This publication concludes that the effort to train the ANDSF needs to continue, and provides recommendations for the SSA programs to be improved, based on lessons learned from careful analysis of real reconstruction situations in Afghanistan. The publication states that the United States was never prepared to help create Afghan police and military forces capable of protecting that country from internal and external threats. It is the hope of the Special Inspector General for Afghanistan Reconstruction (SIGAR), John F. Sopko, that this publication, and other SIGAR reports will create a body of work that can help provide reasonable solutions to help United States agencies and military forces improve reconstruction efforts in Afghanistan. As part of these efforts, the U.S. Army Security Assistance Command (USASAC) and the Navy spent about $120 million to procure small arms and light weapons for ANSF. International donors also provided weapons. GAO analyzed whether Defense can account for these weapons and ensure ANSF can safeguard and account for them. GAO reviewed Defense and State documents on accountability procedures, reviewed contractor reports on ANSF training, met with U.S. and Afghan officials, observed accountability practices, analyzed inventory records, and attempted to locate a random sample of weapons. To improve weapons accountability, GAO recommends the Secretary of Defense (1) establish clear accountability procedures, including serial number tracking and routine physical inventories, for weapons in U.S. control and custody; (2) direct CSTC-A to assess and verify each ANSF unit's capacity to safeguard and account for weapons; and (3) provide adequate resources to CSTC-A to train, mentor, and assess ANSF in equipment accountability matters. Defense concurred with these recommendations, but did not state when the shortcomings we identified would be addressed. State provided no comments.

During fiscal years 2002 through 2008, the United States spent approximately $16.5 billion to train and equip the Afghan army and police forces in order to transfer responsibility for the security of Afghanistan from the international community to the Afghan government. As part of this effort, Defense--through the U.S. Army and Navy--purchased over 242,000 small arms and light weapons, at a cost of about $120 million. These weapons include rifles, pistols, shotguns, machine guns, mortars, and launchers for grenades, rockets, and missiles. In addition, CSTC-A has reported that 21 other countries provided about 135,000 weapons for ANSF between June 2002 and June 2008, which they have valued at about $103 million. This brings the total number of weapons Defense reported obtaining for ANSF to over 375,000. The Combined Security Transition Command-Afghanistan (CSTC-A) in Kabul, which is a joint service, coalition organization under the command and control of Defense's U.S. Central Command is primarily responsible for training and equipping ANSF. 3 As part of that responsibility, CSTC-A receives and stores weapons provided by the United States and other international donors and distributes them to ANSF units. In addition, CSTC-A is responsible for monitoring the use of U.S.-procured weapons and other sensitive equipment.

The Department of Defense (Defense), through its Combined Security Transition Command-Afghanistan (CSTC-A), directs U.S. efforts to develop the Afghan National Army (ANA) and, in conjunction with the Department of State (State), the Afghan National Police (ANP). To follow up on recommendations from GAO's 2005 report on the ANSF, GAO analyzed the extent to which U.S. plans for the ANSF contain criteria they recommended. GAO also examined progress made and challenges faced in developing the ANA and ANP. To address these objectives, GAO reviewed Defense, State, and contractor documents and met with cognizant officials. GAO has prepared this report under the Comptroller General's authority to conduct evaluations on his own initiative. To ensure action on GAO's 2005 recommendation and a 2008 Defense Authorization Act mandate, Congress should consider conditioning a portion of future appropriations on completion of a coordinated, detailed plan for the ANSF, including a sustainment strategy. State expressed concerns about conditioning future funding, and Defense disagreed, stating that current guidance is sufficient. GAO maintains that a coordinated, detailed plan is essential to accountability of U.S. efforts to develop the ANSF.

And despite the best efforts of the U.S. to avoid them, civilian casualties in Afghanistan, Iraq, and Pakistan have been a regular feature of the United States' wars after 9/11. In Accountability for Killing, Neta C. Crawford focuses on the causes of these many episodes of foreseeable collateral damage and the moral responsibility for them. The dominant paradigm of legal and moral responsibility in war today stresses both intention and individual accountability. Deliberate killing of civilians is outlawed and international law blames individual soldiers and commanders for such killing. An individual soldier may be sentenced life in prison or death for deliberately killing even a small number of civilians, but the large scale killing of dozens or even hundreds of civilians may be forgiven if it was unintentional--"incidental"--to a military operation. The very law that protects noncombatants from deliberate killing may allow many episodes of unintended killing. Under international law, civilian killing may be forgiven if it was unintended and incidental to a militarily necessary operation. Given the nature of contemporary war, where military organizations-training, and the choice of weapons, doctrine, and tactics-create the conditions for systemic collateral damage, Crawford contends that placing moral responsibility for systemic collateral damage on individuals is misplaced. She develops a new theory of organizational moral agency and responsibility, and shows how the US military exercised moral agency and moral responsibility to reduce the incidence of collateral damage in America's most recent wars. Indeed, when the U.S. military and its allies saw that the perception of collateral damage killing was causing it to lose support in the war zones, it moved to a "population centric" doctrine, putting civilian protection at the heart of its strategy. Trenchant, original, and ranging across security studies, international law, ethics, and international relations, Accountability for Killing will reshape our understanding of the ethics of contemporary war.
