They are particularly alarming in Saudi Arabia, where more than half of the adult population is overweight and one out of five people are obese. Additionally, the prevalence of all noncommunicable diseases (NCDs)--such as diabetes, cardiovascular diseases, and cancers--has increased substantially in recent decades, and today they are the leading cause of disability and death in the country. While the causes of overweight and obesity are complex and numerous, several innovative and multisectoral evidence-based interventions are emerging globally. Saudi Arabia has already embarked on the design and implementation of several such interventions and is committed to further expanding and scaling up such efforts in order to meet national goals and achieve results. This book builds on this work by laying out the remaining challenges while highlighting the opportunities lying ahead. The book provides new evidence and analysis on obesity in Saudi Arabia in order to support planning efforts on obesity prevention in the country. This includes examining the current prevalence of obesity and its risk factors, estimating the health and economic burden associated with obesity in the country, exploring the relationship between obesity and COVID-19, and identifying existing obesity prevention efforts and ways to enhance their impact based on the latest evidence. Finally, the book explores the use of a food system approach to connect human health and the environment, including through the production of a Saudi-specific Nutrient Profiling Model to help guide nutrition and obesity-related policies.

Book excerpt: Non- communicable diseases (NCDs) such as cardiovascular disease, diabetes, hype rtension, and obesity are dramatically increasing in prevalence worldwide, becom ing major causes of mortality in developed and developing countries. The shift o f dietary habits towards energy dense, high fat foods, and the simultaneous incr ease in sedentary lifestyles are associated with incidence, mortality, and risk factors for NCDs. This cross- sectional study determined the prevalence of physical inactivity and NCD risk factors (fasting serum total cholesterol, LDL- cholesterol, HDL- chole sterol, triglycerides (TG) and glucose; blood pressure (BP), body mass index (BM I) and waist cirumference) and their associations with physical activity (PA) an d dietary habits in a sample of 501 Lebanese men and women of low socio- economi c status. Overweight was prevalent in 56.4% of women and 73.9% of men, and obesi ty, in 23.6% of the sample surveyed. Hypercholesterolemia was found in 32.5% of women and 42.6% of m en, high LDL- C levels in 26.6% of women and 39.3% of men, low HDL- C levels in 47.2% of women and 51.9% of men, hypertriglyceridemia in 22.4% of women and 52.3 % of men, and diabetes in 5.2% of women and 12. 1% of men. Hypertension was prev alent in 11.3% of women and 23.3% of men. 52.9% of subjects were physically inac tive, and only 15.6% performed health- enhancing physical activity. Obesity mark ers were significantly associated with physical inactivity in men. Higher blood lipids were associated with lower PA and with obesity markers in men and women. Dietary habits were close to NCEP recommendations; consumption of omega- 3 fatty acids was associated with lower BP in both men and women; with favorable HDL- C levels in men and TG in women. PA was a stronger determinant of NCD risk factor s, particularly obesity markers. These findings indicate a high prevalence of physical inactivity and NCD risk fa ctors, suggesting further investigations on environmental and behavioral correla tes of PA and their relationships with dietary habits and NCD risk factors throu gh large- scale prospective studies. Better counseling by health care profession als, and nation- wide awareness campaigns constitute a good intervention strateg y.

The proportion of people who are overweight/obese is increasing, and the amount of physical activity is decreasing in most populations, including urban populations in many developing countries. The increasing prevalence of overweight/obesity is presumably due to the increasing availability of highly palatable, high-energy foods, and an increasing sedentary lifestyle due to mechanisation of both workplace and leisure activities. Overweight/obesity and reduced physical activity increases the risk of cancers in various organs. Maintaining a healthy body weight and regular physical activity is the second most important way to prevent cancer, after tobacco control. The suggestions of possible public health actions to tackle these risk factors include the promotion of balanced diets, which are not excessive in energy, and broad education and planning to enable and encourage physical activity during work and leisure. Recommendations and a full discussion of these topics are included in the sixth volume in this series of Handbooks.
