Available in PDF, EPUB and Kindle. Book excerpt: In this age of specialization most patients with diseases of the hind gut and anus are still managed by general physicians or surgeons. The speciality of coloproctology has grown from the art of 'anology', a study of conditions limited to that distance from the anal verge that could be inspected easily by torch- or candlelight or with the aid of a simple speculum. Two centuries ago many proctological ills were often treated by itinerant quacks, partly because the physician considered himself rather too grand to meddle around the anus and the medical profession in general tended to look down on those who studied anal disease. Today, in certain countries, coloproctology has become a speciality every bit as exclusive as urology or orthopaedic surgery, with its own training programme and examinations, usually undertaken after the end of general surgical training. Such super-specialization has undeniable advantages with rapidly advancing technology and therapeutic possibilities. There is no doubt that for the patient suffering from a low rectal carcinoma or severe inflammatory bowel disease there are advantages in being treated by surgeons who are dealing with several cases in a year rather than by a general surgeon who sees such problems relatively rarely. Such specialized colorectal surgery units makes good sense medically and economically in large centres of population with good communications.

Available in PDF, EPUB and Kindle. Book excerpt: Coloproctology is a surgical specialty which dynamically changes every few years. There is a profusion of colorectal textbooks but specialty series on particularly complex topics as well as on the specialized management approach for trainees and training colorectal surgeons are actually few. The aim of this text is a superior quality colorectal book written by world experts targeted at senior surgical and colorectal trainees and young consultant coloproctologists in current areas of subspecialty expertise. The structure of the chapters is current and is based on what does not appear and is not addressed in current colorectal textbooks. This series has proven useful in areas already represented, including Neurosurgery, Vascular Surgery, Transplantation Surgery, etc. The text is aimed at being relatively didactic with an algorithm approach to specialized areas within coloproctology which could potentially be updated every 3 years or so with new topics to create a set for didactic training in colorectal surgery. It is anticipated that these texts will become valuable teaching textbooks and part of every coloproctologist’s armamentarium as well as appealing to all general surgeons and surgical trainees engaged in complex elective and emergency colorectal surgery.

It is also a paradigm for multimodal management, as the combination of surgery, chemotherapy and radiotherapy is often necessary to achieve the optimal outcome. Recently, international experts met in Heidelberg, Germany to discuss the latest developments in the management of rectal cancer, including the anatomic and pathologic basis, staging tools, surgical concepts including fast-track surgery and laparoscopic resection, functional outcome after surgery and the role of radio- and chemotherapy. This monograph summarizes this meeting and gives an extensive overview of the current concepts in management of rectal cancer.

Available in PDF, EPUB and Kindle. Book excerpt: The field of colorectal surgery has undergone tremendous developments in recent decades. Surgery has become less invasive, new perspectives and insights have improved perioperative management and novel outcome measures have been defined. At the same time, the limits of surgical technique and perioperative care are being challenged by more complex patients. In order to realize the immense potential for excellent outcomes in colorectal surgery, coordinated multifaceted care must be delivered within a context of limited resources. Accordingly, there is a pressing need to streamline processes and heighten clinical productivity. Excellent communication and collaboration are required, with constant attention to detail and precision within an environment of learning, research and innovation. This book discusses in depth the transdisciplinary integration of different components of care in colorectal surgery. Pertinent background information on the complexities of current management in colorectal surgery is first provided, followed by discussion of the role of design thinking in care integration. Subsequent chapters focus on a range of issues associated with surgical and perioperative care in patients undergoing colorectal surgery, with emphasis on how the multiple facets of care can be integrated through a transdisciplinary approach. Each chapter provides helpful take-home messages in bullet point form and numerous informative figures and tables are also included. The authors are surgeons, physicians, anesthetists, oncologists, nurses, and allied health professionals with extensive experience in the field.
