Book excerpt: "The relationship between the work environment and the level of burnout perceived by nurses in an acute care setting is the subject of the study. The descriptive method of research was used in this study. To gather primary data, the researcher administered the Maslach Burnout Inventory Survey (MBI-HSS) and the Work Environment Scale inventory survey questionnaires to thirteen nurses in the area. The perceptions of the respondents on dimensions in their work environment are positive with respect to involvement in their jobs, peer or coworker cohesion, supervisor support, autonomy, work pressure, clarity of rules and policies, and managerial control. Their perceptions are equivocal with respect to task orientation, innovation, and physical comfort. The respondents report a moderate degree of burnout feelings in terms of emotional exhaustion; they have not experienced burnout feelings in terms of depersonalization; and they report having moderate level of feelings of professional accomplishment. There is no significant difference in the perceptions of the respondents when grouped according to age, marital status, educational attainment, and years of experience as a registered nurse. The researcher recommends that the hospital administration should promote open communication, encourage manager and peer support, and allow successful participations of employees in planning and implementation of programs." -- From page v.

Role stress, which has three components; role ambiguity, role conflict and role overload, has been identified in new graduate nurses and has been positively correlated with burnout in the empirical literature. Empowerment has an inverse relationship with burnout. Support for these hypothesized relationships in newly graduated nurses has been demonstrated in research conducted in other countries. The purpose of this study was to examine the relationship among role stress, empowerment, and burnout in newly graduated nurses with two years of experience or less working in acute care hospitals. A descriptive correlational research design was utilized to examine the hypothesized relationships in a sample of 107 newly graduated nurses. Participants responded to questions online from various instruments which measured role stress, empowerment and burnout. All three role stress variables had significant positive correlations with burnout and empowerment had a significant inverse correlation with burnout. Regression analysis was used to test the hypotheses that empowerment would moderate the relationship between each of the role stress variables and burnout. In this study, 75% of new graduates reported burnout and the moderation model was not supported. Researchers have identified the presence of burnout in newly graduated nurses and the negative impact on the nurse, the patient, the organization and the profession. The findings from this study suggest that health care organizations need to examine and if necessary, improve their work environments. It may be less costly for hospitals to implement strategies to decrease burnout among new graduates such as establishment of an empowering work environment, elimination of role ambiguity, prevention of role conflict and reduction of role overload that are supported in the literature rather than absorb the costly financial impact of burnout.

Book excerpt: Background: Pediatric nurses care for some of the most vulnerable patients in our healthcare system and the nurses' well-being in the workplace is directly related to the quality of care provided. However, much of the organizational behaviour research conducted on nursing populations neglects to separately study nurses who care for children. In a resource limited system where health care provider well-being is recognized as a priority, it is important for administrators to know the environmental and attitudinal work factors that are the most influential to pediatric nurse work outcomes in order to target their optimization strategies. Purpose: The aim of this dissertation was to identify which modifiable work environment factors make the greatest contribution to the work outcome of burnout in pediatric nurses and provide nurses and organizational leaders with a targeted focus to optimize work outcomes and provider well-being in a resource limited system based on key stakeholder feedback. Method: This was achieved by conducting a scoping review and a two-phased mixed methods study set in a large quaternary care pediatric hospital in Toronto, Canada. The explanatory sequential mixed methods design consisted of a quantitative cross-sectional survey design followed by qualitative semi-structured interviews. Results: The results of this dissertation are represented through three published manuscripts; a scoping review; a cross-sectional survey design study; and a qualitative semi-structured interview study. We identified and measured the modifiable work environment features and work attitudes associated with burnout in pediatric critical care nurses; ranked the factors to determine their contribution to explaining burnout among pediatric critical care nurses; expanded our understanding of the pediatric critical care nurses' experience of burnout in relation to their work environment and work engagement ; and provided recommendations to nurses, nursing leadership, and organization administration on targeted, actionable improvements for nursing work outcomes based on key stakeholder feedback. Conclusion: A better understanding of the relationships between pediatric nurses' work environment, work attitudes, and work outcomes will allow administrators to provide targeted interventions to improve nurse well-being and, ultimately, pediatric patient care.

Book excerpt: Critical care nurses (CCN) work under stressful conditions that can lead to burnout (i.e., Emotional Exhaustion, Depersonalization, and low Personal Accomplishment; Maslach, Jackson, & Leiter, 1996). This study examined how everyday nonwork irritants (hassles) and daily nonwork events that are pleasant (uplifts) contributed to the level of burnout in CCNs. Data was collected from a sample of CCNs (n = 44) employed at a private hospital. CCNs consisted of intensive care nurses (n = 16) and emergency room nurses (n = 28) working either full-time or part-time. CCNs completed a demographic questionnaire, the Maslach Burnout Inventory (Maslach et al., 1996), and a modified version of the Daily Hassles Scale/Daily Uplifts Scale (Lazarus & Folkman, 1989). A multiple linear regression was calculated to predict each component of burnout (Emotional Exhaustion, Depersonalization, and low Personal Accomplishment) based on CCNs perceived daily hassles and uplifts. Daily hassles were found to be a significant predictor of all three burnout components. The results held even when work related hassles were removed from the scale. Book excerpt: This descriptive research used social network analysis to explore the influence of relationships and communication among hospital nursing (RN, LPN, CNA) and discharge planning staff on adherence to evidence-based practices (EBP) for reducing preventable hospital readmissions. Although previous studies have shown that nurses are a valued source of research information for each other, there have been few studies concerning the role that staff relationships and communication play in adherence to evidence-based practice. The investigator developed the Relational Model of Communication and Adherence to EBP from diffusion of innovation theory, social network theories, relational coordination theory, and quality improvement literature.The study sample consisted of 10 adult-medical surgical units, five home care agencies and six long-term care facilities. A total of 273 hospital nursing and discharge planning staff and 69 post-acute staff participated. Hospital staff completed a survey about communication patterns for patient care and patient discharge and about communication quality on the unit. Hospital and post-acute care staff completed surveys about relationship quality and demographic characteristics. Evidence-based practice adherence rates for risk assessment, medication reconciliation, and discharge summary were measured as documented in the electronic medical record. Social network analysis was used to analyze the communication patterns for patient care communication at the unit. These findings were correlated with (1) aggregate responses for communication quality, (2) aggregate responses for relationship quality, and (3) EBP adherence. Statistically significant relationships were found between communication patterns, and communication quality and relationship quality. There were two significant relationships between communication quality, and EBP adherence. Limitations in response rates and missing data prevented the analysis of all of the hypothesized relationships.The findings from this study provide empirical support for the role of social networks and relationships among staff in adoption of, and adherence to, EBP. Social network theory and social network analysis, especially the concept of knowledge sharing, provide ways to understand and leverage the influence of peer relationships. Future studies are needed to better understand the contribution that relationships among staff (social networks) have in the adoption of and adherence to EBP among nursing staff. Further model development and multilevel studies are needed.
