This book was released on 2003 with total page 124 pages. Available in PDF, EPUB and Kindle. Book excerpt: Caring as it applies to the profession of nursing is an often repeated concept. Nurse practitioners (NPs) or advanced practice nurses take the role of the nurse to a different level of care provision. The problem under study was how patients perceive NP caring behaviors that are expressed by NPs within the provision of their care. The conceptual framework used in this study was Jean Watson's theory of human care. The Caring Professional Scale (CPS), a 15-item, 5-point Likert scale questionnaire, was given to fifty people at each of two rural Idaho clinics. A total of 77 people responded by filling out the survey. Frequency statistics were used to determine means, standard deviations, percentages, and numbers reported. The CPS is divided into two subscales, Compassionate Healer and Competent Practitioner, and from results of responses to questions included in these two subscales, it was determined that ratings for Competent Practitioner were significantly higher (p=.008) than ratings for Compassionate Healer. It was also determined that NPs do provide a high level of care and that patients were satisfied with their care.

Available in PDF, EPUB and Kindle. Book excerpt: Since the inception of the nurse practitioner (NP) in the 1960's, health care has undergone multiple changes. Like most concepts or ideas, the NP has come, gone, and come again. Today, NPs are being utilized in a variety of settings with the main objective focusing on health promotion/prevention. With the addition of this healthcare provider, there will naturally be resistance as there is with most change. The purpose of this study was to determine the perceptions registered nurses (RNs) have of NPs and whether or not they understand and support the role. The Innovation-Diffusion Theory was used as a framework to interpret the results in determining acceptance versus resistance to change. A descriptive design was used to answer the question. A convenience sample was obtained from two local area hospitals. A researcher developed questionnaire was distributed to approximately 250 RNs with a return of 47.2% (118 questionnaires) in these facilities asking their perceptions of the nurse practitioner role. Demographic data revealed that although the majority have neither worked with a NP or utilized the services of a NP they do support the role and perceive it positively. These results suggest that this may be due to the current increase in the use of NPs in this area.
Book Synopsis Allocation of Health Care Resources at the Point of Care by : Mary Lind Crowe
Download or read book Allocation of Health Care Resources at the Point of Care written by Mary Lind Crowe and published by . This book was released on 2012 with total page 159 pages. Available in PDF, EPUB and Kindle. Book excerpt: Little is known about the influence of practice site resource scarcities on the clinical decision making of nurse practitioners. This descriptive study explored nurse practitioners' perceptions of practice site scarcities, the strategies used to allocate scarce resources, and the ethical difficulties experienced in doing so. The survey instrument was developed by Hurst et al. (2006). The theory of naturalistic decision making (Klein, 1989, 1993) and the Recognition Primed Decision model (2008) provided the conceptual framework. Nurse practitioners and nurse practitioner students practicing in community clinics in 22 Ohio counties completed the electronic survey (N=68, response rate 27.9%, mean age 44.9 years, mean number of years in NP practice 10.7 years, 90% of NP's held prescriptive privileges, 72% were clinic employees, 69% provided adult primary care services, 30% provided pediatric services, and 95% of the clinics provided indigent care). Practice site resource scarcities were reported by 41%. "Compromised decision making" and "care impacted by the inability to pay for services" were themes emerging from the examples of the ethical issues described by nurse practitioners in narratives. Pharmaceuticals (31%), mental health services (31%) and referrals to specialists (27%), were the services most frequently unavailable, most difficult to obtain, and most often not provided for patients. The most frequently used strategies to manage resource scarcities were substitution of another option (34%) and explanation of why the service could not be provided (44%). Poor efficacy of a therapy (85%), small marginal benefit (78%), short life expectancy (49%), and age > 85 years (47%) were the most frequently identified criteria for not providing services. This study found evidence of practice site resource scarcities resulting in ethical dilemmas for nurse practitioners providing primary care services in community clinics.
