Book excerpt: Since the early 1980s, civil airmen have been allowed to correct refractive error (i.e., myopia, hyperopia, astigmatism) with corrective surgery. Prior Federal Aviation Administration research studies have shown that the number of civil airmen with refractive surgery continues to increase. A study that reviewed refractive surgery use in civil airmen for the years 1994-96, reported that the largest percentage had radial keratotomy (RK). A similar study that reported on the years 1996-2001, however, showed that there had been a substantial increase in the percentage of airmen with laser refractive surgery, i.e., photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK). A reference guide on refractive surgery was published in September of 1998 (DOT/FAA/AM-98/25); however, at that time long-term clinical data on PRK and LASIK were not available. The introduction of new refractive surgical techniques (e.g., laser epithelial keratomileusis [LASEK], laser thermal keratoplasty [LTK], conductive keratoplasty [CK], Intacs, phakic IOLs, and presbyopia surgeries) and technology (e.g., wavefront-guided systems, Femtosecond Lasers, inlays, and onlays) has further added to concerns regarding the use of refractive surgical procedures by aviators. In order to provide the aviation community with information to formulate administrative decisions and policies associated with existing and emerging refractive surgical procedures, this paper reviews current procedures and discusses their applicability in the civil aviation environment.

Visual performance under low light illumination, cockpit performance in military aviators, ability to meet accession and retention standards, and resistance to trauma are among some of the unique considerations of the active duty military. The current study comprises a comprehensive evaluation of safety, efficacy and visual performance after laser refractive surgery. Initial studies cover surgical procedures and pre- and post-operative assessment of visual performance, refractive error and stability, and ocular integrity. Subsequent phases address military operational factors, including occupation-specific visual requirements and effects of various operational environments. Additional militarily relevant investigations will address emerging technologies, both military and medical, as they apply to refractive surgery in the US service men and women. To date, we have enrolled and treated over 500 subjects in one of 16 approved protocols (9 ongoing and 7 completed). We have published 15 peer-reviewed papers, and presented 8 papers and 22 abstracts at national or international meetings. We have research collaborations with a number of DoD and civilian academic institutions including the US Army Night Vision Laboratory and Electronic Sensor Directorate, the US Army Aviation Research Laboratory, the Naval Medical Center San Diego, Stanford Research Institute, the US Army Center for Health Promotion and Preventative Medicine, the Schepens Eye Research Institute, Harvard Medical School, University of Virginia, James Madison University, and Eastern Virginia Medical School. Radial keratotomy (RK), the first widely accepted refractive surgical procedure, involves making radial incisions on the peripheral cornea. These incisions weaken the cornea and allow intraocular pressure to push the peripheral cornea out, flattening the apex and reducing refractive power. There are many disadvantages with RK that raise concerns regarding its use in the aviation environment. These include: progressive hyperopic shifts, reduced corneal strength, fluctuation of vision, glare, poor refractive predictability and altitude-induced corneal changes. In October 1995, the Food and Drug Administration approved the use of the excimer laser to perform photorefractive keratectomy (PRK) to reshape the anterior curvature of the cornea. Since that time, PRK has become the refractive surgical procedure of choice. It has been reported that for low to moderate levels of myopia there is greater predictability, no fluctuation of vision or reduction in corneal strength, and about 85% of patients have uncorrected visual acuity of 20/40 or better. As with RK, there are aspects of PRK that raise concerns about its use in the aviation environment. Some of these include: night vision problems (e.g., glare, halos around lights, haze, starbursts, and dim lighting difficulties), reduced contrast sensitivity, stability of refraction, reduced best-corrected visual acuity, and induced anisometropia. Using a mathematical model, it was estimated that by the year 2000 there may be over 1,200 civil airmen who elect to have PRK performed. To provide the aeromedical community with information to formulate administrative decisions and policies associated with this new refractive surgical procedure, this paper reviews the results of clinical trials on PRK and discusses its applicability in aviation.
