The TSMO Compendium is a collection of cases from across the country where benefit-cost analysis (BCA) has been applied to a specific TSMO technology/strategy or group. These actual project evaluations demonstrate the use of custom spreadsheet analysis developed by the agency or its contractors or the application of available software tools to conduct the BCA. The Compendium also includes hypothetical cases designed to demonstrate how BCA can be used for a specific TSMO technology/strategy or group. FHWA has developed a sketch planning BCA tool, called the Tool for Operations Benefit-Cost Analysis (TOPS-BC), for application to TSMO projects. For the hypothetical cases TOPS-BC is used to assist in the measurement of benefits and costs and in the calculation of the benefit-cost ratio. Each case demonstrates how planners have or could in the future conduct a BCA on one or more TSMO technologies or strategies. There are over two dozen cases presented in the Compendium and they cover a wide range of TSMO technologies and strategies where each case addresses one or more specific issues or procedures.

Health policymakers, in particular, must be able to view the entire range of interventions, preventive as well as therapeutic, and to assess and compare health returns on the Nation's investment in these interventions. The problem, of course, is how to compare different kinds of returns on dissimilar interventions. In addressing this central question, the Comparative Benefits Modeling Project proceeded on four general assumptions: Quantitative methods can aid decision making related to allocation of resources. Cost-effectiveness analysis (cea) and its derivative, cost-utility analysis (cua), have significant potential for this purpose, if carefully conducted and presented in an appropriate (e. G., comparative) context. Many uncertainties are inherent in the conduct of most analyses of cost-effectiveness; dealing with these uncertainties could be facilitated through development of an appropriate model. Application of any such model on a regular basis and across a range of issues would lead to gradual improvement in methods and an improved perspective on the relative returns to society of various health programs. Accordingly, the Project set out to review the state of the art of cea and cua and to develop an appropriate model. To this end, it brought together an expert panel of economists, physicians, public health specialists, and others concerned with health care resource allocation. Although the panel found a high degree of controversy in this young field, it also found a basis for a unified framework through which cua can be applied to health interventions. Book excerpt: Choice is the name of the game. Government sets the size of the public budget and decides which public projects it will invest in and which transfers and regulations it will implement. To do this systematically the government must have a procedure that displays the consequences of the alternatives. This book is an exposition of benefit-cost analysis (BCA), an analytic framework for organizing thoughts, listing the pros and cons of alternatives, and determining values for all relevant factors so that the alternatives can be ranked. A major question illuminated by this text is whether the results of such an analysis can instruct government--in the sense of telling it what it must do to avoid being labelled stupid, corrupt, irrational, and/or inefficient. How and when, we will ask, can the benefit-cost analyst label a particular governmental investment, policy, or regulation as political (in the pejorative sense) as opposed to economic (in the laudatory sense of being economically justified)? This book will argue that BCA is much like a consumer information system. Consumer information neither tells consumers what to do nor tells them what they should want. However, it does tell them which products will perform in selected ways and at what costs. And this information, together with the independently arrived at wants, helps the consumer make intelligent choices.
