Vila Socó, San Juan Ixhuatepec, Bhopal, Saveso, Flixbourough são nomes que nos fazem lembrar algumas catástrofes envolvendo substâncias químicas. A análise de riscos tem contribuído substancialmente para a prevenção de acidentes químicos maiores e seu desenvolvimento tem gerado inúmeros benefícios em diferentes campos. Apesar de todos os benefícios e contribuições gerados pelo desenvolvimento da análise de risco, principalmente nas áreas física, química e biológica, é necessário conhecer os limites e as incertezas dos seus métodos. A contribuição dos fatores sociais é essencial para o desenvolvimento desses métodos, e essa é uma área carente em pesquisas. Apesar de alguns trabalhos desenvolvidos nas ciências sociais, as pesquisas neste campo ainda estão engatinhando quando comparadas com as pesquisas nas áreas tecnológicas. Para avançar no desenvolvimento de uma abordagem interdisciplinar capaz de produzir uma prevenção eficaz dos riscos de acidentes maiores é necessário estimular o diálogo e a integração das várias disciplinas que compõem o universo das análises de risco, visto que nenhuma delas sozinha é capaz de satisfazer os requisitos de análise.

Não seguir práticas e padrões de engenharia reconhecidos e geralmente aceitos (Recognized and Generally Accepted Good Engineering Practices - RAGAGEP); 3) não incorporar novos conhecimentos por meio de lições aprendidas de outros acidentes. Para cada meio de perda de tecnologia, o autor se refere a acidentes ocorridos e que obtiveram grande repercussão mundial por envolver significativas perdas e faz referência com as formas de perda de conhecimento. Adicionalmente, propõe sugestões para manter o conhecimento atualizado e em caso de novas tecnologias o que fazer para evitar o aumento do risco de um acidente. Também, mostra gráfico que relaciona a perda do conhecimento tecnológico e o aumento do risco.
