Book excerpt: Skin flap necrosis is a common postoperative complication after breast reconstruction, with an incidence of up to 43.4% among patients undergoing nipple-sparing mastectomy. Necrosis can adversely impact aesthetics due to the need to excise nonviable tissue, and increase the risks of infection, implant loss, nipple-areola complex sacrifice and malposition. Patient-specific factors including age, body mass index, and breast size may affect the risk of necrosis. Mastectomy and reconstruction techniques (i.e., choosing between skin- and nipple-sparing mastectomy, and between autologous and alloplastic reconstruction) may also influence necrosis rates. Intraoperative measures such as indocyanine green angiography and autologous skin banking, and the postoperative use of nitroglycerin paste for high-risk patients and warming blankets for autologous reconstruction are methods to help prevent and minimize the morbidity of skin necrosis. Herein, we share our institution’s approaches to predicting and mitigating skin necrosis, and methods of optimizing outcomes for breast reconstruction patients.

Rostagno, MD;Background: Digital mammography clearly distinguishes gland tissue density from the overlying nonglandular breast tissue coverage, which corresponds to the existing tissue between the skin and the superficial layer of the fascia superficialis surrounding the gland (i.e., dermis and subcutaneous fat). Preoperative digital imaging can determine the thickness of this breast tissue coverage, thus facilitating planning and reducing the rate of necrotic complications after direct to implant (DTI) reconstruction in nipple sparing mastectomy (NSM). Methods: Thirty NSMs in 22 patients with type 3 tissue coverage (subcutaneous tissue thickness of 2 cm or more) were selected for DTI reconstruction after NSM to evaluate immediate skin flap/nipple areola complex ischemic complications and patient satisfaction. Results: We experienced no wound healing problems or ischemic complications immediately after surgery in our population. Only 1 seroma was observed as a shortterm complication. Quality of life and patientsu2019 satisfaction level were optimal at 3 and 6 months follow-up, respectively. The aesthetic results have been evaluated as good/excellent in all cases. Conclusions: DTI immediate reconstruction with silicone implants following NSM appears to be a safe option in selected cases with enough tissue coverage, also providing a high level of patient satisfaction. The possibility of selecting cases for this procedure according to the preoperative digital mammogram showing more than 2 cm of superficial tissues thickness may help reducing the risk of immediate ischemic complications.

The primary goal is to achieve an optimal cosmetic result with long time local tumor control. Some (non – randomized) studies available in patients following breast conservation oncoplastic surgery showed tumor resection to be associated with wider free margins, less patients needing re-operation surgery for involved or close margins, a widening of the indications for breast conser- tion surgery, a low complication rate and at least an equal local recurrence rate. There is no doubt that oncoplastic breast surgery experienced a r- id rise with more patients demanding this kind of surgery. However, a profound knowledge of the different oncoplastic techniques is - sential for the outcome.

Book excerpt: Introduction/Background: Even though breast conserving therapy has become the gold standard surgical treatment for breast cancer, in our institution 34.7% of the patients still undergo mastectomy. Nipple sparing mastectomy (NSM) is becoming widely accepted, particularly in those patients that undergo prophylactic surgery with reconstruction, due to its superior cosmetic results. NSM can also be used in highly selected cancer patients as an effective procedure if it does not compromise oncologic safety.Methodology: Literature review of the nipple areola complex (NAC) sparing mastectomy on prophylactic and breast cancer management. Additionally we evaluate the impact it would have in our referral centre, selecting patients, among those who underwent a mastectomy between 2015-2018, according to the inclusion criteria of the National Comprehensive Cancer Network (NCCN) 2017 Guidelines.Results: There are no randomized trials of NSM. Long-term follow-up in clinical series is limited and most of the studiesu2019 samples are heterogeneous. The available data shows an overall survival of over 90% and local disease recurrence of 3-4.6%. 2017 NCCN Guidelines suggest the use of NSM in patients with early-stage tumours, biologically favourable carcinomas and infiltrating ductal carcinoma (IDC) or ductal carcinoma in situ (DCIS) that are >2cm from the nipple. Despite total removal of glandular breast tissue and the low rate of recurrence and/or nipple involvement expected, assessment of nipple free margins should be mandatory to reduce the risk of occult NAC involvement. In our Institution (H.U. 12 de Octubre, Madrid) 178 mastectomies were performed for breast cancer treatment and 18 for risk reduction between 2015-2018. According to the inclusion criteria of the NCCN Guidelines, 57 patients (29%) could have undergone a NSM (27 patients with IDC, 12 with DCIS and 18 BRCA mutation carriers). Conclusion: NSM has become a subject of increased attention. Itu2019s a safe option of treatment in patients with early-stage tumours and biologically favourable carcinomas. Disclosures: Prospective trials, with long-term follow up and homogenous samples are needed to establish the inclusion criteria and management of this patients: pathologic factors (tumour stage, molecular subtypesu2026), preoperative imaging study, safety distance of the tumour to the nipple, surgical technique, intraoperative study of the NAC (frozen section examination), and role of radiotherapy.
