Foot drop is caused by a deficiency in the ankle joint in which results in the weakness of ankle and toe dorsiflexion. This research proposes a new approach to design and performance evaluation of AFO fabrication that utilizes modeling, simulation and analysis software (like Open Sim, Ansys Work bench & Solid work) and additive manufacturing technologies to customize the fit form to an individual. By implementing a carbon fibre strut at ankle joint the design will result in a stronger, more comfortable, more flexible AFO that can adaptively constrain ankle movement for various different activities. GAIT Analysis was performed on six patients having foot drop, from the emotion analysis system in the GAIT lab. The data from the GAIT lab was then imported into the Open Sim to create a biomechanics simulation to extract the joint angle and joint moments. These value of ankle angle and moment with & without AFO was compared with normal value. The results show a considerable effect of the use of the AFO by drop foot patients in terms of ankle angle and ankle moments. Furthermore, the muscular analysis in terms of activation and forces also suggest that the dorsiflexors are assisted by the AFO during GAIT and the activation of the plantarflexion are considerably less during the lifting of the foot of the ground.

The research was focused on the structure redesigning, gait cycle analysis and Rapid Prototyping-Ankle Foot Orthosis testbed design. It is tried to provide a. new assembling AFO design, so that lower cost along with a standard process are achieved in order to build a customized AFO. The formal AFO structure was an integrated design. The integrated design was separated into three parts, a. customized base, a. function bar and a standard cuff. The customized base was built by rapid-prototyping method. The surface of the patient's ankle was modeled by the 3D scanner and then built by the 3D printer. The function bar worked as a spring. When the patient is walking, the bar could store the energy and release it. In this process, the weight of the patient is transferred to the spring's potential energy and then the energy is transferred as an assistive force. During designing the structure, it has been tried to provide a range of materials and sizes to meet the different needs of patients who need different assistant levels. The Finite Element Analysis (FEA) was completed on several regular materials. There was no way to formulate or calculate the shape of the AFO, since a customized AFO should be based on the certain patient. As a result, the RP-method was only for assistance, and hence, this work focused on providing the guidance for designing an eligible AFO for a certain patient at a certain status. The standard cuff has a range of sizes, and it could be adjusted by the patients at any time. The testbed for the AFO was based on the gait cycle analysis. For the testbed, an ankle foot simulation system was built. Using this testbed, the AFO can continue working for several hours, so that fatigue results for the proposed AFO can be obtained. The underlying principle here is to test the AFO design rather than simulating the human ankle foot. Hence, some necessary simplification was applied. For this, some necessary degrees of freedom were retained while the rest were eliminated. Three concept designs were compared and then one final design was built. A simple PID controller was designed and implemented as the control system for the testbed. The test result demonstrated the feasibility of the design and applicability of the testbed.

However, AFOs are prescribed and customized to individual patient in a variety of designs by healthcare professionals because there is a lack of strong level of evidence-based studies. In addition, the influence of AFOs' mechanical properties on functional deficit or clinical impairment is still not well understood. Therefore, this study aims to evaluate the biomechanical effects of commonly used AFOs on the lower extremities during the gait cycle. A healthy subject volunteered to be tested walking firstly without an AFO as a control and then wearing a solid carbon fibre reinforced, a solid and two posterior leaf spring AFOs. Strain gauges were attached to each AFO at ankle level to allow the measurement of the moment generated by the orthosis in the sagittal plane. The subject, wearing each instrumented AFO walked in the gait analysis laboratory where kinematic and kinetic data were acquired. This method allows the contribution of the AFOs during walking to be determined, and this information in conjunction with the kinematic and kinetic data, the anatomical moments, which are the moments generated by the muscles and ligaments can be calculated. The results imply that an AFO that is stiffer than the optimal requirement could lead to restriction of ankle motion during the stance phase and have an indirect effect on the knee and hip joints. The findings suggest that the stiffness of AFO, applied in stroke patients should be tailored base on the clinical impairment to get the optimum benefit. It is planned to continue this investigation on stroke patients.
