Sustainable Development Goals.

Available in PDF, EPUB and Kindle. Book excerpt: The novel coronavirus SARS-CoV-2, which causes the disease known as COVID-19, has infected people in 212 countries so far and on every continent except Antarctica. Vast changes to our home lives, social interactions, government functioning and relations between countries have swept the world in a few months and are difficult to hold in one’s mind at one time. That is why a collaborative effort such as this edited, multidisciplinary collection is needed. This book confronts the vulnerabilities and interconnectedness made visible by the pandemic and its consequences, along with the legal, ethical and policy responses. These include vulnerabilities for people who have been harmed or will be harmed by the virus directly and those harmed by measures taken to slow its relentless march; vulnerabilities exposed in our institutions, governance and legal structures; and vulnerabilities in other countries and at the global level where persistent injustices harm us all. Hopefully, COVID-19 will forces us to deeply reflect on how we govern and our policy priorities; to focus preparedness, precaution, and recovery to include all, not just some. Published in English with some chapters in French.

Book excerpt: Taking a ‘decentred’ approach to the analysis of health policy means being attentive to the historical contingencies and circumstances within which reforms are located, the influence of dominant or elite narratives in the shaping of policy, the local traditions and customary practices through which policies are mobilised, and the way local actors contest, negotiate and co-construct policy. This book offers a unique analysis of the changing landscape of healthcare reform in Britain, as an example of decentralized reforms across the developed world. The collection is framed by the recognition that healthcare reform has resulted in variegated and decentralized forms of governance. The chapters look at distinct aspects of reform within the British NHS to bring to light the influence of local histories, traditions, coalitions, and values, in the remaking of a national healthcare system. Each chapter focuses on a different aspects of reform, and in others developing cross-national and comparative analysis. However, each offers a unique contribution and analysis of contemporary theories of healthcare governance. This book will be of key interest to scholars, students and practitioners in healthcare, health and social policy, political science, and public management and governance.

Available in PDF, EPUB and Kindle. Book excerpt: This is the first book to offer a comprehensive guide to involving patients in health technology assessment (HTA). Defining patient involvement as patient participation in the HTA process and research into patient aspects, this book includes detailed explanations of approaches to participation and research, as well as case studies. Patient Involvement in HTA enables researchers, postgraduate students, HTA professionals and experts in the HTA community to study these complementary ways of taking account of patients’ knowledge, experiences, needs and preferences. Part I includes chapters discussing the ethical rationale, terminology, patient-based evidence, participation and patient input. Part II sets out methodology including: Qualitative Evidence Synthesis, Discrete Choice Experiments, Analytical Hierarchy Processes, Ethnographic Fieldwork, Deliberative Methods, Social Media Analysis, Patient-Reported Outcome Measures, patients as collaborative research partners and evaluation. Part III contains 15 case studies setting out current activities by HTA bodies on five continents, health technology developers and patient organisations. Each part includes discussion chapters from leading experts in patient involvement. A final chapter reflects on the need to clearly define the goals for patient involvement within the context of the HTA to identify the optimal approach. With cohesive contributions from more than 80 authors from a variety of disciplines around the globe, it is hoped this book will serve as a catalyst for collaboration to further develop patient involvement to improve HTA. "If you’re not involving patients, you're not doing HTA!" - Dr. Brian O’Rourke, President and CEO of CADTH, Chair of INAHTA

Available in PDF, EPUB and Kindle. Book excerpt: "Health 2020 is the new health policy framework of the WHO European Region. The policy aims at significantly improving the health and well-being of populations, reducing health inequalities, strengthening public health and ensuring sustainable people-centred health systems. Health 2020 is for the whole of government and the whole of society. It envisages actions and outcomes well beyond the boundaries of the health sector and beyond the remit of the ministry of health. Health 2020 therefore proposes reaching out and working together with other ministries, departments, sectors, organizations, stakeholders and civil society organizations. Health 2020 also proposes reaching out to, and working together with, citizens, patients and consumers, providing more opportunities for empowerment. Progress towards all these goals will be achieved by policy action in four areas: investing in health through a life-course approach and empowering citizens; tackling Europe's major disease burdens of noncommunicable and communicable diseases; strengthening people-centred health systems and public health capacities, including preparedness and response capacity for dealing with emergencies; and creating supportive environments and resilient communities. This volume presents in a practical way an analysis of how to reach out and work together. It focuses and gives clear advice on intersectoral governance structures that can facilitate intersectoral action. It is hoped that the volume will help WHO Member States assess and revise their practices of intersectoral collaboration, and inspire new ways of reaching out and working together. We also hope that it will encourage the exchange of good practices between countries. The book was developed in parallel with Health 2020 and it has continuously informed the technical consultations with Member States and experts that have taken place over the last two years. It comes as part of a package of studies that has provided scientific background to the development of Health 2020, including a study on governance for health in the 21st century and a review of social determinants and the health divide, both of which also provide examples on how to implement whole-of-society and whole-of-government approaches."--P. [ix].
