Book excerpt: Introduction : la prévention des chutes, dont l'incidence chaque année en France chez les plus de 80 ans dépasse les 50%, est un enjeu majeur de santé publique. Une chute peut avoir des conséquences désastreuses chez des patients fragiles, comme le sont la plupart des patients hospitalisés en gériatrie aiguë. Les troubles de la marche, présents chez plus d'un tiers des patients de plus de 70 ans, sont des facteurs prédisposant et précipitant de chute. Ils sont accessibles à la rééducation, et peuvent être analysés via des outils recueillant les paramètres spatio-temporels (PST) de la marche, comme le tapis de marche GAITrite®. Objectif : l'objectif de notre étude était de rechercher des différences entre les paramètres spatio-temporels de la marche chez les patients chuteurs par rapport à des patients non-chuteurs dans une population de patients hospitalisés en gériatrie aiguë. Matériel et méthodes : dans cette étude prospective, observationnelle, mono centrique, des patients âgés de plus de 75 ans, hospitalisés en gériatrie aiguë à l'hôpital de la Robertsau ont été inclus de juin 2019 à février 2020. Pour chaque patient, les paramètres spatio-temporels de la marche ont été recueillis en simple tâche puis en double tâche grâce au tapis GAITrite®. Les patients étaient répartis en deux groupes selon la présence ou non d'antécédent de chute. Les paramètres spatio-temporels et les caractéristiques démographiques ont été comparés entre les deux groupes et par rapport à la population générale. Des coefficients de variabilité ont également été calculés pour certains paramètres spatio-temporels de la marche, puis comparés entre les deux groupes. Résultats : 67 patients, âgés en moyenne de 85,9±6 ans, ont été inclus. Soixante-dix pourcents des patients avaient des troubles cognitifs et 66% nécessitaient une aide technique à la marche. Les patients étaient poly-morbides (score de Charlson en moyenne à 7.00 (± 1.87)) et poly-médiqués (7,04±3,25 médicaments par patients). Il y avait significativement plus d'antécédent d'AVC (17 vs 2, p=0,023), d'HTO (12 vs 0, p=0,013) et d'ostéoporose (15 vs 2, p=0,044) dans le groupe de patients chuteurs. La vitesse de marche moyenne était de 51,4±19,9 cm/s chez les non chuteurs, et de 47,3±17,7 cm/s chez les chuteurs (p=0,539), témoignant d'une marche pathologique en comparaison à la population générale du même âge (moyenne 100 cm/s). Il n'a pas été retrouvé de différence statistiquement significative pour tous les paramètres spatio-temporels analysés, et la variabilité des paramètres spatio-temporels était comparable dans les deux groupes. Conclusion : dans notre population, aucune différence statistiquement significative de paramètres spatio-temporels n'a été mise en évidence entre chuteurs et non chuteurs, probablement lié au faible effectif de notre étude, et à de nombreux facteurs confondant tels que la pathogénicité de la marche de nos patients et leurs caractéristiques démographiques. Une étude avec un nombre de patients plus important est nécessaire afin de trouver des modifications de paramètres spatio-temporels statistiquement liées au risque de chute, dans le but de mettre en place des protocoles de rééducation spécifiques.
