Book excerpt: La prescription sous-optimale en EHPAD peut impacter l'autonomie et la qualité de vie des 600 000 personnes âgées qui y résident, et est responsable d'hospitalisations dites évitables. Ces établissements ont un potentiel avéré d'amélioration de la prescription. Le dispositif OPTIM-EHPAD propose une démarche d'optimisation des prises en charge, au travers d'une collaboration entre Médecin Traitant (MT) et Médecin Coordonnateur (MCO). L'objectif de notre étude était de comprendre les facteurs qui facilitent ou freinent son appropriation par les médecins traitants. Une étude qualitative par entretiens semi-dirigés a été menée auprès de 30 médecins généralistes de la région Auvergne-Rhône-Alpes, intervenant au sein d'EHPAD où l'intervention OPTIM-EHPAD a été proposée. L'intervention a été jugée bénéfique par les MT concernant l'amélioration des prescriptions, et la collaboration avec le MCO a permis un apport de compétence et une aide à la prise de décision partagée. La réalisation des concertations a cependant été hétérogène. Il n'a pas été possible d'évaluer la perception du support papier OPTIM-EHPAD, très peu de MT l'ayant utilisé de manière rigoureuse et répétée. Il semble que la finalité de l'intervention n'ait pas été saisie dans toutes ses dimensions ce qui a pu limiter son appropriation par les MT. Le bon déroulement de l'intervention est apparu dépendant de facteurs organisationnels, relationnels et motivationnels. Si l'intervention OPTIM-EHPAD est pertinente en théorie, il apparait nécessaire de promouvoir l'investissement et l'accueil des MT dans les EHPAD, et de leur proposer une formation structurée à sa réalisation.
