Book excerpt: Dental amalgam and its substitutes are regulated under Council Directive 93/42/EEC concerning medical devices, according to which they must comply with the essential requirements laid out in the directive, in particular in relation to the health and safety of patients. Dental amalgam has been used for over 150 years for the treatment of dental cavities and is still used, in particular, for the treatment of large cavities due to its excellent mechanical properties and durability. Dental amalgam is a combination of alloy particles and mercury and contains about 50% of mercury in the elemental form. Overall, the use of alternative materials such as composite resins, glass ionomer cements, ceramics and gold alloys, is increasing, either due to their aesthetic properties or alleged health concerns in relation to the use of dental amalgam. On 28 January 2005, the Commission adopted the Communication to the Council and the European Parliament on a Community Strategy Concerning Mercury.^The Strategy addresses most aspects of the mercury life cycle. Its key aim is to reduce mercury levels both in relation to human exposure and the environment. It identifies twenty priority actions to be undertaken, both within the EU and internationally. The Strategy was welcomed by Council Conclusions on 24 June 2005 as well as by a European Parliament Resolution on 14 March 2006. Pursuant to Action 6 of the Strategy, the use of dental amalgam should be evaluated with a view to considering whether additional regulatory measures are appropriate. The Commission services consulted two Scientific Committees on the use of dental amalgam, the Committee for Environmental and Health Risks (SCHER) and the Committee for Emerging and Newly Identified Health Risks (SCENIHR). The opinions of both Committees were not conclusive regarding the appropriateness of additional regulatory measures to restrict the use of dental amalgam.^Concerning the environmental aspects, the SCHER opinion concluded that on the basis of the information available, it was not possible to "comprehensively assess the environmental risks and indirect health effects from use of dental amalgam in the Member States of the EU 25/27", and identified a number of gaps that need to be addressed.

This updated 2015 Opinion evaluates the scientific evidence on the potential association between amalgam and possible alternatives and adverse health effects, such as allergies and neurological disorders. The SCENIHR recognises that dental amalgam is an effective restorative material and is a material of choice for specific restorations. Currently in the EU, there is a shift away from the use of dental amalgam in oral health care towards an increased use of alternative materials. Because dental amalgam is neither toothcoloured nor adhesive to remaining tooth tissues, alternative tooth-coloured filling materials have become increasingly used. The change is indicated by trends in education on dental treatment towards an increased use of alternative materials instead of amalgam. This reduction is in line with concern about the use of mercury, the metallic element used in dental amalgam and the general aim to reduce mercury use within the European Union. The exposure of the general population to mercury is mainly due to fish consumption (organic mercury, methyl mercury) and dental amalgam (elemental mercury, inorganic mercury). The present Opinion reviews only the toxicology of elemental and inorganic mercury being relevant to amalgam safety considerations.

Book excerpt: In this review, we have examined the scientific literature related to the benefits and possible adverse effects of dental amalgam and of alternative restorative materials. The information available is inadequate to determine whether dental amalgam poses any significant risk to the health of the population (except for relatively rare cases of allergy or hypersensitivity to amalgam. If low-level mercury does have adverse effects, they are most likely subtle changes in central nervous system functioning, which might cause small deficits in short-term memory, for example. Other health effects are, given current evidence, very unlikely to result from amalgam fillings. Any potential benefit of removing intact amalgams and replacing them with other materials is very uncertain. Health Canada's recommendations to reduce the use of amalgam in the primary teeth of children, in pregnant women, and in individuals with kidney disease are consistent with the uncertain evidence regarding possible effects of dental amalgam.(Extract)

Toxic Effects of Mercury intends to facilitate among its readers the understanding of the importance of mercury pollution in the environment and the health consequences associated with exposure to this metal. The knowledge on methylmercury (MeHg) toxicity collected over the years is undoubtedly robust creating an impression all that is to be learnt about this metal has already been accomplished. However, in large measure, past knowledge has merely laid the ground for interesting questions that have yet to be fully addressed and concepts have yet to be deciphered. One of my major goals was to make a valiant attempt to include state-of-the-art information on the mechanisms of mercury toxicity, describing its effects on cultured cellular systems as well as in whole living organisms, starting from the lessons learned from the tragic events in Minamata Bay, Japan. A special focus of the book is on the neurotoxic effects of MeHg. An understanding at the cellular level is necessary to gather information on the structural and functional alterations induced by MeHg and how they possibly become unmasked and evident at the behavioral level, 32 chapters of the book have been organised having these considerations in mind. This book will provide state-of-the-art information to the graduate students training in toxicology, risk assessors, researchers and medical providers at large. It is aimed to bring the readers updated information on contemporary issues associated with exposure to methylmercury, from its effects on stem cells and neurons to population studies. It is a valuable resource for individuals interested in the public health effects and regulation of mercury. The report provides an excellent example of the implications of decisions in the risk assessment process for a larger audience and is written with the hope that the information will provide better understanding of the mercury problems which confront us.

Numerous new illnesses have cropped up from unknown causes, among which environmental exposure to toxics seems to be a most likely suspect. As these investigations continue you can expect to find that many of the most common dental materials will no longer be considered appropriate for use due to their potentially toxic nature. Modern technology has focused on developing techniques for evaluating biocompatibility that look for minimal damage rather than gross disease. But in the telling words of astronomer Carl Sagan, "Absence of evidence is not evidence of absence." In this paper I will review the state of the research on the patient's exposure to mercury from dental fillings, the occupational hazards of dentistry, the environmental impact, and how best to protect the dentist, the patients, and the dental staff from injury. This subject has been a source of controversy for over a century. The reason the argument has lasted so long is because investigators did not rely upon the documented scientific literature. Through the use of modern science we can dispel many of the common myths about dental amalgam.
