Book excerpt: One-liner: A set of policy recommendations to promote the development and maintenance of communities in which children with asthma can be swiftly diagnosed, effectively treated, and protected from exposure to harmful environmental factors. An estimated 5 million U.S. children have asthma. Too many of these children are unnecessarily impaired. Much of the money spent on asthma is for high-cost health care services to treat acute periods of illness. Many asthma attacks could be avoided--and much suffering prevented and many medical costs saved--if more children received good-quality, ongoing asthma care and if the 11 policy recommendations presented in this report were implemented in a oordinated fashion. A national call to action, the policy recommendations span public and private interests and compel integration of public health activities across local, state, and federal levels. This report summarizes the findings of an effort funded by the Robert Wood Johnson Foundation as part of the Pediatric Asthma Initiative, whose purpose is to address current gaps in national childhood asthma care. It is the first national initiative that simultaneously addresses treatment, policy, and financing issues for children with asthma at the patient, provider, and institutional levels. The purpose of RAND's effort was to:--identify a range of policy actions in both the public and private sectors that could improve childhood asthma outcomes nationwide--select a subset of policies to create a blueprint for national policy in this area--outline alternatives to implement these policies that build on prior efforts. The effort developed a comprehensive policy framework that maps the identified strategies to one overall policy objective: to promote the development and maintenance of asthma-friendly communities--communities in which children with asthma are swiftly diagnosed, receive appropriate and ongoing treatment, and are not exposed to environmental factors that exacerbate their condition. This report is intended as a working guide for coordinating the activities of both public and private organizations at the federal, state, and local community levels.

Available in PDF, EPUB and Kindle. Book excerpt: This comprehensive account of the biological basis and clinical management of childhood asthma and other wheezing disorders brings together all the aspects necessary for full understanding by the trainee or practising respiratory physician, paediatrician, or specialist nurse of therapist. The text reviews the nature of childhood asthma, its developmental and biological origins and its impact both for the individual and for society. In each section the scientific background is brought together with comprehensive clinical information, reviewed where possible with an evidence base in mind, providing the reader with informed guidance to best practice. Thoroughly revised and updated throughout, this second edition reflects the significant developments that have occurred in the field during recent years. Advances in medical genetics are discussed, new techniques for investigation of inflammatory processes and for the evaluation of the impact of childhood asthma are described fully, additional information on the aetiology of this condition is provided and the pharmacology section includes a description of the leukotriene receptor agonists, the first novel drug therapy to emerge for 20 years. An entirely new section devoted to 'asthma around the world' including personal perspectives from clinicians from Japan, Africa, Latin America, and elsewhere has also been added. The book continues to place childhood asthma in a wider social context, with sections on teachning and training for health professionals outside the hospital setting and the delivery of care in the community. It remains essential reading for all paediatricians and respiratory physicians in training and in practice, and will also provide invaluable reference material for other health care professionals caring for babies, children and adolescents with asthma and wheezing disorders.

In the last 10 years, substantial progress has been made in terms of understanding some of the mechanisms that drive severe asthma; there have also been concomitant advances in the recognition of specific molecular phenotypes. This ERS Monograph covers all aspects of severe asthma – epidemiology, diagnosis, mechanisms, treatment and management – but has a particular focus on recent understanding of mechanistic heterogeneity based on an analytic approach using various ‘omics platforms applied to clinically well-defined asthma cohorts. How these advances have led to improved management targets is also emphasised. This book brings together the clinical and scientific expertise of those from around the world who are collaborating to solve the problem of severe asthma.

Available in PDF, EPUB and Kindle. Book excerpt: The second edition of this highly acclaimed text has been extensively revised and greatly expanded to reflect the considerable advances made in our understanding of the mechanisms of asthma and rhinitis. Containing the contributions of 242 experts of international standing, presented in 133 chapters, Asthma and Rhinitis provides an up-to-date, authoritative reference for both the clinician and scientist. The global approach given in this book mirrors the universal approach to the understanding of allergic disease. The editors have carried out a thorough and radical revision of the content by adding 6 new sections and 44 new chapters. Most of this expansion is due to greatly increased coverage of the clinical aspects of asthma, with new sections on childhood asthma and on drug treatment (each drug class has its own chapter). Also, the expansion of research into the genetic basis of asthma has necessitated a whole new section on Genetics, comprising some six chapters. There are also new chapters on adult-onset asthma and the relationship of asthma to sinusitis. A new section on Asthma in Special Circumstances includes chapters on asthma in pregnancy, asthma and surgery, asthma in the elderly and asthma in the context of critical care. In bringing the Second Edition fully up to date, the book has inevitably increased in size, and is now presented in two volumes. The second edition of Asthma and Rhinitis will continue the tradition of its predecessor of providing an up-to-the-minute reference for all those involved in the management of, and research into, asthma and rhinitis.

Book excerpt: Brain disordersâ€"neurological, psychiatric, and developmentalâ€"now affect at least 250 million people in the developing world, and this number is expected to rise as life expectancy increases. Yet public and private health systems in developing countries have paid relatively little attention to brain disorders. The negative attitudes, prejudice, and stigma that often surround many of these disorders have contributed to this neglect. Lacking proper diagnosis and treatment, millions of individual lives are lost to disability and death. Such conditions exact both personal and economic costs on families, communities, and nations. The report describes the causes and risk factors associated with brain disorders. It focuses on six representative brain disorders that are prevalent in developing countries: developmental disabilities, epilepsy, schizophrenia, bipolar disorder, depression, and stroke. The report makes detailed recommendations of ways to reduce the toll exacted by these six disorders. In broader strokes, the report also proposes six major strategies toward reducing the overall burden of brain disorders in the developing world.

Book excerpt: The Institute of Medicine's (IOM's) Roundtable on Research and Development of Drugs, Biologics, and Medical Devices evolved from the Forum on Drug Development, which was established in 1986. Sponsor representatives and IOM determined the importance of maintaining a neutral setting for discussions regarding long-term and politically sensitive issues justified the need to revise and enhance past efforts. The new Roundtable is intended to be a mechanism by which a broad group of experts from the public* and private sectors can be convened to conduct a dialogue and exchange information related to the development of drugs, biologics, and medical devices. Members have expertise in clinical medicine, pediatrics, clinical pharmacology, health policy, health insurance, industrial management, and product development; and they represent interests that address all facets of public policy issues. From time to time, the Roundtable requests that a workshop be conducted for the purpose of exploring a specific topic in detail and obtaining the views of additional experts. The first workshop for the Roundtable was held on April 14 and 15, 1998, and was entitled Assuring Data Quality and Validity in Clinical Trials for Regulatory Decision Making. The summary on that workshop is available from IOM. This workshop summary covers the second workshop, which was held on May 24 and 25, 1999, and which was aimed at facilitating the development and proper use of drugs, biologics, and medical devices for infants and children. It explores the scientific underpinnings and clinical needs, as well as the regulatory, legal, and ethical issues, raised by this area of research and development.

The last comprehensive review of this evidence by the Department of Health and Human Services (DHHS) was in the 1986 Surgeon General's report, The Health Consequences of Involuntary Smoking, published 20 years ago this year. This new report updates the evidence of the harmful effects of involuntary exposure to tobacco smoke. This large body of research findings is captured in an accompanying dynamic database that profiles key epidemiologic findings, and allows the evidence on health effects of exposure to tobacco smoke to be synthesized and updated (following the format of the 2004 report, The Health Consequences of Smoking). The database enables users to explore the data and studies supporting the conclusions in the report. The database is available on the Web site of the Centers for Disease Control and Prevention (CDC) at http://www.cdc.gov/tobacco.

Book excerpt: This book provides readers with a comprehensive review of severe asthma in children and adolescents, covering epidemiology, genetics, risk factors, co-morbidities, clinical presentation, diagnosis and treatment. Written by leaders in the field, chapter discussions draw on the mechanisms driving the disease, genetics, multidisciplinary approaches, immunomodulators, and other important aspects in the management of the disease. Severe Asthma in Children and Adolescents: Mechanisms and Management is designed to be a practical guide, opening with an introduction on the epidemiology of severe childhood asthma, as well as a discussion of special considerations of the disease unique to the preschool-aged patient. The distinctions between the diagnosis and management of asthma in young children of various ages, and specific approaches for the adolescent with severe asthma, including the transition into adult care are then addressed. Finally, the book closes with a discussion on the current state and future avenues for severe asthma research. Severe Asthma in Children and Adolescents is an indispensable reference for the healthcare professional, for basic and translational researchers, as well as for students, residents and fellows.
